About this file
This document is a Cost Proposal Workbook for a state and local contract opportunity issued by the Massachusetts Department of Mental Health (DMH) to establish a pool of qualified vendors for Community Empowerment and Crisis Response Services. The workbook requires applicants to identify all costs or charges for which reimbursement will be sought, including personnel, program support, administrative, and other costs. Applicants must provide details on how these costs will be calculated, whether as fixed amounts, fixed percentages, or variable percentages/rates. Applicants must also attach a description of any other categories or items that may impact pricing and a narrative justifying all proposed costs.
The contract opportunity seeks to address the immediate and long-term mental health needs of communities exposed to trauma, with services to be purchased on an as-needed basis. Applicants must complete a separate Cost Proposal Workbook for each proposed service, which will be reviewed individually. The workbook provides instructions on the required format and page limits for the attached cost justification narrative.

PREFERRED MODEL
Cost Reimbursement Identify the costs associated with the categories in the drop down menu below (Cell A11). These categories will form the foundation of any cost reimbursement budget that is incorporated into a SOW used to engage a Contractor for a project / service under the Master Agreement. This sheet should list all costs that could be included in a Cost Reimbursement Budget for a project / service. All costs stated will be considered to be fixed unless otherwise stated. If any cost will be based on a fixed percentage amount, then the fixed percentage amount and what it is based on (e.g., project size, location, etc. ) shall be stated. If a cost will be determined based on a variable percentage or rate, that shall be indicated as well as how the variable percentages/rates are determined. For example, if the administrative cost is based on a set percentage of a project size, the percentage amount must be stated as being fixed. If the percentage varies based on project size, that must be identified, as well as to how the increase is determined.
Also, an Applicant must attach one document including (1) a description of any other categories or items that may impact the pricing of a SOW. This shall include known costs and other costs that historically are needed, but which may vary depending on service requirements, location and required capacity (e.g., rental of space for hosting an event, advertisement costs, etc.). The Applicant must indicate how it will be able to quickly identify and quantify these costs to allow for timely SOW negotiations, should a crisis occur; and (2) a narrative that explains and justifies all of the proposed costs. The narrative must be sufficiently detailed so that an individual reviewing the costs would be able to understand how they were determined (assumptions that were made, etc.) and the appropriateness and reasonableness of the costs. The format and page limit for the attachment is set forth in the Workbook overall instructions.
