PART ONE: INSTRUCTIONS
The company who has provided you with this form is proposing on Department of Navy/United States Marine Corps (USMC) solicitation, M67854-14-R-6002, to provide an Abrams Integrated Display and Targeting System (AIDATS) that will meet the mission needs of the Marine Corps. Past Performance is an important part of the evaluation criteria for this acquisition. Your assistance is requested by the USMC to assist with establishing the performance history for the Contractor named below. In efforts to expedite receipt of the requested information, the Contracting Office respectfully requests that you do not mail hard copies. Instead, please e-mail the completed questionnaire(s). Please provide an honest assessment and return the questionnaire to the following Contract Specialist: eliana.york@usmc.mil with copy to PCO: lynn.frazier@usmc.mil.
