1. PURPOSE:
This attachment provides the processes utilized for fault isolation of non-conformances identified by the Government, as well as the means for tracking both Government and Contractor failure analysis efforts to closure.
2. FAILURES IDENTIFIED BY THE GOVERNMENT:
When non-conformances are identified by the Government, either within an Intermediate Maintenance Activity (IMA) or during a Government test event, the Government will document the nonconformance via a failure TMIS (Torpedo Management Information System) within the Automated Process Control System (APCS). The Government will then begin preliminary fault isolation, to determine the party responsible for the failure.
3. FAULT ISOLATION TEAM:
The Government will host and the Contractor will support a Fault Isolation Team (FIT) weekly meeting, in accordance with the contract’s Statement of Work (SOW). The purpose of the FIT is to track nonconforming material that is identified during Government and Contractor testing. The FIT will then determine the appropriate actions to prevent additional nonconforming material. Only Contractor failures reportable under CDRL A005 will be tracked within the FIT meetings.
3.1 FIT RESPONSIBILITIES
The FIT is collectively responsible for:
a. Determining the responsibility of nonconforming material
b. Discussing and guiding failure analysis efforts and corrective actions
c. Determining actions required to prevent processing of additional nonconforming
d. material
e. Ensuring that action is taken to prevent recurrence of the nonconformance
f. Tracking Government failure analysis efforts to closure
g. Tracking Contractor FRACA reports to closure and providing disposition of those
h. Reports
3.2. FIT COMPOSITION:
The FIT is chaired by the Production Acceptance Lead or other PMS 404 appointed Government representative and will include members consisting of the following: Subject Matter Experts (SMEs) from the In-Service Engineering Agent (ISEA), Intermediate Maintenance Activity (IMA), NUWC Keyport’s Production Acceptance team, the Contractor, NUWC Newport, PMS404, NAVSEA02, and Defense Contract Management Activity (DCMA).
3.3. FIT WORKING PROCESS:
New non-conformances will be introduced to the FIT either via a new TMIS documented by the Government, or via new FRACA reports submitted by the contractor. The FIT will discuss preliminary fault isolation of the failure. Once enough information is known about the failure, the FIT Chair, with recommendation from the FIT members, will assign the nonconformance against either the Government or Contractor. This practice results in a list of open FRACA reports and Government TMIS documents that are discussed and tracked to closure during the weekly meetings.
3.4. NON-CONFORMANCE RESPONSIBILITY
3.4.1. CONTRACTOR RESPONSIBILITY
In the event a nonconformance is determined to be the Contractor’s responsibility and the hardware has yet to be final accepted per Section E of the contract, the Contractor shall conduct failure analysis IAW with the FRACA requirements of the contract.
I. GOVERNMENT RESPONSIBILITY
In the event a nonconformance is determined to be the Government’s responsibility, the ISEA or TDA, partnered with the Production Acceptance Engineer (PAE), will determine what actions are necessary to bring the hardware back to a useable condition. The Government will document and track the nonconformance to closure.
