It uses a rating scale from Poor/Unsatisfactory to Exceptional. Key evaluation areas include program planning/control, technical services quality, safety/mission assurance, risk management, cost control, schedule adherence, workforce management, and small business utilization. The form requires detailed contract information, performance ratings, narrative explanations, and specific information about contract values, personnel turnover, safety incidents, and organizational conflicts of interest. The reference must provide their contact information and role in evaluating the contractor's performance.
