| A-003 | AFW-88 | 1000 | there is a cable hanging from the car near the door | 7/Aug/2020 | R | 30/Jul/2020 | N | The door had a cable hanging from it. I verified this and the cable was broken. Also found that the interlock assembly is broken. Will need to return and replace both. Unit out of service. Picked up parts and returned on 8/7/20 to install interlock assembly and cable. Tested unit and left in service | TC-22052 | ||
| A-004 | AFW-90 | 1000 | car is on the 1st deck and will not respond to hall or car calls. Car doors will open and close. | 30/Jul/2020 | R | 30/Jul/2020 | N | Unit toggle was turned off in the box. Found the safety circuit open. I put the switch back to normal position. Tested and returned to service. | TC-22053 | ||
| A-005 | AFW-78 | 1000 | car gate is not fully closing (very slow to close when it does) | 30/Jul/2020 | R | 30/Jul/2020 | N | Unit was running but the door had a problem with the limits causing it to act abnormally. Took information for the limit sensor that needs to be replaced. Unit in service. | TC-22054 | ||
| A-006 | AFW-88 | 1000 | 1st floor doors are not closing all the way. Car has been taken out of service via penthouse disconnect. | 18/Sep/2020 | R | 18/Sep/2020 | N | 4 hours | $0.00 | Found the car on the 1st floor with the doors approximately 2 inches from being fully closed and the power off in the motor room. I went to the motor room and visually checked the door circuit relays and they appeared to be good. I next physically moved the relays in the door circuit and they appeared to be in working order. I turned the power on and put in a car call and could see all the proper relays energize but the car did not move. I went to the car and the doors were still two inches apart from being fully closed. I separated the car and hall doors and could feel that the doors were hitting something at that point. I put the car in access and ran the car up to where I could work on it from inside the car. I found the interlock contacts on the stationary keeper side were being hit by the keeper at a point in the contact that pushed the contact backwards instead of down. I adjusted the contact so it would not come up so much and slightly tilted the keeper so it would go into the interlock box and make contact with the interlock contacts. I also sanded down the hall door hangar tracks and lightly lubricated all moving parts on the hall doors. I ran and tested the car with no issues and left car in service. | TC-23590 |
| A-007 | AFW-98 | 1000 | car is stuck on 2nd deck | 24/Sep/2020 | R | 25/Sep/2020 | N | 7 hours | $0.00 | Arrived at the site and found the elevator shutdown. Indicator showed the 2nd floor but the elevator was stuck between the 2nd and 3rd floors. I went directly to the computer to see the fault being displayed and found fault 1 active. Translated and it said that the elevator run timer expired. This means that the elevator was traveling to a floor but for some reason took too much time and that timer finished counting and that shutdown the elevator in between floor. When I read this I reset the controller and the microprocessor unit. I put unit in inspection and was running with the breaks picked, so the unit was extremely slow and won't move down. I verified and troubleshoot the Break circuit, finding a problem in the Break contactor(BK2) that was not picking and sending voltage to the coil of the brakes to perform the command of open. I tested and verified this contactor but was found in operation, I verified the respective electrical circuits that energizes this contactor, and found that the RELAY STOP WAS BAD. The relay would energize but one of the contacts (11,2) was not sending the voltage to the A1 and A2 break contactor coil to close it and then send voltage to the break coil. I replaced this RELAY, I had a spare in the machine room so I used this one. Instantly the brakes would open and the car would move normally. I tested a lot and verified the equipment and the break unit was now operational. Performing this test I could perceive a small sound coming from inside the motor, like a small cracking sound, but I did not see any harm in the motor and I did not see any visible of performance problems with the motor, I did not touched anything of this equipment, I will keep an eye in the preventative visits and if found something abnormal will notify. Tested the unit operation from inside in all floors with Dan, I tested and left in normal operation. | TC-23821 |
| A-008 | AFW-78 | 1000 | car is stuck on 3rd deck and doors will not work | 26/Oct/2020 | R | 26/Oct/2020 | N | Found car on third floor with doors open. Went to motor and had no power to the main line. I was told the circuit breaker had tripped once before, so before we rest it I checked out the controller, looking for wires that could be grounded. Only found a bunch of lantern flies in one of the resistors and they were cooked. Removed lantern flies and went to first floor to reset breaker after receiving permission to do so. Back to the car and still had no power. Went back to the motor room and found two main line fuses had blown, both 60 amps. I changed both and we were able to run the car with no issues. Left in service.- | TC-24914 | ||
| A-009 | AFW-77 | 1000 | car is stuck on 4th deck and will not respond to calls | 28/Oct/2020 | R | 28/Oct/2020 | N | Unit was shutdown on arrival. I checked the main line power but it did not have power to the top three terminals in the disconnect switch. The disconnect had power. I verified again and the fuses in the main line disconnect were blown. Cause unknown. Will need to order new fuses. Left out of service. | TC-25141 | ||
| A-010 | AFW-88 | 1000 | investigate hall door operation on each floor. Per the NAVFAC inspector, "the door pick-up rollers on AFW-88 need some TLC soon. Each floor is adjusted differently or has a different problem." | 18/Nov/2020 | R | N | 2 | During last maintenance I found the crank arm link bearing broken. I came today to replaced the whole link with new bearing. I tested and adjusted it. Tested in all floors numerous times. Tested and left back in service. | TC-28697 | ||
| A-011 | AFW-78 | 1000 | the elevator is stuck on the 3rd floor and will not move | 25/Nov/2020 | R | N | |||||
| A-012 | AFW-90 | 1000 | elevator is stuck on the 4th floor with the doors open and will not respond to calls or door close button | 10/Dec/2020 | R | 11/Dec/2020 | N | 4 | $0 | Arrived at the building and troubleshoot the unit and found the input 00 ZNR not active. This causes the unit doors to be disconnected. Went floor by floor to very all interlock switches. Found the first floor was not active, which I repaired and put elevator back in service. | TC-25815 |
| A-013 | AFW-77 | 1000 | passenger stuck in elevator. Navy Fire Department responded and removed passenger from elevator. Disconnect power in the machine room. | 14/Dec/2020 | R | 16/Dec/2020 | N | 4 | Unit found shutdown. Unit was found shutdown at the 4th floor in between 4 and 3rd floors. I opened the door and inspected the locks, gate, sensors and release cam for any problems. I found everything normal. I went up and found the disconnect switch off. I tested fuses for any blown one. Was normal. I turned the power on and tested the unit in all floors. 1,2,3,4, and from top to bottom numerous times to replicate for any issue. But I did not found any. This controller is an old relay unit, and when the power is turned on it releases the relays and if I had a issue this panel does not have the resources to keep a fault list of events or anything. I continued and verified the car gate door and found that the problem with the sheave is getting worse and had a lot of first from the sheave. I decided to leave the elevator shutdown because if this part breaks the whole car gate door will fall into any passenger, it can cause serious injury to passengers and for this reason is best to leave it shutdown until it is fixed. I will notify the office for this repair. Left the elevator locked out and tagged. Return and meet with Bob Nazian to show him the Pelle parts that need repair. | TC-25874 & TC-28698 | |
| A-014 | AFW-94 | 4 | elevator doors are not closing properly on any floor. The car's breaker in penthouse was opened to prevent doors from constantly trying to close | 21/Dec/2020 | R | 22/Dec/2020 | Y | 5 | Arrived at service call and found unit at the top floor with doors not able to close, they were reopening. I verified and opened the sensors, I found water damage in the curtains. After that I went to see the main board in the car top and I saw marks of water all over the car top. Verified the Curtain board and it was bad with water damage too. I tested and declared that the board is bad and the curtain, this needs to be replaced. The generator guy came in and said that they had a water overflow in the weekend and that water came down to the elevator, this explain all the marks and the water damage. I saw water in one of the interlocks of the door, I will recommend to open them and take water out. The safety edge needs to be replaced with new one and needs to be retrofit into this controller. I left unit shutdown and turned off for approval. | TC-25934 | |
| A-015 | AFW-90 | 1000 | elevator is responding intermittently. The car has stopped on several floors and the doors are reported as not closing. The car has been reported as not responding to hall calls. | 23/Dec/2020 | R | 8/Jan/2021 | N | 2 | Found the interlock for the retiring cam was not lifting like 1/4" less so it was not letting the elevator leave the fourth floor. Adjusted and tested elevator operation. Left in service | TC-28928 | |
| A-016 | AFW-90 | 1000 | Doors are not closing automatically on 4'th floor (hall doors stop short of fully closing on 4'th floor, car does not respond to hall calls until doors are closed with the close button and the button held down for an extended period). | 4/Jan/2021 | R | N | |||||
| A-017 | AFW-97 | 633 | doors have an intermittent door closing issue. They are reported to begin to close, then reopen before closing again. Please investigate. | 8/Jan/2021 | R | 1/8/2021 & 2/3/21 | N | 1.25 & 3.5 | I got on the elevator and went to each floor and discovered the problem. Cleaned out the floor guide rails on each floor and the doors worked fine. Tested and left unit in service. TC-26536 Got on car top and cleaned the special Lens on the back. This resolved the issue. Took the information for the type of lens and voltage. Left in service | Did while doing maintenance and TC-26536 | |
| A-018 | AFW-96 | 633 | Investigate elevator doors opening and closing repeatedly without any human input. | 3/Feb/2021 | R | N | |||||
| A-019 | AFW-88 | 1000 | doors are closing too soon. Please adjust increase time doors remain open. | 5/Feb/2021 | R | N | |||||
| A-020 | #1 | 77H | Breaker was found tripped for elevator. Coworker is reporting elevator is making a lot of noise and rough starting but then greatly improves once car begins to travel. | 11/Feb/2021 | R | 12/Feb/2021 | N | 7 & 13 | Rode elevator and felt the harsh start and also squeaking. Went to control room and made adjustments to the unit up acceleration (#2). Made additional adjustments while helper operated elevator until the ride was smoother. While performing this I noticed the wires for the pressure switches were missing insulation. Recovered with electrical tape and noted that the switch will need replacement. Also found 2 rollers on the bottom limit switches and lubricated them. This was causing the squeaking. Tested and left in service. March 31 returned and installed new pressure switch. Tested and left in service. Also performed PM. | TC-26625 | |
| A-021 | AFW-93 | 4 | Dispatch was reporting elevator phone was calling them. | 16/Feb/2021 | R | 17/Feb/2021 | N | 2 | Elevator was turned off, waiting for repair. I verified that phone was okay. I opened panel and found water marks on phone button. Needs to be replaced. | TC-26676 | |
| A-022 | AFW-97 | 633 | AFW-97 is on the first floor and not responding to hall or car calls. | 22/Feb/2021 | R | 22/Feb/2021 | N | 4 | Unit was running. Verified and tested, the only problem identified was the safety edge reopening when not commanded to. This needs to be replaced and was previously reported during monthly maintenance. Problem cannot be resolved until replaced. | TC-26766 | |
| A-023 | AFW-90 | 1000 | car is not leveling properly | 2/Mar/2021 | 3/Mar/2021 | N | 4 | Adjusted floor leveler zone and tested | TC-26926 | ||
| A-024 | AFW-44 | 29 | Car is on mezzanine and not responding to calls | 16/Mar/2021 | 16/Mar/2021 | N | 5 | Upon arrival I observed the 1st floor hall call button with its light on indicating that it had been pressed but the car never came. Dan and I found the car on the M floor. The car door would open if a hall call was pressed but the car wouldn't move. The doors appeared to be closed completely so We went on top of the car. The top of the car including the selector tape and the magnets appeared to be in working order. We went to the motor room and I checked the fuses which using my meter read okay, next I checked the safety circuit using the print tapped to the wall. The safety circuit was open. We reset the car and it came down. We went in the car and cleaned the tracks in the car and the M floor. We wiped down the car door hanging track and cleaned up the rollers. The track had a grease ramp which we sanded and then light lubricated and the rollers had grease coming out of them. We cleaned the grease and again lightly lubricated the car door track, rollers and all other moving parts. While we had the car at chest level we cleaned the door operator limit contacts. I also changed the DO, T1, T2 relays in the controller. Ran and tested car with no issues. Left car in service. The electric safety edge is damaged on the bottom and is bypassing the bottom few infrared sensors, I recommend this safety edge be replaced. | TC-27097 | ||
| A-025 | AFW-90 | 1000 | Cars doors not closing fully on 4th floor preventing car from operating | 23/Mar/2021 | 24/Mar/2021 | N | 4 | Unit was found with the door closed. I operated the door and it worked fine but intermittently the car would not leave the fourth floor. Adjusted the door close limit switch and verified operation. Back in service. | TC-27219 | ||
| A-026 | AFW-90 | 1000 | Car became suck 5 feet below fourth floor trapping two employees. Fire department responded and shut down elevator | 13/Apr/2021 | |||||||
| A-027 | AFW-78 | 1000 | Elevator stuck on third floor and doors will not close | 14/Apr/2021 | 14/Apr/2021 | N | 3 | Verified the doors were deactivated. Found the problem was the fourth door stop switch was deactivated. Unsure why this occurred because you need a key to open the box but I reset switch and all was normal. Back in service | |||
| A-028 | AFW-98 | 1000 | West elevator is stuck on fourth floor and not responding | 14/Apr/2021 | 14/Apr/2021 | N | 5 | Troubleshoot the unit and had to call technical support. Found the rope grip tripped but the problem resetting it is that the terminal block in the board was not making good contact with the board. I had to jump the solder points in the board to get it working. Tested and found no issues. need to order DZ relay and a terminal block. Returned to service | |||
| A-029 | AFW-90 | 1000 | Car stuck on fourth deck | 28/Apr/2021 | 29/Apr/2021 | N | 9 | Unit was shutdown. Found governor linking pin out. Looked in hoistway and found it. Repaired and sealed the pin. Performed manual inspection speed governor test and unit passed. Returned to service |
