About this file

This document is an Attachment 2 CQM Offeror Project Experience Form for an NIH Multiple Award Task Ordering Contract (MATOC) for Architect-Engineering Services. The form is designed for potential contractors to document their project experience for a pre-solicitation opportunity at the National Institutes of Health (NIH), Office of Research Facilities.

The form requires detailed project information including firm name, project title, contract details, work type, and a comprehensive project description. Project work types span critical infrastructure categories such as laboratories (including bio-medical research and animal research facilities), hospitals, healthcare facilities, data centers, utilities infrastructure, and public buildings. Offerors must provide specific details about their role (prime contractor, joint venture, etc.), contract values, performance periods, and demonstrate relevance to the NIH contract. The form emphasizes documenting precise work performed, especially for joint ventures or subcontractor roles, with a five-page limitation for describing project scope, size, and complexity.

SF-330 - Block 24 - Brief Description of Project and Relevance to this Contract:

Offerors should review criterion in the Presolicitation Notice document for each project submitted. Include specific responses to the following situations. If any of the below situations apply, address this situation in your first paragraph of your Project Description below:

· Explanation for any variance from award value.

· If a project was performed by a joint venture, and not all joint venture partners are on the team proposed for this contract, the offeror/team should specifically address the work performed by the joint venture partner offering/teaming on this contract, rather than the work performed on the project as a whole.

· If the Offeror is a joint venture, project experience shall be submitted by the joint venture entity; however, if there is no past experience or limited past experience available for the joint venture, project experience shall be submitted for all joint venture partners.

· If the contractor served as a JV member on a project, indicate the values of the work performed directly by the Offeror, as well as, the overall project value.

· If the Offeror/team member worked as a subcontractor on a project, the description should clearly describe the work performed by the Offeror/team member, rather than the work performed on the project as a whole.

· If the contractor served as a subcontractor on a project, indicate the values of the work performed as a subcontractor, as well as, the overall project value.
