About this file

This document is a Past Performance Questionnaire (PPQ) for the National Institutes of Health (NIH) Construction Quality Management (CQM) Multiple Award Task Order Contract (MATOC). The form is designed to evaluate a contractor's performance across five key areas: quality of work, schedule performance, cost control effectiveness, management effectiveness, and overall performance. The questionnaire uses a rating scale from Exceptional to Unsatisfactory, with references being asked to provide ratings and comments on the contractor's performance.

The PPQ is part of the NIH's FY2026 Architect-Engineering (A/E) MATOC solicitation, which seeks to establish multiple indefinite-delivery indefinite-quantity (IDIQ) contracts for professional A/E services, including Commissioning Support Services and Construction Quality Management. Completed forms must be submitted via email to three NIH contacts: Thuy Le, Ana Marie Pagulayan, and Magan Miller. The document is marked as "FOR OFFICIAL USE ONLY" and contains source selection sensitive information, indicating its importance in the contractor selection process.
