SCOPE OF WORK

The contractor shall follow all applicable MDNR, EPA, and OSHA rules and regulations. The Contractor shall install plywood barriers and/or lock doors with sufficient signage to isolate the work area in order to prevent access by unauthorizes personnel. Barriers shall be constructed in accordance with MDNR rules and regulations.

Contractor shall provide all worker paperwork to Geo-Enviro Consultants, Inc.'s on-site project manager at the beginning of the shift on the first day of the project. If additional workers arrive during the duration of the project, their paperwork shall also be provided to the Geo-Enviro Consultants, Inc.'s project manager. For asbestos workers and supervisors, this includes copies of their current MDNR license, training certificate, physical, and fit test for each person. All persons involved shall have his/her MDNR “hard card” license on-site each and every day said person is on-site. Any and all workers who do not provide the required documentation shall not be admitted to the project site by the Geo-Enviro Consultants, Inc. project manager.

The asbestos abatement materials include the removal of pipe system insulation and fittings.

Lead-Based paint to be abated includes white paint on plaster ceilings and walls, blue paint on concrete walls and yellow ceramic tile.

Mold remediation to include the cleaning of all surfaces and the removal and disposal of all ceiling tiles in Rooms G04 and G05 ONLY.

All asbestos abatement work shall utilize wet methods.

Tented mini-containment areas shall consist of critical seals and one layer of 6-mil poly on the floors, walls, and ceiling of the tent.

Each tented mini-containment work area shall include critical seals over all doorways, windows, and ventilation system openings. Negative air pressure may be provided by HEPA vacuum cleaning equipment (remaining ibn operation for the duration of the project) and negative air may be discharged to the interior or exterior of the building. Contractor shall install a 1-chamber airlock to each tented containment work area for the worker entry/exit. A remote 5-chamber decon with negative air machine attached to the dirty room and exhausting outside the building shall be constructed.

For tented mini-containment work areas, the contractor shall employ a double disposable suiting procedure. The outer “dirty” suit shall be removed immediately prior to exiting a mini-containment, and a new “clean” outer suit shall be donned immediately prior to entering another mini-containment.

Full containment work areas shall consist of critical seals, two layers of 4-mil poly on the walls, and one layer of 6-mil poly on the ceiling. An attached 5-chamber decon and 2-chamber waste out shall be constructed at the entry to the full containment work area.

The contractor shall provide a manometer for the full containment work area to ensure that a negative pressure of 0.02” w.c. is maintained throughout the duration of the project. All negative air exhaust tubing shall be vented outside the work area.

All waste shall be adequately wet and double-bagged. Waste containers shall be labeled and leak-proof. All waste with sharp-edged components (e.g. nails, screws, metal lath, floor tile, etc.)

shall be placed into drums for disposal. All equipment and double-bagged/labeled waste shall be transported from the work area directly to an enclosed, lockable, signed, and poly-lined dumpster. All double-bagged and labeled waste shall be transported from the work area in a covered gondola. No waste shall be left within the building overnight.

All electrical systems shall be shut down and locked out. All electrical power within the work areas shall be “ground-fault protected” utilizing Ground Fault Circuit Interrupters. The contractor is required to handle all electrical shutdowns. Temporary lighting contractor's responsibility and shall be provided as necessary.

The owner will assist the contractor with electrical shutdowns and HVAC isolation within the work area. The building owner shall be responsible for maintaining appropriate life safety code requirements, including fire protection, for the building for the duration of the project.

After all abatement and cleaning activities are complete in any particular work area, the contractor shall apply a lock-down encapsulant on all materials in which asbestos or asbestos-contaminated materials were removed. This shall be conducted prior to clearance air sampling. Clearance air samples shall be collected when the cleanliness of the work area is to the satisfaction of the on-site project manager, and the required amount of hangtime has elapsed.

In all areas of abatement, contractor shall be responsible for investigating all rooms adjacent to all work areas during teardown activities. If any debris of any kind is identified in any room adjacent to an abatement work area, the contractor shall be responsible for the cleanup of that debris utilizing HEPA-vacuuming and wet methods.

Clearance is to be performed by aggressive TEM sampling methods and will be provided by Geo-Enviro Consultants, Inc's on-site project manager.

NOTES - GENERAL

1. Work will be completed following all applicable requirements.

2. The owner will not accept any change orders based on the contractor's failure to recognize unique conditions.

3. All permits and fees that need to be filled are the responsibility of the contractor. A copy of these permits and receipts shall be supplied to the owner's representative prior to start of work.

4. The owner will assist in the coordination for hooking up electrical equipment, if needed. The contractor will be responsible for supplying a licensed electrician.

5. The contractor is to use only MDNR-licensed workers.

6. The contractor's MDNR-licensed supervisor must be on-site at all times.

INFORMATION CONCERNING THE LOCATION(S) AND SIZES OF EXISTING MECHANICAL EQUIPMENT, ELECTRICAL EQUIPMENT, PIPING, PLUMBING AND DUCTWORK WAS OBTAINED FROM ORIGINAL

DESIGN DRAWINGS AND/OR CURSORY FIELD OBSERVATION, HOWEVER, ACTUAL "ASBUILT"

DRAWINGS WERE NOT AVAILABLE, CERTAIN INFORMATION CONCERNING THE LOCATION OF THE

EXISTING MECHANICAL EQUIPMENT, ELECTRICAL EQUIPMENT, PIPING, PLUMBING AND DUCTWORK HAS BEEN ASSUMED IN THIS DRAWING. THE EXACT LOCATION(S) AND DIRECTION OF FLOW OF ALL EXISTING WORK IS UNKNOWN. REASONABLE EFFORT HAS BEEN MADE TO ACCURATELY DEPICT THE

EXISTING CONDITIONS, HOWEVER, ALL EXISTING WORK MUST BE VERIFIED IN THE FIELD TO

DETERMINE THE EXACT LOCATIONS, DIRECTIONS OF FLOW, DEPTH, SIZE, ETC.. PRIOR TO STARTING CONSTRUCTION. ANY CONFLICT BETWEEN THESE PLANS AND ACTUAL FIELD CONDITIONS SHALL BE

REPORTED IMMEDIATELY TO THE ENGINEER FOR VERIFICATION AND/OR CORRECTION.

FIELD VERIFY:
