About this file
This document is a Proposed Subcontractor Identification List related to a Request for Application (RFA) issued by the Massachusetts Department of Mental Health (DMH) for Community Empowerment and Crisis Response Services. The RFA seeks to establish a pool of qualified vendors from which DMH can purchase services to address the immediate and long-term mental health needs of communities exposed to trauma. Applicants proposing to use subcontractors for any of the required services must complete this list, providing the subcontractor name and address, the specific CIR/CORE services they will provide, and whether the subcontractor qualifies as a small business under the Massachusetts Small Business Program. All subcontracts will be subject to DMH approval prior to execution.
The RFA requires that any approved subcontractors agree to the Terms and Conditions set forth in the RFA, and that subcontracts be in writing in accordance with the Commonwealth's Terms and Conditions. The RFA does not provide details on pricing, set asides, incumbents, funding sources, or other salient commercial terms.
