SEPULVEDA AMBULATORY CARE CENTER, L.A., CA
A. This Request for Qualification (RFQ) is issued as Phase I of a Two-Phase Design-Build procurement (Reference
FAR 36.3). Proposals will be evaluated in Phase I to determine which offerors will submit proposals for Phase II.
A maximum number of three (3) offerors will proceed to Phase II unless the contracting officer determines that a number greater than three (3) is in the Government’s interest and is consistent with the purposes and objectives of this two-phase design build project. Phase II Request for Proposal will be issued only to those successful offerors determined to be most highly qualified from Phase I. One firm fixed price contract will be awarded using competitive negotiation.
B. This is a Design-Build project for the design and construction of a new Animal Research Building, 7 Annex to replace Buildings 47 and 103 at the VA Sepulveda Ambulatory Care Center, Los Angeles, CA. The Design-Build
Team (DBT) shall provide all labor, materials, tools, equipment, and design-build services necessary for design and construction of a project described herein as well as other specific tasks as further defined by this RFQ.
C. As a Design-Build project, the Government is seeking a construction process and anticipates the following as opposed to traditional design and construction methods:
• Single Source Point of Contracting Contact for Design and Construction
• Greater quality of design due to collaboration with construction contractor
• Time savings due to the procurement of design and construction in a single phase
• Greater collaboration with Government end users
The RFP package is to be used as For Information Only (FOI). If the contractor decides to use the conceptual design provided, it is the contractor’s responsibility to assure that all calculations, reports, technical details meet the VA requirements. Therefore, if the contractor picks up the design from the RFP, the expectation is to receive a
65% submittal for review. If the contractor is following a new design approach, then a 35% submittal will be the first submission. The VA will decide and approve on which approach the contractor proposes during Phase II of the solicitation. Prior to awarding the DB contract, the VA and the DBT will review required submissions as annotated on PG 18-15 Vol DB.
Construction Document submission will occur at 35%, 65%, 90%, and 100% Design Package based on approach approved by the VA. Therefore, once the 90% submission has been approved by the Government, the final 100% construction documents shall ensue. Subsequently, once the 100% submission has been approved by the
Government, construction activities shown on those documents shall ensue. The contractor will provide the
Construction Support Services and Medical Center Leadership presentation on the 90% and 100% submission for approval. Construction drawings must conform to VA Standards per the Technical Information Library website and current California Building Codes. Construction drawings must comply with VA Standards as outlined on the Technical Information Library website and the current California Building Codes. Where the codes do not align, the contractor must follow the most stringent criteria. Drawings must be wet stamped by an architect, structural engineer, and geotechnical engineer each licensed in the State of California.
D. The function of the Sepulveda Animal Research Facility is critical to the mission of the entire medical center.
The contractor will provide all labor, material, and equipment necessary to complete a final project design along with complete physical construction necessary to deliver a new Animal Research Facility at the Sepulveda

In Phase I, design-build teams will be asked to submit qualifications to the Government, which will subsequently select no more than three (3) of the most highly qualified offerors.
Section A-3 describes the evaluation factors and procedures for the Phase I process.
The selected offerors from Phase I will be invited to submit a design approach, management approach, and pricing proposals for Phase II. Contract award will be made at the conclusion of Phase II to the offeror who presents the best overall value to the Government. This source selection, after evaluation of all the factors, may result in an award being made to an offeror that is not the lowest priced.

Graphic presentations, including tables, while not subject to the same font size and spacing requirements, shall have spacing and text that is easily readable PDF format.
3.6 Confidential Information: The Freedom of Information Act (FOIA) and its amendments have resulted in an increasing number of requests from outside the Government for copies of contractor qualifications and proposals submitted to federal agencies. If an offeror’s submissions contain information that he/she believes should be withheld from such requestors under FOIA on the grounds that they contain “trade secrets and commercial or financial information” [5 USC§552(b)(4)], the offeror should mark its submissions in the following manner:
• The following notice should be placed on the title page: “Some parts of this document, as identified on individual pages, are considered by the submitter to be privileged or mailto:ferdinand.gawaran@va.gov mailto:deborah.stigen@va.gov http://www.fbo.gov/ confidential trade secrets or commercial or financial information not subject to mandatory disclosure under the Freedom of Information Act. Material considered privileged or confidential on such grounds is contained on page(s) _______”.
• Each individual item considered privileged or confidential under FOIA should be marked with the following notice: “The data or information is considered confidential or privileged, and is not subject to mandatory disclosure under the Freedom of Information Act”.
3.7 Two-Phase Evaluation: Phase I is intended to select offerors to participate in Phase II
(See A4 below).
3.8 Phase I Information to be provided:
Offeror MUST clearly indicate the following information
• Name and Address of Proposer
• Business Arrangement (Joint Venture or Teaming Arrangements clearly identified)
• Contact Name and Phone Number
• Email Address
• UEI (Unique Entity Identifier) Number
• CAGE Code

• Business Category
3.8.1 Phase I Evaluation Factors:
3.8.1.1 Factor Identification: The Phase I evaluation will select approximately three (3) of the most highly qualified offerors to advance to Phase II. A responsibility determination will be made in accordance with FAR Part 9.1, Responsible Prospective Contractors.
Offerors will be evaluated using the following factors:
Factor I – Technical Approach
Sub-factor A: Design Approach
Sub-factor B: Construction Approach
Sub-factor C: Management Approach
Factor II – Technical Qualifications
Sub-factor A: Specialized Experience and Technical Competence
Sub-factor B: Capability to Perform
Sub-factor C: Past Performance of Construction and Architect-Engineer Teams
Factor III – Quality Control Procedures
Factor IV – Safety Record
3.8.1.2 Order of Importance: Technical Approach, Technical Qualifications, Quality Control
Procedures and Safety Record are equal in importance. Sub-factors under each factor are equal in importance.
3.8.1.3 Past Performance Evaluation Rating Standards: The VA will assign an adjectival rating based on the information provided by the references and any other source. In the case of an offeror without a record of relevant past performance or for whom information on past performance is not available, the offeror may not be evaluated favorably or unfavorably on past performance and will receive a neutral rating. However, the proposal of an offeror with no relevant past performance history, while rated neutral in past performance, may not represent the most advantageous proposal to the Government. The Evaluation Team may consider past performance data from a wide variety of sources both inside and outside the Federal Government. The Government evaluation team will conduct an evaluation based on past performance of offerors as it relates to the probability of successfully performing the solicitation requirements, and the Past Performance evaluation will assess an offeror’s likelihood of success in fulfilling the solicitation’s requirements based on the relevancy and confidence assessments standards as indicated below.
The following weightings apply to relevancy regarding the standards listed above for the past performance rating:
Relevancy Rating Chart
Adjective Definition
Very Relevant Past/present performance efforts involved essentially the same scope and magnitude of effort and complexities this solicitation requires
Relevant Past/present performance efforts involved similar scope and magnitude of effort and complexities this solicitation requires
Somewhat Relevant Past/present performance efforts involved some of the scope and magnitude of effort and complexities this solicitation requires
Not Relevant Past/present performance efforts involved little or none of the scope and magnitude of effort and complexities this solicitation requires
The second aspect of the past performance evaluation is to determine how well the contractor performed on the contracts. The past performance evaluation performed in support of a current source selection does not establish, create, or change the existing record and history of the offeror’s past performance on past contracts; rather, the past performance evaluation process gathers information from customers on how well the offeror performed those past contracts.
Sources of Past Performance Information for Evaluation are as follows:
1. Past performance information shall be obtained from any other sources available to the Government, to include, but not limited to, the Past Performance Information Retrieval System (PPIRS), Federal Awardee Performance and Integrity Information System (FAPIIS), Electronic Subcontract Reporting System (eSRS), or other databases; interviews with Program Managers, Contracting Officers, and Fee Determining Officials; and the Defense Contract Management Agency (DCMA).
2. Past performance information may be provided by the offeror, as solicited.
3. Past performance information may be obtained from questionnaires tailored to the circumstances of the acquisition as outlined in the past performance questionnaire attachment.
The Past Performance Evaluation Team will review this past performance information and determine the quality and usefulness as it applies to performance confidence assessment.
Performance Confidence Assessments
Rating Description
Substantial Confidence
Based on the offeror’s recent/relevant performance record, the Government has a high expectation that the offeror will successfully perform the required effort.
Satisfactory Confidence
Based on the offeror’s recent/relevant performance record, the Government has a reasonable expectation that the offeror will successfully perform the required effort.
Limited Confidence Based on the offeror’s recent/relevant performance record, the Government has a low expectation that the offeror will successfully perform the required effort.
No Confidence Based on the offeror’s recent/relevant performance record, the Government has no expectation that the offeror will be able to successfully perform the required effort.
Unknown Confidence (Neutral)
No recent/relevant performance record is available or the offeror’s performance record is so sparse that no meaningful confidence assessment rating can be reasonably assigned.
3.8.1.4 Technical Evaluation Rating Scale: The evaluation team will utilize the scale below to assign adjectival ratings to offerors for Phase I and Phase II. All technical subfactors will be evaluated utilizing the technical evaluation rating scale.
Adjective Definition
Outstanding Proposal meets requirements and indicates an exceptional approach and understanding of the requirements. Strengths far outweigh any weaknesses. Risk of unsuccessful performance is very low.
Good Proposal meets requirements and indicates a thorough approach and understanding of the requirements. Proposal contains strengths which outweigh any weaknesses. Risk of unsuccessful performance is low.
Acceptable Proposal meets requirements and indicates an adequate approach and understanding of the requirements. Strengths and weaknesses are offsetting or will have little or no impact on contract performance. Risk of unsuccessful performance is no worse than moderate.
Marginal Proposal does not clearly meet requirements and has not demonstrated an adequate approach and understanding of the requirements. The proposal has one or more weaknesses which are not offset by strengths. Risk of unsuccessful performance is high.
Unacceptable Proposal does not meet requirements and contains one or more deficiencies. Proposal is not awardable.
