About this file

This is a Past Performance Questionnaire (PPQ) template for solicitation 36C10F24R0075, a VA design-build project to replace seismically deficient animal research buildings at the Sepulveda VA Medical Center. The PPQ requires contractors to provide details about previous contract performance, including contract information, pricing, and project descriptions.

The questionnaire includes evaluation criteria across seven key areas: quality, schedule/timeliness, communication, management/personnel/labor, cost/financial management, subcontract management, and an overall summary. Evaluators must rate performance using a scale from Outstanding to Unsatisfactory. The PPQ is not included in the page count for SF 330 submissions and must be returned to the offeror for incorporation into their SF 330 submission rather than being submitted directly to VA CFM. The related contract opportunity is for a $20-50 million design-build project for a new 7,500 GSF animal research facility, with RFQ responses due February 14, 2025, and a planned contract duration of 546 calendar days.
