About this file
This is a Past Performance Questionnaire (PPQ) template for solicitation 1333ND26QNB610122 issued by the National Institute of Standards and Technology (NIST), Department of Commerce, for Roll Up Door Maintenance & Repair Services. The questionnaire is designed to be completed by clients or end-users who have previously purchased products or services from contractors bidding on this opportunity. It requests information about the contractor, the client/agency, and the specific product or service provided, including make and model, quantity, and purchase date.
The form evaluates contractor performance across multiple dimensions, including whether the product arrived in operating condition and met technical requirements, on-time delivery, and overall performance rating. Performance is assessed using a five-point scale ranging from Excellent (exceeds contractual requirements with few minor issues) to Unsatisfactory (does not meet most contractual requirements with ineffective corrective actions), with intermediate ratings for Very Good, Satisfactory, and Marginal performance. The questionnaire includes a Not Applicable option when information is unavailable. Instructions indicate that clients should complete and submit the questionnaire directly to the offeror, who will then include it with their quotation to the contracting officer. The form may be duplicated for future NIST solicitations as applicable.
