About this file

This document is a First Amended to the Community Empowerment and Crisis Response Services Master Agreement Request for Application issued by the Massachusetts Department of Mental Health (DMH). The amendments include clarifying application requirements for multiple services, correcting an erroneous reference, and allowing applicants to enter their names in Part I of the Application Form. The RFA seeks to establish a pool of qualified vendors from which DMH can purchase services to address immediate and long-term mental health needs of communities exposed to trauma. Services will be purchased on an as-needed basis. The application has three components: the Application Form, the Cost Proposal Workbook, and Application/Cost Proposal Attachments. If applying for multiple services, applicants must include additional Application Part III, Cost Proposal Workbooks, and attachments for each service. The application must follow specific file naming conventions.

The amended Application Form includes the DMH RFR ID number, allows applicants to complete the "Organizational or Individual Name" in Part I, and clarifies instructions for existing Master Agreement contractors submitting subsequent applications. Pricing, set-asides, funding details, and other salient commercial terms are not included in this amendment.

The Department of Mental Health is amending the above referenced Request for Application (RFA) for the following three reasons: (1) to clarify what needs to included in an application that is being filed for multiple services and the naming conventions that are to be used when filing such an application; (2) to correct an erroneous reference that was made in Subsection 1.3.5 of the RFA; and (3) to correct the Application Form to allow Applicants to enter their names in Part I of the Application.

The following three amendments are made to the Community Empowerment and Crisis Response Services Master Agreement Request for Application.

1. Subsections 5.2 of the Request for Application is deleted in its entirety and replaced in full by the following new Subsections 5.2.

5.2 Response Requirements

5.2.1 There are three parts to the Response.

a. The Application Form. The Application Form included in the Bid’s COMMBUYS file must be used for this purpose. The instructions for completing the form are also included in the Bid’s COMMBUYS file.

b. The Cost Proposal Workbook. The Cost Proposal Workbook and instructions that must be used can be found in the Bid’s COMMBUYS file.

c. Application and Cost Proposal Attachments. All attachments must be submitted as one (1) PDF with all Attachments numbered and in the order they are listed in the list of permitted attachments included in the Pain application Form (including those required by the Cost Proposal Workbook). The attachments must be limited to those specifically listed in the Application Form. Any attachment that is submitted that was not specifically asked for in the Application will not be reviewed.

If an Applicant submits a response to be approved for multiple services, the Applicant needs to include in their response the following additional parts for each additional service:

i. An additional Application Part III;

ii. An additional Cost Proposal Workbook; and

iii. Additional attachments that are required by Part III of the Application.

Each of these parts must indicate what additional service they address.

5.2.2 Naming Convention. The different parts of the response must be submitted on COMMBUYS using the file naming convention specified below. The Description entered during the file upload process ensures each file is readily identified by Applicant’s name and content. The File Name assigned by the Applicant as stored on their computer or network must be structured such that each file can be processed by the upload tool. The upload tool will reject any file name that includes spaces or symbols, like the brackets [ ] some systems apply when files are downloaded from the Internet.

For purpose of applying the naming convention, the services included in the Application shall be numbered in the order that they are checked off in Table 2 in Part I of the Application, starting with the first row of the Table and reading right to left; For example if an application is submitted for CIR Assessment & Referral; CIR Community Outreach & Networking, CORE Community Outreach & Networking and CORE Public Education, the services would be numbered as follows:
