About this file
This document is a Statement of Work (SOW) for a Community Empowerment Crisis Response Master Agreement between the Department of Mental Health (DMH) and a qualified vendor. The SOW outlines the services to be provided, which include individual and group counseling, public education, community outreach and engagement, assessment and referrals, and training, resources, and consultation for community organizations. The SOW also covers the scope of services, required staffing, populations and communities to be served, service locations, service goals and performance measures, and reporting and invoicing requirements. The payment mechanism can be either a deliverable project, cost reimbursement, or a rate project, and the total budget for the engagement is specified.
The related state and local contract opportunity is a Request for Application (RFA) issued by the DMH to establish a pool of qualified vendors from which DMH Area Directors and Central Office Division Heads can purchase services to address the immediate and long-term mental health needs of communities exposed to trauma. The services will be purchased on an as-needed basis, and the contract term is not specified.
