ATTACHMENT (10) PAST PERFORMANCE QUESTIONNAIRE FOR THE

TRAINING DATA PRODUCTS CONTRACT(TDPC)

Questionnaire Instructions: The form is designed to be e-mailed to the Customer and completed using Microsoft Word 2007.

PART A - The offeror shall complete PART A only. After completing Part A the offeror shall forward one copy to each PCO, Program Manager and Technical Lead for each relevant project, WHICH DOES NOT HAVE A CPARS SUBMISSION, to be considered for the TDPC Past Performance Volume 3. In all cases where the offeror performed as a subcontractor on the effort the form shall be submitted to the prime contractor’s Program Manager (PM) in addition to the Government representatives previously mentioned.

PART B - The respondent(s) (Contracting Officer, Program Manager and Technical Lead (Prime’s PM if applicable)) will each complete PART B of the form received. The respondent(s) may elect to complete long hand and return by fax if needed. After completing PART B the respondent shall deliver the form to NAWCTSD.

Form Delivery Method - The respondent(s) will return the completed Past Performance Questionnaire directly to Mr.Thomas Courtney and Mr. Rafael Soberal at Thomas.courtney@navy.mil and Rafael.soberal@navy.mil. Mr. Soberal can be reached by phone at 407-380-4077 or by fax at 407-380- 8406.

What was the contractor’s record of timeliness, completeness and quality of problem identification, corrective action plans, proposal submittals (especially responses to change orders, engineering change proposals, or other undefinitized contract actions), the contractor's history of reasonable and cooperative behavior, effective business relations, and customer satisfaction?

4.2 Subcontract Management. What was the contractor’s success with timely award and management of subcontracts, including whether the contractor met or exceeded small business, small disadvantaged business, small business HUBZone, veteran-owned small business, service disabled veteran owned small business, women-owned small business participation and subcontracting goals?

4.3 Program Management and Other Management. To what extent does the contractor discharge their responsibility for integration and coordination of all activity needed to execute the contract; identify and apply resources required to meet schedule requirements; assign responsibilities for tasks/actions required by contract; and communicate appropriate information to affected program elements in a timely manner?

4.4 What were the contractor’s risk management practices, especially the ability to identify risks and formulate and implement risk mitigation plans?

4.5 If applicable, identify and provide information on any other areas that are unique to the contract, or that cannot be captured elsewhere under the Management Assessment.
