About this file

This is a Quality Assurance Surveillance Plan (QASP) for monitoring contractor performance on Boiler Safety Device Testing Services provided to the VA Central California Health Care System in Fresno, California. The QASP establishes the framework for the Contracting Officer's Representative (COR) and Government personnel to systematically monitor and verify contractor compliance with the Performance Work Statement (PWS) throughout the contract period. The plan does not modify contract terms and conditions; in case of conflict with the PWS, the PWS controls.

The QASP defines ten performance standards with corresponding surveillance methods and 100% acceptable quality levels: monthly boiler safety device testing activities, annual boiler safety device testing and inspection cycles, completion of Government-provided testing forms and checklists, submission of complete inspection and testing reports with specified details, deficiency identification and reporting, operational coordination with VA Central Energy Plant personnel, OSHA and facility safety compliance, use of qualified personnel with required certifications and licenses, timely documentation submission, and safety protocol adherence. Surveillance methods include direct observation, document review, coordination verification, and follow-up inspections. The COR maintains comprehensive documentation including completed surveillance records, inspection reports, forms, checklists, identified deficiencies, corrective actions, and invoice validation records to support contractor performance evaluations, contract administration, and future acquisition planning. Performance deficiencies, such as failure to complete required testing, incomplete documentation, unauthorized outages, or use of unqualified personnel, shall be documented and reported to the Contracting Officer for appropriate action.
