This is a Prime/Subcontractor Form for Contract No. C-2014 (ITB No. 25-B-##XX) issued by the County of Volusia, Florida, for rigging services at the Ocean Center convention center and arena facilities. The project requires qualified rigging contractors to provide exclusive rigging services, equipment, and personnel for all events requiring overhead rigging systems. The Ocean Center comprises two facilities: a convention center with 93,028 square feet, 32-foot ceiling height, and 3,500-pound per load point capacity supporting approximately 50 events annually, and an arena with 7,010 fixed seating capacity, 172'5" x 250' playing surface, 45-foot ceiling height, and a permanent structure rigging grid supporting approximately 60 events annually. Primary services include motor and chain hoist services, truss system design and installation, rigging hardware supply, lighting and audio rigging, and scenic rigging. The County provides facility equipment including two forklifts, two scissor lifts, and one boom lift for contractor use.
Bidders must complete the Prime/Subcontractor Form by listing all proposed subcontractors and their respective dollar values and percentage of work performed, with all green-shaded cells requiring completion and other cells auto-calculating. The form requires that the total bid dollar amount match the bid price sheet submission and that all percentages equal 100%. The prime contractor retains responsibility for ensuring all subcontractors hold appropriate licenses, and any subcontractor changes during project completion require prior approval from the County Project Manager. The County reserves the right to clarify subcontractor information but will not permit changes to the total submitted base bid pricing. Failure to complete and return this form may result in a non-responsive bid determination. The form will also be used to evaluate local preference in accordance with Section 2.8, D of the solicitation.
