Pursuant to paragraph (35) of reference (b), this project qualifies for the following categorical exclusion: " (35) Demolition, disposal, or improvements involving buildings or structures when done in accordance with applicable regulations including those regulations applying to removal of asbestos, PCBs, and other hazardous materials.." This project will not result in any extraordinary circumstances.

Enclosure 3: Applicable Conditions

AST/UST/IR/MRP (Soil):

1. If contaminants are suspected through odors or visual observation during construction activities, immediately stop work, call 911, and contact EQB at 910-451-5878 for further instructions.

2. There are no known monitoring wells in the project areas but if an unmarked well is discovered and inadvertently damaged, immediately contact EQB at 910-451-5878 for further instructions.

3. 3R UXO Safety Awareness Training is recommended for all construction workers (available through EOD, the OICC or EMD) as a precaution given that Camp Lejeune has been an active military base since the 1940s.

Should a munitions-related item be found during construction, follow appropriate notifications as detailed in the 3R training. Any munitions related item found during project construction should additionally be reported to EMD within 10 days (K. Hiortdahl, 910-451-5878) to determine if additional evaluation and precautions are necessary (Explosives Safety Submission, UXO Tech Support, etc).

5. Building Comments for Above Ground/Underground Storage Tank Concerns

1203: Former POL Clean up site at SW end of building - Potential for contaminated soils in this area

AS4128: Near active POL remediation site with known groundwater impacts. Potential for contaminated soils. Existing monitoring wells are present in this area and should be protected. If they are damaged or will need to be demolished, the wells should be properly abandoned in accordance with State regulations by a State-licensed well driller.

Replacement wells will need to be installed, please coordinate with UST Program manager for abandonment.

AS849: Former POL Clean up site - Potential for contaminated soils in this area.

BA130 Area (BA128-BE172 & SBA131, -132): Several Former POL Clean up sites - Potential for contaminated soils in this area. Active remediation site for groundwater, would only have impact if deep digging or dewatering.

FC240: Former POL Clean up site - Potential for contaminated soils in this area.

M234 & M235: Shallow groundwater contamination present. Active POL Clean up site - Potential for contaminated soils in this area.

RR15, RR51, RR49: Former POL Clean up site - Potential for contaminated soils in this area.

SAS3533: This site is located within a Land Use Restriction (LUR) for soil. Any disturbed soil within the LUR boundary must remain on-site.

If soil is to leave the site, it must be sampled and properly disposed of.

Potential soil contamination outside the LUR for this area. See attached figure.

6. If contaminated soils are encountered construction activities that require digging into the soil will require additional safety precautions (such as 40 hour HAZWOPER training) and waste management (disposal) for soils in accordance with the most current Waste Management Plan. Contact the UST Program Manager (T. Richard, 451-9461) for additional details.

7. AST Tank removal - If any removal of ASTs are included in this project please coordinate with Program Manager (T. Richard, 451-9461). Ensure the tank is properly drained and cleaned. The tank shall be triple rinsed to ensure all vapors are removed before disposal. Waste water from the rinsing shall be containerized and properly disposed of. Demolished tank information will need to be provided (Location, Size, contents, etc).

8. There are no known Installation Restoration issues associated with the subject project. However, these projects are within historic and/or known operational range areas and it is recommended (although not regulatorily required) that the area be assessed for possible surface or buried UXO before construction begins. Recommend coordinating with Range Control on appropriate safety requirements for construction.

9. Building Comments for Installation Restoration Concerns
