About this file

This document is a Past Performance Questionnaire for a potential contractor being considered to replace the fire suppression system in Hanger 3211 at Joint Base McGuire-Dix-Lakehurst, NJ. The form is designed to evaluate a contractor's past performance across 14 key performance areas, using a 0-5 rating scale with options for supporting information and remarks.

The questionnaire covers critical performance aspects including management capabilities, quality control, problem-solving, project supervision, maintenance professionalism, cooperation with government personnel, and overall contract performance. The document allows the previous project point of contact to provide detailed feedback on the contractor's performance, including whether they would award another contract to the same contractor, if any discrepancies occurred, and if subcontractors were paid in a timely manner. The form is intended to be completed and emailed to Wanda Wilson (wanda.wilson.1@us.af.mil) and Kristen Rodgers (kristen.rodgers.2@us.af.mil) from the Air Force contracting office.
