MEMORANDUM FOR: T-5 Potential Offerors, HT940220R0005
SUBJECT: T-5 Draft Request for Proposal, Section L update, Data Sets
Section L of the T-5 Draft Request for Proposal are restated here for industry consideration.
1. As stated in the T-5 Draft Request for Proposal, Section L 3.3. states “Detail level data on purchased care costs, direct care workload, eligible beneficiaries, and pharmacy workload may be ordered by potential Offerors for a processing fee of $2,500 that will include the original data set and updated data sets, if any, for the acquisition. The Government encourages offerors to consider this invitation for access to this detail level data. Offerors who wish to participate should initiate the process by sending a cover letter and a check for the administrative processing fee of $2,500.00 made payable to the U.S. Treasury.
The cover letter shall include shipping instructions and an administrative point of contact for your organization.

2. Upon receipt and processing of the administrative processing fee, as well as validation of any applicable DHA Data Sharing Agreement and other associated documents if applicable which may include a Business Associate Agreement, HIPAA Security Review and a De-identification plan.
Once the DHA Privacy Office gives approval, we will ship a USB Hard Drive loaded with the healthcare data set.
