Base Initial Submission (RFP: N00024-15-R-6406) 27 July 2015

1.0 PURPOSE

This Quality Assurance Surveillance Plan (QASP) defines the Government approach to monitoring, managing and executing appropriate action with regard to Contractor performance against contractual objectives. The QASP establishes the methods used to measure the effectiveness of the Contractor’s Quality Program Plan (QPP) described in Contract Data Requirements List (CDRL) A016, provides delivery criteria with thresholds, and reduces the risk of nonconforming material being delivered to the Fleet.

This document establishes surveillance and quality performance criteria which demonstrate that MK 54 LWT Common Parts Kits (CLINs 0003, 0103, 0203, 0303, and 0403), PGA and W4 cables (CLINs 0004, 0104, 0204, 0304, 0404), and Exercise Fuel Tanks (CLINs 0010, 0110, 0210, 0310, and 0410) not only pass the individual Factory Acceptance Test

(FAT) process, but also are a product of a controlled quality process. Factory delivery or non-delivery status is established based on compliance with the criteria detailed in this QASP. MK 54 LWT hardware shall only be presented for sell-off while the factory is in delivery status. Acceptance of MK54 LWT Common Parts Kits, PGA, W4 cables, and

Exercise Fuel Tanks shall be made as established in Section E of the contract.

2.0 SCOPE

The QASP provides Government actionable criteria to ensure that the Contractor's quality control efforts are timely, effective and are delivering the results specified in the contract. The QASP is not intended to duplicate the Contractor's QPP, but instead is provided to support the Contractor's efforts in developing a QPP which ensures the QASP criteria are met.

3.0 AUTHORITY

Authority for issuance of this QASP is provided under FAR Subpart 46.4.

4.0 ROLES AND RESPONSIBILITIES

Procuring Contracting Officer (PCO) – The PCO ensures performance of all necessary actions for effective contracting and compliance with the terms of the contract and safeguards the interests of the United States in the contractual relationship. It is the PCO that assures the Contractor receives impartial, fair, and equitable treatment under the contract. The PCO is ultimately responsible for the final determination of the adequacy of the Contractor’s performance.

Contracting Officer Representative (COR) – The COR is responsible for technical administration of the project and assures proper Government surveillance of the Contractor’s performance. The COR is not empowered to make any contractual commitments or to authorize any contractual changes on the Government’s behalf. Any changes that the

Contractor deems may affect contract price, terms, or conditions shall be referred to the PCO for action.

5.0 METHODS OF SURVEILLANCE

The primary means of surveillance for the criteria outlined in the QASP is through CDRL deliverables. The Government, at its discretion, may independently verify the provided information using available Government resources in accordance with contract and Federal Acquisition Regulation (FAR) requirements.

5.1. PERFORMANCE OF SURVEILLANCE

The Government representative(s) will monitor performance and review performance reports and CDRLs furnished by the Contractor to determine how the Contractor is performing against communicated performance objectives. The Contractor is responsible for making required changes in processes and practices to ensure performance is managed effectively against the QPP (CDRL A016), Statement of Work, and the thresholds described in Table 1.

5.2. SURVEILLANCE LEVEL ADJUSTMENTS

The COR and PCO may periodically review the QASP to determine if changes in surveillance levels or methods are necessary, and update the QASP accordingly. When a change in the Contractor’s required work (e.g. a change of design) or change in Contractor’s performance results in a change of program risk, surveillance changes may be considered.

6.0 DEFINITIONS

Nonconformance - A unit of product that departs from a specified requirement with respect to one or more features, dimensions, properties, or parameters that may affect subsequent operations, form, fit, or function. A nonconformance may be detected by any process, inspection, or test.

Key Quality Indicator (KQI) – Process indicator or other quantitative measurement criteria with defined thresholds upon which delivery of supplies is based. KQIs provide indication of the expected quality level at subsequent tests or inspections.

Factory Acceptance Test (FAT) – Test and/or processes detailed by CDRL A034.

Category I Nonconformance – A nonconformance discovered during a Factory Acceptance Test as defined by CDRL

A034.

Category II Non-conformances – Non-conformances found during Government inspection, assembly, disassembly, in-water test, environmental test, or any other Government test or inspection during the warranty period.

7.0 DELIVERY CRITERIA, KQI USAGE and THRESHOLDS

All KQIs established in this QASP are based on non-conformances as documented by CDRL A005 (Failure Reporting, Analysis, and Corrective Action (FRACA) Reports). The KQIs are categorized by the test phase where the nonconformance is found and by which Contract Line Item Number (CLIN) the test article is associated. For a graphical representation of the KQIs, see Figure 1. For KQI thresholds, the tested quantity and the allowable number of open non-conformances are assessed against each production year’s material. For example, tested quantity and Category I or II non-conformances generated while producing and testing CLINs 0103 and 0104 are not applicable to CLINs 0003 and 0004 in terms of KQI thresholds. For a graphical representation of the KQIs, see Figure 1.

Figure 1: KQI Graphical Representation

7.1. CATEGORY I NON-CONFORMANCES

Allowable quantities of open Category I (FAT) non-conformances are dependent upon the quantity of hardware tested.

Table 1 shows the correlation between the allowable number of open Category I non-conformances and tested quantities.

Category I non-conformances are attributed to the following components during FAT testing:

Processor Group Assembly (PGA)

Control Group Assembly (CGA)

The threshold is scaled to the total quantity of CGAs and PGAs tested independently (i.e. if 100 CGAs are tested and 0

PGAs tested, total quantity tested equals 100).

7.2. CATEGORY II NON-CONFORMANCES

Allowable quantities of open Category II non-conformances are dependent upon the total number of kits that have been delivered under each applicable CLIN and tested by the Government. Table 1 shows the correlation between the allowable number of open Category II non-conformances and tested quantities. Allowable open Category II non-conformances and tested quantities are assessed only on hardware in the warranty period. Total quantity tested will be defined by the total number of CGAs tested by the Government. However, non-conformances of all delivered hardware will count against the threshold.

7.3. KQI DELIVERY SUSPENSION THRESHOLDS

Exceeding the KQI thresholds defined in this QASP may be used as cause for suspension of production deliveries, as detailed in Section 3.3.6.3.5 of the contract’s Statement of Work (SOW). Table 1 shows the correlation between allowable open non-conformances and tested quantities.

8.0 DOCUMENTATION

All data and information supporting the requirements of the QASP will be provided to the PCO for inclusion in the contract file.
