Quality Assurance Surveillance Plan (QASP)
Contract: N00024-15-R-6400
26 March 2015
Attachment 7 Quality Assurance N00024-15-R-6400 Surveillance Plan (QASP)
For Official Use Only
SOURCE SELECTION SENSITIVE INFORMATION – SEE FAR 2.101 and 3.104
1.0 PURPOSE
This Quality Assurance Surveillance Plan (QASP) defines the Government approach to monitoring, managing, and executing appropriate action with regard to Contractor performance against contractual objectives. The QASP establishes the methods used to measure the effectiveness of the Contractor's Quality Program Plan (QPP) described in Contract Data Requirements List (CDRL) A020, provides delivery criteria with thresholds, and reduces the risk of nonconforming material being delivered to the Fleet.
This document establishes surveillance and quality performance criteria which demonstrate that Guidance and Control (G&C) Sections and Upgrade Kits not only pass the individual Factory Acceptance Test (FAT) process, but also are a product of a controlled quality process. Factory delivery or non-delivery status is established based on compliance with the criteria detailed in this QASP. MK48 Mod 7 G&C Sections and MK48 CBASS Upgrade Kits will only be presented for sell-off while the factory is in delivery status. Acceptance of the MK48 Mod 7 G&C Sections and MK48 CBASS Upgrade Kits shall be made by the Procuring Contracting Officer (PCO) as detailed in Section E of the Contract.
2.0 SCOPE
The QASP provides Government actionable criteria to ensure that the Contractor's quality control efforts are timely, effective and are delivering the results specified in the contract. The QASP is not intended to duplicate the Contractor's QPP, but instead is provided to support the Contractor's efforts in developing a QPP which ensures the QASP criteria are met.
3.0 AUTHORITY
Authority for issuance of this QASP is provided under Federal Acquisition Regulations (FAR) 46.401, Contract Section E - Inspection and Acceptance.
4.0 ROLES AND RESPONSIBILITIES
Procuring Contracting Officer (PCO) – The PCO ensures performance of all necessary actions for effective contracting; ensures compliance with the terms of the contract; and safeguards the interests of the United States in the contractual relationship. It is the PCO that ensures the Contractor receives impartial, fair, and equitable treatment under the contract. The PCO is ultimately responsible for the final determination of the adequacy of the Contractor's performance.
Contracting Officer Representative (COR) – The COR is responsible for technical administration of the project and ensures proper Government surveillance of the Contractor's performance. The
COR is not empowered to make any contractual commitments or to authorize any contractual changes on the Government's behalf. Any changes that the Contractor deems may affect contract price, terms, or conditions shall be referred to the PCO for action.
5.0 METHODS OF SURVEILLANCE
The primary means of surveillance for the criteria outlined in this QASP is through CDRL deliverables. The Government, at its discretion, may independently verify the provided information using available Government resources in accordance with the contract and FAR requirements.
5.1 Performance of Surveillance
The Government representative(s) will monitor performance and review performance reports and CDRLs furnished by the contractor to determine how the contractor is performing against communicated performance objectives. The contractor is responsible for making required changes in processes and practices to ensure performance is managed effectively against the QPP (CDRL A020), Statement of Work, and the thresholds described in Table 1.
5.2 Surveillance Level Adjustments
The COR and PCO may periodically review the QASP to determine if changes in surveillance levels or methods are necessary and update the QASP accordingly. When a change in the Contractor’s required work (e.g., a change of design) or a change in the Contractor’s performance results in a change of program risk, surveillance changes may be considered.
6.0 DEFINITIONS
Nonconformance (NC) – A defect; a unit of product that departs from a specified requirement with respect to one or more features, dimensions, properties, or parameters that may affect subsequent operations, form, fit, or function. A nonconformance may be detected by any process, inspection, or test.
Key Quality Indicator (KQI) –Process indicator or other quantitative measurement criteria with defined thresholds upon which delivery of supplies is based. KQIs provide indication of the expected quality level at subsequent tests or inspections.
Functional Item Replacement (FIR) – Items which are typically installed or replaced as subassemblies during torpedo turnaround at an Intermediate Maintenance Activity (IMA) and can be independently tested prior to installation.
Category I NCs – Those NCs which occur during a FIR level factory acceptance test as defined by CDRL A025.
Category II NCs – Those NCs which occur during a System Level factory acceptance test as defined by CDRL A025.
Category III NCs – Those NCs found during Government inspection, assembly, disassembly, in-water test, environmental test, or any other Government test during the Conditional Acceptance period, as defined in Section E of the contract.
7.0 DELIVERY CRITERIA, KQI USAGE and THRESHOLDS
All KQIs established in this QASP are based on nonconformances as documented by CDRL A032 (Failure Analysis and Corrective Action Report). The KQIs are categorized by the test phase where the nonconformance is found and by which Contract Line Item Number (CLIN) the test article is associated. For a graphical representation of the KQIs, see Figure 1.

7.1 Category I Nonconformances; FIR Level Factory Acceptance Test KQIs
7.1.1 Category I, Type A; CBASS Upgrade Kit FIRs
CDRL A032 Nonconformances attributed to the following FIRs found during CDRL A025 testing:
• Guidance Control Box (GCB) (8553364-1*)
• Broadband Sonar Analog Receiver (BSAR) (8553534-1*)
• Preamplifier (8553523-1)
• Electro Magnetic Interference (EMI) Filter (8553508-1*)
• EMI Cable Assembly (7561289-1)
• Wirelink Cable Assembly (7561448)
• Processor to GCB Cable Assembly (8176782-1)
• GCB to Processor Cable Assembly (8176782-1)
• CCA Interconnect Cable Assembly (8176826)
• Preamplifier to Forward Receiver Cable Assembly (8176784-1)
• Preamplifier to Aft Receiver Cable Assembly (8176785-1)
• Power Signal Cable Assembly (8176786-1).
* New part numbers are expected for redesigned FIR components.
7.1.2 Category I, Type B; Non-CBASS Kit FIRs
CDRL A032 Nonconformances attributed to the following FIRs found during CDRL A025 testing:
• Nose Array (6879925)
• Tuning Box (5856624)
• Depth Sensor Assembly (DSA) (5856460*)
• Wirelink (5728320)
• IMU (6240279*)
• Transmitter (5728260)
* New part numbers are expected for redesigned FIR components.
7.2 Category II Nonconformances; System Level Factory Acceptance Test KQIs
Allowable quantities of Category II NCs will be dependent upon the total number of System- Level (CDRL A025) tests. Table 1 shows the correlation between allowable open NCs and tested quantities. CDRL A032 NCs found during CDRL A025 testing of the following CLINs:
0003, 0103, 0203, 0303, and/or 0403.
7.3 Category III Nonconformances; Post-delivery
CDRL A032 NCs found during Government testing will be dependent upon the total number of CBASS Kits and G&C Sections that have been tested by the Government delivered under CLINs 0003, 0004, 0103, 0104, 0203, 0204, 0303, 0304, 0403, and/or 0404. Table 1 shows the correlation between allowable NCs and tested quantities. Allowable open Category III NCs and tested quantities are assessed only on hardware in the conditional acceptance period.
7.4 KQI Delivery Suspension Thresholds
Exceeding the KQI thresholds defined in this QASP may be used as cause for suspension of production deliveries, as detailed in Section 3.3.12.4 of the Contract’s Section C, Statement of Work (SOW). Table 1 shows the correlation between allowable open NCs and tested quantities.
