About this file
This is a Past Performance Questionnaire template designed to evaluate contractor suitability for the Library of Congress Wayfinding Signage fabrication, delivery, and installation contract (Solicitation 030ADV26R0030). The questionnaire is distributed to current or previous customers of prospective contractors to provide honest assessments of their performance capabilities, with responses due to Ramon Samuel at rsamuel@loc.gov by May 27, 2026 at 12:00 PM ET.
The questionnaire requests evaluations across ten performance dimensions using a five-point scale (Outstanding, Good, Satisfactory, Marginal, Unsatisfactory, or Not Applicable). Assessment areas include overall contract management, quality of products and services, project phase management, schedule compliance and timeliness, report submission, subcontractor performance, key personnel competency and communication, cost control, and contract requirement completion. Respondents must provide narrative explanations for any Marginal, Unsatisfactory, or Not Applicable ratings and are asked to supply general company information including point of contact, telephone number, contract number, contract type, period of performance, dollar value, and contract description. Past performance is identified as a considerable factor in this acquisition, making customer feedback critical to the proposal evaluation process.

PAST PERFORMANCE QUESTIONNAIRE
The purpose of this questionnaire is to gather information on the contractor named below to determine their suitability to provide services for the Library of Congress (Library). Your prompt completion of this questionnaire is sincerely appreciated as it is critical to the completion of the proposal evaluation process.
Contractor: (Provide Contractor’s Name)
1. Please complete this questionnaire based on the following guidance:
a) Handwritten responses are acceptable.

RATING
DEFINITION
Outstanding
Performance meets contractual requirements and exceeds many to the Government's benefit. The element being assessed was accomplished with few minor problems for which corrective actions taken by the contractor were highly effective.
Good
Performance meets contractual requirements and exceeds some to the Government's benefit. The element being assessed was accomplished with some minor problems for which corrective actions taken by the contractor were effective.
Satisfactory
Performance meets contractual requirements. The element being assessed contains some minor problems for which corrective actions taken by the contractor appear or were satisfactory.
Marginal
Performance does not meet some contractual requirements. The element being assessed reflects a serious problem for which the contractor has not yet identified corrective actions.
Unsatisfactory
Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The element being assessed contains a serious problem(s) for which the contractor's corrective actions appear or were ineffective.
2. Please provide narrative explanations for any Marginal, Unsatisfactory or No ratings and supplement your knowledge of the contractor’s performance with judgement of others in your organization.
3. The Contractor submitted a proposal to the Library for the provision of Wayfinding Signage fabrication, delivery and installation services. Past performance is of considerable importance to this acquisition. Therefore, input from current or previous customers of the contractor is very important.
