About this file
This document is a Request for Application (RFA) for the Community Empowerment and Crisis Response Services Master Agreement issued by the Massachusetts Department of Mental Health (DMH). The RFA seeks to establish a pool of qualified vendors to provide services that address the immediate and long-term mental health needs of communities exposed to trauma, including rapid deployment of services to impacted communities. The services are structured into two categories: Community Resilience & Crisis Response Services and Community Outreach, Resilience, and Empowerment (CORE) Services. Applicants can apply for one or more service components within these categories. The application period is rolling, and interested parties can apply at any time until June 30, 2034.
The RFA outlines the payment models, including unit rate, cost reimbursement, and fixed cost, as well as preferences for selection, such as small businesses, businesses with bilingual or diverse staff, and a commitment to subcontracting with small businesses. The RFA also provides information on accessing the Small Business Purchasing Program, the Supplier Diversity Office, and COMMBUYS, the Commonwealth's online procurement system.

3. Attachments
**Note : Applicants wanting to qualify for more than one service component must submit for each additional service component: a separate Application Form Part III; Cost Proposal Workbook; and the Attachments required by those parts of the Application that address the additional service.
The Application Form has three sections. If the applicant is seeking to provide more than one service:
| Application Section | Requirement |
| Section 1: Application Overview | Complete Only Once |
| Section 2: Applicant Background | Complete Only Once |
| Section 3: Proposed Service (inc. Cost Proposal) | Complete for Each Service |
In the Cost Proposal Workbook…
Applicants need to include all their potential costs for providing the service.
Personnel costs are to be provided as an hourly rate (inc. fringe, travel, etc.), including all direct service personnel.
Other costs are to be provided as a fixed amount, or else the method for determining how those costs will be calculated must be clearly articulated (e.g., administrative cost is 3% of the total of the personnel cost included in the applicable SOW).
Given the range of possible services, the intent of the Cost Proposal Workbook is to provide DMH the basis for developing a SOW / to predict the cost of a service based on different scenarios.

1. Small Businesses registered with the SBPP
3. Businesses with staff who are minimally 25% bilingual, or 25% ethnically or racially diverse
2. A commitment to subcontract with one or more small businesses (registered with the SBPP or not)
