Workers' Compensation policies must include a Waiver of Subrogation Endorsement in favor of the City and its representatives, or a Workers' Compensation Waiver if the contractor is self-employed with no employees. Contractors must submit a copy of the Declarations and Policy Endorsements pages for the General Liability policy along with the Certificate of Insurance, and copies of all Additional Insured Endorsements must be attached to verify proper policy endorsement.
