About this file
This is a Past Performance Questionnaire template for use by previous customers of contractors bidding on solicitation AOCSSB26Q0038 for Blanket Purchase Agreements (BPAs) for uniforms with the Architect of the Capitol (AOC). The form is submitted by bidding contractors to obtain performance references from past government or commercial clients, with responses to be returned directly to the Contracting Officer, Roman Davydov, at roman.davydov@aoc.gov by the solicitation deadline. The questionnaire requests detailed evaluations across 16 performance areas using a five-level confidence rating scale (Substantial, Satisfactory, Limited, No, or Unknown Confidence), including assessments of delivery schedules, quality control, work quality, pricing reasonableness, labor and safety compliance, management conduct, customer satisfaction, team relationships, and subcontractor coordination.
Respondents are instructed to provide supporting information and specific examples of performance deficiencies if ratings fall below Satisfactory levels, with responses treated as source selection sensitive information protected under FAR 42.1503. The questionnaire also requests an overall rating and any additional comments, with provision for additional documentation as needed. This template is designed to gather comparative past performance data for the multiple-award BPA solicitation, which anticipates awarding up to two agreements for uniforms (NAICS 315990) with a one-year base period and four one-year options totaling five years, contingent on contractor qualifications and best value determination.
