(1) Transportation Management, Licensing, Operation, and Utilization of Government Hotor Vehicles Guidelines
1. Purpose. To provide instruction concerning the management, licensing, operation, and utilization of government motor vehicles assigned to Fleet Readiness Center East (FRC East) in accordance with references (a) through (r).
2. Cancellation. NAVAIRDEPOTINST 11200.1D.
3. Summary of Changes. This instruction includes major revision and should be reviewed in its entirety.
4. Policy. The use of all transportation equipment and automotive vehicles assigned to the FRC East will be limited to

that which is required to conduct official business unless health and safety factors are involved. Military and civilian employees who drive or operate a government motor vehicle or industrial equipment must be licensed in strict compliance with reference (a) . The elimination of motor vehicle-related mishaps and the resulting deaths, injuries, and property damage by applying risk management strategies to achieve significant reductions in all mishaps, with the ultimate goal of zero mishaps is policy as is noted in reference (b) .
5. Scope. This instruction and the term "transportation equipment" as used herein are applicable to all driver-operated, self-powered vehicles used by military personnel and DoD civilian personnel in a duty status, on or off a DoD installation as well as all persons in or on a government-owned or -leased vehicle while of this Command. Bicycles, while not self-powered, are specifically included in this category of vehicle.
6. Responsibilities
a. All Departments. All levels of supervision and employees of this Command who operate transportation equipment, whether bicycles, electric or gaseous carts or other self propelled equipment, are responsible for being thoroughly familiar with the contents of this instruction.
7. Instruction Responsibility. The Industrial Plant Services Division (Code 6.3.3) shall maintain this instruction in a current status.
8. Forms Required
a. OF 346, "Motor Operators Identification Card (U. S.
Government)" is stocked by the License Official (M30), located in Building 160. This is a controlled release card issued only in complete form, and only through authorized channels.
Employees shall keep forms on hand when operating equipment and when license expires employee shall return with renewal requests.
b. FRC East 5100/2A, "Property Damage Report" is located on Naval Forms Online. Forms are processed and filed per reference

c. FRC East 11200/2, "Vehicle Maintenance Checklist" is located on Naval Forms Online. Each supervisor shall maintain one year's worth of completed forms for each vehicle assigned to shop.
d. NAVMC 10964, "Application for Government Vehicle Operator's Permit" is located on Naval Forms Online. Completed forms are submitted to the Station Motor Transport Officer (LM), Facility Directorate MCAS where they are filed.
e. OPNAV 5100/11, "Navy Employee Report of Unsafe or Unhealthful lflorking Condition" is located on Naval Forms Online.
Completed forms are forwarded to Industrial Occupational Health and Safety Division, Code 7.10.1. They are processed and filed per reference (d).
f. OPNAV 4790!108, "Support Equipment Misuse/Abuse" is located on Naval Forms Online. Completed forms are forwarded to the SE Misuse and Abuse officer and filed as required in reference (e).
g. OF 345, "Physical Fitness Inquiry for Motor Vehicle Operators" is located on the Naval Forms Online. Completed forms are submitted to the Station Motor Transport Officer (LM), Facility Directorate MCAS where they are filed.
h. FRC East 5102/1, "Mishap Report/Investigation" is located on the Naval Forms Online. Completed forms are submitted to the Integrated Support Division (Code 7.10.1).
They are processed and filed per reference (d) .
i. SF 182, "Authorization, Agreement, and Certification of Training" are located in training system of record. Completed forms are stored on the same database.
j. Form NDR-EMP, "Request for National Driver Register File Check on Current or Prospective Employee" may be obtained from an FRC East Designated Approving Official for Licenses or the Cherry Point License Official (M30), located in Building 160.
Completed forms are submitted to the Station Motor Transport Officer (LM) , Facility Directorate MCAS where they are filed per reference (f).
k. Vehicle Trip Tickets are located on the FRC East Vehicle Dispatch Tracking System and stored electronically on this system as is required per reference (f).
l. SF 91, "Motor Vehicle Accident Report" may be obtained from the Transportation Administration personnel located in Building 188. One is attached to each trip ticket provided by Transportation Administration personnel. The form is available online via the GSA Forms Library. Completed forms can be located in the GSA Vehicle Folders located in Building 188 and shall be stored until removal of vehicle from usage at FRC East.
m. DD Form 518, "Accident-Identification Card" is located on GSA Forms library. A copy is located in the glove box of each vehicle provided by Transportation Administration personnel. Completed copies shall be provided to personnel who are involved in an accident that involves a FRC East motor vehicle as is required per refe~ (f). ~~~~~J

Chapter 1 General Information

It is the policy of the Navy to ensure that all military personnel, civilian employees, and contractor personnel operating vehicles and equipment on a Government installation or operating Government owned vehicles or equipment on or off Government property is properly licensed. Most states now have a classified motor vehicle operator's licensing program that includes appropriate examinations to ensure knowledge of traffic laws, ability to read road signs, vision, and color recognition;
and to ensure by road test competency to operate a specific class of vehicle. This chapter provides general licensing requirements for all Government Motor Vehicles.
1. License Requirements. See Appendix A for training and licensing matrix.
a. Vehicles under 10,000 lbs. Personnel are not required to possess OF-346, "U.S. Government Motor Vehicle Operator's Identification Card," to operate government-owned or leased commercial or administrative use vehicles under 10,000 lb gross vehicle weight (GVVv) , provided they possess a valid State driver's license. The exception will be drivers of tactical vehicles and emergency type vehicles such as ambulances and crash crew who will be required to possess an OF-346.
b. Vehicles over 10,000 lbs. Vehicle operators will have a valid state driver's license that is applicable for the class and type vehicle that will be operated and an OF-346 for operation of industrial equipment. (i.e., trucks and other vehicles over 10,000 lbs, see Appendix A).
C. Commercial Motor Vehicle. As of 1 April 1992, personnel who operate a commercial motor vehicle on public highways must possess a state-issued Commercial Drivers License (CDL) . Title 49 of the CFR, Part 383 defines commercial motor vehicles as:
(1) A motor vehicle that has a manufacturer's gross vehicle weight rating (GVV1lR) or gross combination weight rating (GCif./R) of 26,001 pounds or more, or has a GC\,JR inclusive of a towed unit with a GVVIJR of more than 10,000 pounds; or
(2) A motor vehicle designed to transport 16 or more passengers, including driver; or

(3) A motor vehicle of any size and used in the transportation of materials found to be hazardous for the purposes of the Hazardous Materials Transportation Act and which require the motor vehicle to be placarded under the Hazardous Materials Transportation Regulations (49 CFR Part 172, Subpart F).
The following personnel are waived/exempted from the Act's requirements and are not required to obtain/possess a CDL:
(1) Military personnel on active duty who operate equipment owned or operated by the Department of Defense in pursuit of military purposes.
(2) All operators of fire fighting and other emergency equipment as well as operators of farming equipment.
The CDL must have a "P/B" endorsement in order to be valid to drive passenger vehicles (16 people) or bus. The CDL must have a "C" endorsement in order to be valid for transportation of hazardous material. See Appendix A.
d. Hazardous Material and Explosives Motor Vehicle Operators. Operators qualified to transport dangerous cargo must possess a medical certificate and an OF-346 on which is noted "Explosives Driver". See Appendix A.
e. Operation of Vehicles with Emergency Warning Devices.
Personnel must have a valid state driver's license, an OF-346 and a medical certificate for Special Purpose Vehicle usage (712 Tactical/720 Explosives (including chemicals).
f. Forklift Operators. Personnel must have a valid state operator's license and an OF 346 with the type and maximum load capacities of the forklift(s) they are trained and certified to operate listed. Operators qualified for Material Handling Equipment (MHE) must possess a medical certificate. See Appendix A.
g. Man-lif Personnel must have a valid state operator's license and an OF 346 with the maximum height they are trained and certified to operate. Operators qualified for ~Ieight Handling Equipment (villE) must possess a medical certificate. See Appendix A.
h. Department of Defense (DOD) Contract Personnel.
Contracts and agreements will require that operators comply with licensing requirements of the state and local motor vehicle laws. DoD contract personnel shall not be issued OF-346 as is stated in reference (g) . DoD contract personnel assigned to operate either government owned/leased equipment in performance of contract shall be certified, by the contractor and at contractor's expense, as being fully qualified to operate the vehicles/equipment to which they are assigned. The prime contractor shall document all operator qualifications. This documentation shall be provided to the contract administrator prior to any contract employee engaging in any mode of equipment operation. The contract administrator shall retain documentation as is required in reference (h) .
2. Scheduling of Physicals
a. Initial Physical. Initial physical examinations must be requested by the employee supervisor via the division office who will then request the required physical through the Integrated Support Division (Code 7.10.1). Hazardous Materials Drivers require a code 711 physical prior to submitting a license application. Employee's who require a DOT CDL Drivers license for government licensing must complete a code 706 physical.
Employee's who require a forklift operator's license must complete a code 710 physical prior to submitting a request for an OF 346. Employee's who require a self-propelled Aerial Lift/Scissor Lift, Mobile Crane or any other Weight Handling Equipment on a government license must complete a code 704 physical. A medical examiner's certificate for drivers shall be provided by the Occupational Medicine Clinic and a copy is required to accompany all government licensing applications.
b. Physical Exam Frequency. Once the initial exams have been scheduled, the employee's division point of contact for physical exams will be notified when reexaminations are due.
Supervisors will be required to notify the division point of contact when an employee no longer requires government licensing medical examinations. Government licenses are only valid when the proper medical examiner's certificate for the specific equipment is in the possession of the driver.

Code 7.10.1 will distribute a cation letter to the supervisors of medically l-3 l 6 disqualified government vehicle operators when a medical disqualification is determined by the Occupational Health Clinic. Supervisors will ensure that medically disqualified employees do not operate government vehicles and that any previously issued government licenses along with a copy of the disqualification letter is forwarded to a FRC East Designated Approving official so that it may be turned in to the Station Motor Transport Officer.
4. Designated Approving Official for License Issue. The FRC East Commanding Officer shall assign in writing FRC East designated officials to sign required documents for licensing of depot personnel operating transportation equipment as is required in MCO 11240.66. The designation letter will be addressed to Commanding General, Station Motor Transport Officer (LM), Facility Directorate MCAS, PSC Box 8003, Cherry Point, NC 28533-003 as is required per reference (i).
5. New License Request. The proper selection of personnel to be licensed as motor vehicle operators is of utmost importance.
Maturity, decorum, neatness, and stability are some of the prime requisites to consider. Supervisors shall ensure that only employees who meet prime requisites and have been determined as physically capable are permitted to apply for government licensing.
a. Submission of Application for Government Vehicle Operator's Permit. The applicant completes Part I of NAVMC 10964, Application for a Government Vehicle Operator's Permit, which is located on Naval Forms on-line. Item 4; will include FRC East, applicants office symbol and shop phone number.
Supervisor's will observe operators driving habits and will only forward sign item 17 of the NAVMC 10964, when it is demonstrated that the operator follows all requirements that qualify as knowledgeable and safe operation of the equipment. A designated Approval Official for license issue will also sign and date item 17 once the employee's supervisor has signed and submitted. If applicant is Civil Service employee, item 17 is to be signed and dated by their responsible supervisor. The designated official of FRC East will complete Part II and then sign/date item 25.
b. Submission of Request for National Driver Register File Check. The applicant completes the center portion of Form NDR-
EMP, "Request for National Driver Register File Check," which is located on Naval Forms on-line. This form must be notarized prior to submission.
c. Submission of OF-345, "Physical Fitness Inquiry for Motor Vehicle Operators." The applicant completes items 1-10 of the Physical Fitness Inquiry for Motor Vehicle Operators, OF- 345, which is located on Naval Forms on-line. FRC East designated official will complete the Review and Certification by Designated Official Portion of this form.
d. FRC East Designated Approving Official Processing of Forms. The FRC East Designated Approving Official will verify that the applicant has completed all forms as is required.
NAVMC 10964 Part II and Part II up to and including item 26 must be filled out by the Approving Official. The OF 345, Review and Certification by Designated Official portion must be completed and signed by the Approving Official as well. A copy of the applicants State Drivers License (both front and back), copy of all required medical examiner's certificate, copy of training/refresher certificates and any previously issued OF- 346, U.S. Government Motor Vehicle Operator's Identification Card. All forms will be forwarded to Station Motor Transport Office Licensing Office located in building 160. Once the designated approving official is notified by Station Motor Transport that the applicant's OF-346 is completed, they will pick up all documents, notify applicant that license can be picked up, have applicant sign OF-346 and make a copy of the signed OF-346 for the Station Motor Transport Licensing Office to file. Any copies of submission package forms returned from the Station Motor Transport Licensing Office will be returned to the applicant to provide to his supervisor for filing.
6. Government Vehicle Operators Permit/Certification Validity, Currency and Revocation
a. Validi Supervisors will ensure employees have a current government license/certification to operate the specific type of vehicle prior to assigning the vehicle to the driver. Applicable safety rules and regulations will be enforced that pertain to the operation and authorized use of transportation equipment.
b. Vehicle Operator Permit Certification Suspension or Revocation. Division directors are responsible for revoking or l-5

suspending an operator identification card, but immediate supervisors are required to forward any information that will assist the division director in making decisions. Suspension or revocation of the operator's permit/certification may be taken separate from any actual or contemplated disciplinary action for the same offense as is documented in reference (j). Suspension or revocation of the operator's permit will normally be taken for, but is not limited to, the following reasons:
(1) The operator proves to be incompetent to operate the equipment.
(2) The operator has demonstrated flagrant disregard for the rights and safety of others by acts of recklessness, speeding, or disobeying traffic regulations and signs.
(3) The operator fails to comply with the requirements of this instruction.
(4) Medical or state driver's license requirements are not met.
(5) Base driving privileges have been suspended.
(6) Felony involving the use of a vehicle (One-year mandatory suspension) .
(7) Conviction of Dh!I or driving with a blood alcohol content of .08 percent or greater (One-year mandatory suspension).
(8) Any other reason deemed sufficient by the immediate or higher level supervisor/manager.
(9) As recommended by the Quality Management Division (Code 6.4.1) Support Equipment Misuse/Abuse Program Manager, equipment misuse and abuse or other government agency recommendation, (i.e., security or safety). A suspended OF-346 can be reinstated after a predetermined designated period, but a revoked OF-346 shall not be restored until it is determined the operator is fully qualified through evaluation, re-examination, and testing. All revoked or suspended licenses shall be returned to the Station Motor Transport Licensing Office immediately.

7. Unofficial Use of Transportation Equipment. Penalties for willfully using or authorizing the use of a motor vehicle for other than official purposes are contained in reference (f).
Use of other government-owned vehicles for transportation of equipment (including, but not limited to bicycles, tractors, forklifts, golf carts, or platform trucks, etc.) will not be used or authorized for other than work related requirements.
Improper use of any type of vehicles that is not covered under reference (f) is considered to be unauthorized use of government property and subject to disciplinary action.
8. General Services Administration. General Service Administration (GSA) vehicles will not leave the Air Station fence line without a trip ticket and required accident forms.
Trip tickets and SF 91, "Motor Vehicle Accident Report," will be issued by Transportation, Building 188. The SF 91 will be utilized for all accidents that occur involving a GSA vehicle and will be turned into Transportation.
9. Reporting of Violations
a. Violations Noted Internally. Any supervisor or employee observing any violations of this instruction or any negligent or unsafe practice on the part of the operator that endangers employees or equipment is responsible for reporting the violations to their immediate supervisor. The immediate supervisor will report the violation to the Quality Management Division (Code 6.4.1) Support Equipment (SE) Misuse/Abuse Program Manager verbally, followed by initiation of the OPNAV 4790!108, "Support Equipment Misuse/Abuse" located in Naval Forms On-line as is noted in reference (e) .
b. Federal or State Violations. Whenever a government employee is served with federal or state court civil or criminal process pleadings (including traffic tickets) arising from actions performed in the course of official duties, the employee shall immediately deliver all process and pleadings served upon them to their supervisor as is required in reference (j). An investigation to ascertain the facts surrounding the incident will be accomplished and the Government will then take appropriate action in accordance with JAGINST 5822.2. The government employee will be advised concerning all rights to remove civil or criminal proceedings from State to Federal Court.

10. Accident Reporting. All operators involved in an accident shall:
a. Stop immediately.
b. Assist the injured and call 911. Do not move injured people unless absolutely essential for their protection.
c. \\Tarn other motorists/operators of any existing highway hazard. During hours of darkness or poor visibility, flares or reflectors shall be used.
d. Do not leave the scene of the accident except as authorized by a State law enforcement officer, FRC East security or other proper authority. Notify iFmediate supervisor if within FRC East areas.
e. Complete SF 91; if the SF 91 cannot be prepared by the driver because of injury or death, the report will be completed by the next senior person responsible for equipment.
Expeditiously deliver completed SF 91 to the motor transport officer.
f. Do not express oral or written opinions to claimants or their agents concerning liability, investigation findings, or possibility of a claim approval.
g. Comply with State and local laws governing the reporting of motor vehicle accidents. Official reports shall be submitted through channels to the appropriate claims officer for review to ensure that U.S. Government rights are not prejudiced by an admission of liability for the Government.
h. Do not provide official accident investigation reports to a claimant or any individual or representative of any non-DoD organization, including State or local officials, without first obtaining clearance. Obtain clearance from FRC East Counsel Group (Code 11.0).
i. If the accident occurs outslae Jv!CAS Cherry Point, complete DD Form 518, Accident-Identification Card, at the scene of the accident, or as promptly as possible thereafter, and provide to all persons directly concerned.

11. Enforcement of Safety Rules and Regulations. All levels of supervision and operators of transportation equipment, including bicycles, will adhere to all safety rules and regulations pertaining to their operation and proper use as contained in this instruction, and/or as established by this Command, relative to the movement and control of vehicular traffic, the safety of personnel, the prevention of accidents, and the prevention of damage to government property.

a. Cross roads at intersections or crosswalks only.
b. Walk on the left side of the road facing oncoming traffic if there are no sidewalks available as is required in reference (b) .
c. Comply with orders of Military Police or traffic control personnel.
d. Do not enter any crosswalk or roadway when vehicle traffic is so close that it constitutes an immediate hazard to pedestrians or vehicles.
e. Do not stand on any roadway to solicit a ride in any vehicle.
f. Cross railroad tracks at designated crosswalks only. Do not cross in front of approaching train. Never climb over, under or through stopped railroad cars.

CHAPTER 3
BICYCLE SCOOTER

Bicycle and scooter operators are exempt from the requirements of operator's licenses. Bicycle riders will obey all applicable traffic regulations, signals, signs, and devices governing the operation of motor vehicles, except when dismounted at which time pedestrian regulations apply as is stated in references (b), (k), and (l). All requirements listed in this chapter are located in references (b), (k), and (l).
1. Bicycle/Scooter Safety Requirements
a. Bicycles are permitted only on streets, roads, and parking areas NOT inside of any buildings, or on lawns, or sidewalks. Handicapped scooters and three wheeled bicycles are permitted inside of buildings as long as special care is taken and all safety rules are complied with. Bicycles and scooters will be ridden with the traffic and kept to the right edge of all roadways.
b. The use of portable headphones, earphones, cellular phones, iPods, or other listening and entertainment devices (other than hearing aids and work required communication means) are prohibited while walking, jogging, running, or driving.

d. One hand must be on the handle bars at all times. Ride as near to the right side of the roadway as practical and ride in single file only.
e. Bicycles/scooters will neither be towed by another vehicle, nor tow another vehicle except trailers manufactured for that purpose.
f. Bicyclists shall not operate bicycles without an approved and properly fitted and fastened bicycle helmet. Two exceptions to this requirement are:

( 1) \1/orkers operating bicycles in indus trial/ flight line areas designated no-hat area due to potential of FOD are not required to wear a bicycle helmet.
(2) Operators of three-wheeled bicycles or scooters are not required to wear a bicycle helmet.
g. Bicycles and scooters must be equipped with:
(1) A crystal (white) reflector of at least 9 square inches facing forward and unobstructed when viewed from the front.
(2) An amber reflector with a minimum of 9 square inches on each side of the bicycle/scooter facing outward.
(3) Amber reflectors on both front and rear surfaces of all pedals.
(4) A red reflector with a minimum of 9 square inches facing rearward and unobstructed when viewed from the rear.
(5) For night time riding, bicycles must also have a functioning headlight beam of 30 feet and a tail light or red reflector.
h. Privately owned bicycles should be registered with the Provost Marshall Office (PMO). Lost or missing bicycles as well as found bicycles should be reported promptly to military police. To deter theft, all bicycles should be secured with a bicycle lock while not in use.
l. Dismount and walk the bicycle when inside buildings, passing through pedestrian gates, and on pedestrian bridges or on walkways.

k. Do not enter any crosswalk or roadway when vehicle traffic is so close that it constitutes an immediate hazard to pedestrians or vehicles.
l. Bicycles may be ridden at night only if properly outfitted and the bicyclist is wearing a reflective upper garment as follows: reflective material sewn or ironed on to a garment or a commercially available mesh/fabric vest that is bright yellow, international orange, or lime green with at least two 1.5 to 2 inch wide vertical or horizontal retro-reflective strips going from front to back.
m. All plant equipment, prior to usage, requires a preoperational check. FRC East furnished bicycles/scooters are plant equipment and require a visual preoperational check and compliance with (m). Operational Risk Management fundamentals will be utilized by all bicycle and/or scooter operators to ensure safe operation. See reference (n) for specifics.
n. Three wheeled bicycles and scooters when operated inside of a building shall follow directional traffic arrows for one way traffic in aisle ways.

Chapter 4 Electric/Gas Cart Requirements
The Navy requires that commands using golf carts establish vehicle standard operating instructions, develop a map of authorized travel routes, perform vehicle inspections, conduct operating training and mark units per references (f), (n) and ANSI ITSDF B56.8-2006, Items 7.1 and 7.2.2. Cart operators shall maintain a valid state driver's license on their person while using a cart. Training is required prior to authorization of cart usage. No additional medical certification is required.
This chapter ensures these requirements are met.
1. Vehicle Standard Operating Instructions
a. All cart operators shall read and follow the operator's manual for their specific cart.
b. All cart operators shall follow all motor vehicle traffic regulations as well as all requirements listed within this instruction.
c. Riding in a cart by individuals other than the operator is authorized only on personnel seat(s) provided by the manufacturer.
d. Prior to carts being left unattended (see definition of unattended in Chapter 10), the following actions are required:
(1) Stop the cart;
(2) Place directional controls in park or neutral;
(3) Apply the parking brake;
(4) Stop engine or turn off power;
(5) Turn off control or ignition circuit;
(6) Remove key (if provided); and,
(7) Block/chock the wheels if cart is on an incline.
e. Routine battery service checks are required for electric carts. Operators shall take carts to Ground Support

Equipment/IRML/Asset Management Branch (Code 6.3.3.4), located in Building 154 to have batteries checked and serviced. Battery checks should be completed on Friday's. Cart usage and manufacturer's recommendations affect the frequency of battery service checks. Accomplish the checks on a weekly basis provided a different frequency is not suggested by Ground Support Maintenance.
f. If cart comes equipped with seat belts, they shall be worn.
g. Only use cart authorized travel routes as per enclosure (1), Appendix B of this instruction. Yellow paths are routes preferred. See Chapter 10, items 15-17, 19 and 23. Red paths may be utilized if yellow path directions cannot be complied with. Chapter 10, Item 16 compliance will be required.
h. Do not pass another cart or vehicle traveling in the same direction at intersections, blind spots, or other dangerous locations.
i. Carts are prohibited from crossing pedestrian bridges.
j. Carts are prohibited from driving on sidewalks and on pedestrian crosswalks.
k. Electric carts shall only be charged in outdoor areas designated as cart recharging areas.
1. Tie down/secure materials to the bed of the cart if possible they may become flying debris or if they are unstable without additional securing methods.
m. Follow manufacturer's guidelines for load capacity.
n. Avoid material loads exceeding the physical dimensions of the cart.
o. Accomplish preoperational check utilizing FRC East 11200/2 as is required of all government vehicles.
p. If cart was originally provided with doors, it shall be operated with doors installed and closed properly. If doors are missing, cart shall be removed from service until such time as doors can be installed.

q. If during summer months, heat hazards exist and doors were made to be safely removed, Code 7.10.1 may authorize exemption from door requirement in paragraph above. Any exemptions will be granted on a model by model basis.
r. All FRC East carts will only be used by trained and authorized individuals.
s. Code 7.10.1 will maintain a list of authorized instructors for Carts and will maintain and distribute training materials as required.
t. Training shall be documented in the training system of record.
u. Code 6.3.3.4 will pursue disposition of the cart whenever any repair cost exceeds one third of the current value of the cart.

Chapter 5 \~eight Handling Equipment (CvHE) Requirements
Weight Handling Equipment such as Loaders, Sweepers, Cranes, Man-lifts/Aerial Platforms require a periodic weight handling physical examination by a medical officer and issuance of U.S.
Government Motor Vehicle Operator's Identification Card, OF-346 prior to equipment usage, reference (f). United States Department of Labor states, Aerial lifts are vehicle-mounted, boom-supported aerial platforms, such as pickers or bucket trucks. United States Department of Labor, Occupational Safety and Health Administration differentiate aerial platforms in two basic categories; Self-propelled and Non-self-propelled.
Specific training is required for both. Licensing via OF-346, "U.S. Government Motor Vehicle Operator's Identification Card," is required for Self-propelled Aerial Platforms and Platforms.
Scissor lift and Mobile Scaffolding requirements are covered by the Navy Fall Protection Guide and are not included in this instruction. Mobile Cranes requirements are covered by references (e) and (k).
1. Weight Handling Equipment Physical. All weight handling equipment require completion of a weight handling equipment physical, code 704 every two years. The medical certification card verifying completion of this physical is required prior to licensing and must be kept with the driver while operating the equipment.
2. Weight Handling Equipment Training/Testing. Training and testing must be accomplished prior to issuance or due to suspension or revocation of OF-346 for all vveight Handling Equipment. Training must be specific to the piece of equipment.
See reference (f), 3.8.4 for additional information.
3. Preoperational Checks. Preoperational checks for Weight Handling Equipment are completed using FRC East 11200/2, "Vehicle Maintenance Chacklist" which is available on Navy Forms on-line. An exception to this is Mobile Cranes which are governed by the references listed below.
4. Mobile Cranes. Mobile Cranes training and licensing fall into one of two categories. Mobile Cranes that are considered support equipment per reference (e) will be licensed and trained per the reference. Mobile cranes that are not considered support equipment will follow guidelines listed in reference

(o). All mobile cranes require the weight handling equipment physical as listed in this chapter and reference (f).
5. Man-lifts/Aerial Platforms
a. Federally approved fall protection equipment is required whenever operating scissor lifts, man-lifts and aerial platforms. Fall protection training is required prior to using fall protection equipment in addition to the training required that is specific to the use of man-lifts and aerial platforms.
Fall Protection Training shall comply with reference (p).
b. Always be aware of site hazards (uneven ground, power lines, pot holes) that could cause aerial platforms and man lifts to tip or become energized.
c. ~mere there is a danger of tools, materials, or equipment falling from a man-lift or aerial work platform, the area below the scaffold to which objects can fall shall be barricaded, and employees shall not be permitted to enter the hazard area.
d. Be aware of everyone in the immediate area when lowering your boom.

f. Only one designated person shall operate the platform at any time. This designated person is the "captain" of the platform and shall be obeyed while the platform is in operation, with the following exceptions.
(1) In an emergency situation when a ground operators assistance is required.

g. Never use fall protection gear to tie off to an adjacent structure. Movement of the man-lift or aerial work platform could result in serious injury or death.
h. Never use ladders, planks, ropes or any other device to extend your reach or provide greater height.

l. If your man-lift or aerial work platform is equipped with outriggers, extendible axles, or any other form of stabilization device, use it according to manufacturer specifications.
j. Never travel without wearing your fall protection devices and being tied into the appropriate point using the approved lanyard. Whenever possible travel with the platform in the down and stowed position.
k. Never operate any man-lift or aerial work platform without first reading the operator's manual. It is, by law, required to be on all such devices. It is the operator's responsibility to read, understand, and use the specific instructions in the operator's manual.
l. The number one cause of injury or fatalities involving man-lifts and aerial platforms is contact with electrical wires.
Maintain a minimum clearance of at least 10 feet from the nearest live power lines and Category III crane (Hoist) rails.
m. Do not exceed the load limits of the equipment. Allow for the combined weight of the worker, tools and materials.
n. Ensure that a ground operator who has received instruction on the lifts emergency ground controls, remains within site of the equipment in case of unexpected failure of platform controls

Chapter 6 Material Handling Equipment (MHE) Requirements
Material handling equipment is equipment that relates to movement, storage, control and protection of materials, goods and products throughout the process of manufacturing, distribution, consumption and disposal. Material handling equipment is generally separated into four main categories;
storage and handling equipment, engineered systems, industrial trucks, and bulk material handling. This instruction covers the only the industrial trucks portions of weight handling equipment. Industrial trucks usually refer to operator driven motorized warehouse vehicles, powered manually, by gasoline, propane or electricity. Forklift trucks are the most common example of industrial trucks but certainly are not the extent of the category. If the equipment being used is self-propelled and the operator rides upon the vehicle it falls into the class of forklifts requiring an OF-346 (i.e. rider lift trucks). Tow tractors are an additional example of weight handling industrial trucks which are covered and licensed per reference (e).
1. Material Handling Equipment Physical. All material handling equipment requires completion of a material handling equipment physical, code 710 every three years. The medical certification card verifying completion of this physical is required prior to licensing and must be kept with the driver while operating the equipment. Operators of material handling equipment shall also hold a valid state operator's license and U.S. Government Motor Vehicle Operator's Identification Card, OF-346 with the type and load capacities of the forklift they are trained and certified to operate listed on the back.
2. Material Handling Equipment Training/Testing. Training and testing must be accomplished prior to issuance or due to suspension or revocation of U.S. Government Motor Vehicle Operator's Identification Card, OF-346 for all Material Handling Equipment. Training must be specific to the piece of equipment.
See reference (f), 3.8.4 for additional information. Forklift operators are required to be recertified every three years.
Supervisor's will observe operators driving habits and will only forward a NAVMC 10964, Application for a Government Vehicle Operator's Permit, when it is demonstrated that the operator follows all requirements that qualify as knowledgeable and safe operation of the equipment. Any operator that is found to have questionable operating practices will not operate the equipment
6-1 until after additional successful completion of the Fleet Readiness Center East Forklift certification training. All operators who recertify shall receive updated training on the requirements stated in this instruction and this recertification will be documented in EMMA under course code 5810102.
3. Additional Training Requirements. All forklift operators who handle aircraft components shall complete and document completion of Parts and Material Handling training located on FRC East's Intranet Page on an annual basis. This training will be documented in EMMA under course code 4510016. Each operator shall also demonstrate their knowledge of proper securing of loads via truckers hitch knots, packaging and protection materials as part of Parts and Material Handling training. Any operator who may require additional training on either of these items will not operate the equipment. On-the-job hands-on training needed on these items may be accomplished by instruction provided by personnel knowledgeable in these practices in Production Activity Control, Code 6.3.2.
4. Preoperational Checks. Preoperational checks for Material Handling Equipment are completed using FRC East 11200/2 which is available on Navy Forms on-line.
5. Forklift Standard Operating Instructions
a. Only trained and authorized operators shall be permitted to operate a forklift. Never operate any forklift unless you are fully qualified for the specific model of forklift you are driving.
b. Operators shall insure when forklifts are parked that the forks are tilted forward and lowered and the vehicle is not blocking fire exits, walkways, elevators, stairways or in an area that would hamper fire-fighting equipment.
c. When the load being carried obstructs forward view, forklift operators shall be required to travel with the load trailing.
d. An elevated load shall not be tilted forward except when the load is in a deposit position over a tack or stack.
e. Operators shall keep forks no more than 4" off deck when moving with empty forks. 1i11hile carrying a load, the load
6-2 engaging means/forks will be placed under the load as far as possible; the mast will be carefully tilted back to stabilize the load. Operators will insure loads are balanced and maintain 4" clearance off the deck when traveling.
f. Check to verify that the floor being traveled on is strong enough to hold the combined weight of the operator, the forklift and the load. This may include removing floor debris to fully inspect floor strength.

Chapter 7 Explosives and Hazardous Material
All Navy and Marine Corps military, civilian and contractor personnel shall be qualified and properly licensed to operate motor vehicles transporting ammunition, explosives and related hazardous materials (A&E) . License requirements differ between civilian, military and contractor personnel. Regulations established by references (f) and (q) are to be applied to all applicants of an OF-346 where operators shall be engaged in transporting ammunition, explosives, or other dangerous/hazardous materials. A summary of basic licensing requirements for use of the U.S. Government Motor Vehicle Operator's Identification Card, OF-346 as related to explosives drivers is presented in reference (q). All drivers of government owned motor vehicles used for transporting A&E on and off base shall possess a valid OF-346 card, with the explosives driver endorsement. If civilian drivers are required to transport A&E off base, a CDL Hazmat endorsement on a valid state driver's license is required.
1. Ammunition ives and Related Hazardous Materials Physical. Physical examination for E&A are required every two years until the age of 60 and then annually thereafter. The medical certification card verifying completion of this physical is required prior to licensing and must be kept with the driver while operating the equipment. Operators of material handling equipment shall also hold a valid state operator's license and an OF 346 with "Explosives Driver - Must hold a current Medical Certificate" annotated.
2. Ammunition osives and Related Materials Personnel who transport, certify or prepare and load A&E for shipment must receive formal initial and refresher training per 49 CFR 172.704, DOD regulations and reference (q). Training will be provided by the station Joint Safety Office, Explosive Ordinance Office, 466-2730. A certificate of training will be required for all licensing.
Chapter 8 Commercial Motor Vehicles

Vehicles over 26,000 lbs. and/or Passenger Vehicles (16 People or more) fall under the category of Commercial Motor Vehicles and require a Commercial Drivers License (CDL). The CDL Program (Public Law 99-570) was established by the Commercial Vehicle Act of 1986. The Act applies both to interstate and intrastate drivers. The Act requires that drivers of commercial vehicles:
(1) Have a single driver's license; (2) be tested for the knowledge and skills needed to drive a commercial motor vehicle;
and (3) be disqualified from driving a commercial motor vehicle if the driver commits certain criminal or traffic violations.
See enclosure (1) for specific requirements. Licensing via U.S.
Government Motor Vehicle Operator's Identification Card, OF-346 is required for commercial motor vehicles. A copy of a valid state CDL, and required physical certificate is required prior to issuance of the OF-346 to civilian personnel.

Training and testing must be accomplished prior to issuance or due to suspension or revocation of U.S. Government Motor Vehicle Operator's Identification Card, OF-346 for all Commercial Motor Vehicles. Training must be specific to the piece of equipment.

Each individual driver (military and civilian) shall meet these additional requirements and fully understand the operational peculiarities of an emergency vehicle and shall be issued an OF- 346 (except contractors) regardless of where the vehicle is operated and the type, size and weight of the vehicle.
