About this file
This document is an Evaluation Tool Worksheet related to a Community Empowerment and Crisis Response Services Master Agreement Request for Application (RFA) issued by the Massachusetts Department of Mental Health (DMH). The RFA seeks to establish a pool of qualified vendors from which DMH can purchase services to address the immediate and long-term mental health needs of communities exposed to trauma. Services to be evaluated include Critical Incident Response (CIR), Community Outreach, Resilience, and Empowerment (CORE), Assessment & Referrals, Individual/Group Counseling, Public Education, and Training, Resources and Consultation. The Evaluation Tool outlines the minimum submission requirements, as well as the criteria for reviewing the Application Form Part II, Part III, and the Cost Proposal Workbook to determine if an applicant is qualified to provide the requested services.
The Cost Proposal Workbook must demonstrate that the applicant's proposed costs are reasonable, cost-effective, and comprehensively reflect the staffing model and other resources needed to deliver the services. The Evaluation Tool also notes key information to be considered for the Master Agreement and future Statement of Work selection, such as the applicant's current Master Agreement status, Supplier Diversity Office certification, and service area preferences.
