About this file
This document is a Performance Work Statement (PWS) attachment that specifies the Government-provided testing forms and checklists required for boiler safety device testing activities under the contract.
The attachment outlines three categories of testing activities with associated checklists: Monthly Safety Checks (CLIN 0001) requiring completion of multiple checklists for three boilers covering low water alarms and cutoffs, high water alarms, and flame scanner devices; Semi-Annual Boiler Safety Device Testing Cycle (CLIN 0002) requiring annual testing checklists for igniter timing, main flame ignition timing, forced draft pre-purge proving switches, combustion air pressure switches, forced draft motor interlock switches, low atomizing media pressure switches, and automatic fuel oil shutoff valves on three boilers; and Annual Safety Device Testing and Inspection Support (CLIN 0003) requiring testing of emergency stop/panic buttons and steam safety valve accumulation tests at high fire on three boilers. The contractor must complete and submit all applicable Government-provided forms and checklists documenting services performed. The document notes that during contract performance, the Government may add, remove, update, or replace these forms and checklists without changing the equipment covered, testing frequency, scope of work, or required inspection and testing services. The Contracting Officer's Representative (COR) or designated VA representative will coordinate the inspection and testing schedule based on facility operations, VHA Directive 1810 requirements, and boiler plant availability.

ATTACHMENT B
Government-Provided Testing Forms/Checklists The following are the current Government-provided testing forms/checklists applicable to the required monthly boiler safety device testing activities and the assigned annual boiler safety device testing/inspection cycle. The contractor shall complete and submit all applicable Government-provided forms/checklists for the services performed under this contract.
The checklist groupings below identify the current forms/checklists anticipated for use at the time of solicitation. During contract performance, the Government may add, remove, update, or replace forms/checklists used to document the same inspection and testing services required under the PWS.
Any additions, removals, updates, or replacements of forms/checklists shall not change the equipment covered, testing frequency, required inspection/testing services, or scope of work.
The COR or designated VA representative will coordinate the schedule for the assigned annual boiler safety device testing/inspection cycle based on facility operations, VHA Directive 1810 requirements, and boiler plant availability.
