About this file
This document is a City of Miami Questionnaire for RFP No. 2130386 related to Group Benefit Dental Plan(s), seeking comprehensive proposals for dental insurance coverage. The questionnaire is designed to evaluate potential dental plan providers across multiple dimensions, including organizational experience, plan design, claims adjudication, network access, member services, COBRA administration, and performance guarantees. While completion is not mandatory, the city strongly encourages respondents to provide detailed answers, as incomplete responses may negatively impact the overall proposal evaluation score. The RFP appears to be targeting a January 1, 2027 effective date for the selected dental plan(s).
The questionnaire comprehensively explores both Dental Health Maintenance Organization (DHMO) and Preferred Provider Organization (PPO) plan structures, requesting detailed information on benefit designs, coverage limitations, network provider details, claims processing capabilities, customer service protocols, and pricing models. Key areas of focus include provider network composition, geo-access standards, member service capabilities (such as multilingual support and mobile app features), performance metrics, compliance with regulations like HIPAA and ADA, and potential performance guarantees. Respondents are expected to provide in-depth responses covering technical capabilities, service delivery mechanisms, and potential cost structures to enable the City of Miami to make an informed selection of its group dental benefits provider.
