About this file

This is a Mandatory Response Worksheet for a Request for Proposal issued by Henry County, Georgia's Purchasing Department for Invitation to Bid #23-34, which concerns the purchase and installation of equipment for five 2022 Chevrolet Tahoe vehicles designated for the Police Department's K-9 units. The bid submission deadline was 3:00 p.m. local time on February 23, 2023. A mandatory pre-bid meeting was scheduled for January 10, 2023 at 10:00 a.m. Eastern Time via Zoom. Prospective bidders were required to furnish all labor, materials, equipment, and services necessary to complete the installation work on the five vehicles. Bid documents and specifications were available for download from the Henry County Purchasing Department office located at 140 Henry Parkway, Room 209, McDonough, Georgia 30253. West Chatham Warning Devices Inc was ultimately awarded the contract.

The Mandatory Response Worksheet requires all offerors to answer eight pass/fail questions with "YES" responses to be considered responsive and responsible for contract award. These mandatory requirements include completion of the General Information Sheet, Non-Collusion Document, Certification Form, Immigration/Security Form, Tax Compliance Form, and submission of a completed W-9, JCSS Terms & Conditions, and Contractor Affidavit. Failure to answer all questions affirmatively or leaving any question blank results in disqualification from consideration. Offerors are explicitly instructed not to include any cost information in their responses to this worksheet, and any attachments not specifically requested will not be evaluated. The contract was awarded in the amount of $126,006.25, and bids could not be withdrawn for sixty days following the bid closing deadline except as permitted by the Official Code of Georgia Annotated. Henry County reserved the right to reject any and all proposals and to waive any technicalities.
