1C2.2.2.3“Alternatively, the contractor shall, if requested by the SC at least 2 weeks prior to the group PHA event, have a BHS readily available via a toll free number during the event.” Does this refer to a BHS who would be available at the Call Center or a BHS who is actually present in, and licensed in, the state in which the event is occurring? This paragraph refers to having a BHS available at the Call Center.
2Will the government provide an estimate of the number of MHAs that are conducted per year? The best data from which to estimate work load is that for the period January 1, 2012 through April 22, 2012 there have been 16,690 MHAs conducted.
3Will contractor be allowed to integrate with SM e-mail accounts to post information and alerts for healthcare screening, immunization news, and reminders? No.
4C.9.2.1Section C.9.2.1 indicates that OCI may exist with TMA contractors that currently perform the specified support. Will the OCI exist should the contractor have "recently" (within a year) performed such work?As stated in C.9.2.1 it is incumbent on all Offerors to use sound business judgment and determine the significance of any conflict of interest.
5C.9.2.1Section C.9.2.1 indicates that OCI may exist with TMA contractors that currently perform the specified support. Will those contractors having current work under TMA Contract WXIXW11-08D (TEAMS) be considered an OCI risk?See question 4.
6L.1.11Contractor Support notes contractor employees (whose company is also a TEAMS contractor) involved with this TMA acquisition, , have signed non-disclosure agreements restricting information received in response to this RFP. Given that the contractor is a competitor of my company on other contracts what provisions are in place to protect us from their gaining an unfair competitive advantage on other TMA or similar competitions?C.9 explains in detail what contractors are required to do, particularly C.9.2.3
7F.dThere are 126 reports listed under 47 deliverables, are the reports hard copy or electronically submitted? If electronically, does the government have the electronic format or template? Can the government provide copies of the required reports to determine required level of effort? These reports are electronically submitted. The information required within the reports is provided in PWS. The current templates are not required, nor provided so that offerors can use their own systems and ideas.
8C.2.1.3On page 5 of 74 (Amendment 1 document) section C.2.1.3 Transition Support a. Incoming Transition the government states "The Government has allowed for a four (4) month transition period from the date of contract award, after which the incoming Contractor is expected to provide the full complement of RHRP services to meet the SCs’ needs" however, on page 63 of 74 (Amendment 1 document) section L.4.3.3 Section 3: Transition Plan states " The Government anticipates a three (3) month transition period for the establishment of operations and infrastructure in preparation for full performance of this contract." Can the government please clarify if there will be a 4 month or 3 month transition period?Section C.2.1.3 now reflects the 3 month transition period
9L or MSection L or M does not give guidance on the executive summary. Can the government allow for the executive summary to be submitted outside of the page counts listed in Section L.3.2 Page Limitations for each volume? No. The executive summary is included in the page limitations for each volume.
10C.1.1On page 3 of 74 (Amendment 1 document) section C.1.1 Scope the government references " These services are provided in every state, U.S. Territory, the District of Columbia, and Germany, in group events (gatherings of Service Members [SMs] for provision of services face-to-face) at SC-designated sites (e.g., Armories, drill halls), through the Contractor’s Call Center, and within the Integrated Network. " Can the Government please provide guidance relative to Assessments and examinations in Germany? Are the assessments to be performed in clinics and on site as in the United States? Will overseas travel be the method of visiting or is it the expectation that in-country HCA's will be utilized?Group events in Germany have occurred on military posts and include PHA, dental exams and treatment, and IMR services. There currently is one dental provider near Kaiserlautern, Germany who provides dental exams and treatment in-clinic. The contractor will be expected to provide the most cost-effective provision of services at the group events in Germany.
11C.2.2.3.6Vision Testing is very clear on the examinations, adjustments, ordering etc., but appears to be silent on the preparation and delivery of the frames for the glasses. Can the Government please clarify if the contractor has to provide these frames to service members or will the Government provide them? The Government will provide the frames thru the Naval Ophthalmic Support and Training Command (NOSTRA) but the Contractor will be responsible for ordering via the Spectacle Request Transmission System (SRTS).
12L.4.4.1.6Pricing Support it reads in part….”you are required to provide a breakdown for each fixed unit price and estimated cost that shows the application of direct labor hours, direct labor rates, overhead and other indirect cost rates, subcontract costs, material and any material handling charges, and profit/fee. The Offeror shall prepare and submit the pricing support in accordance with Table 15-2 of FAR Section 15.408. The Offeror will not be required to certify this pricing support.” Is the government requiring a complete cost breakdown of only the cost plus CLINS or is it intending that Offeror’s breakout the cost structure of all the FFP CLINS as well? Is the Government requiring specifically that each Offeror provide a cost buildup/breakdown of each of the FFP unit price(s) for each of the medical or dental procedural items? The Offeror should provide a breakdown of fixed unit pricing by completing Attachment 1 "Tab B - Detail". The Offeror should provide the breakdown for each "Element" in Attachment 1.
13C.9.1If a TMA TEAMS contractor acquired another non-TEAMS TMA contractor is the parent company exempt from OCI?See question 4.

41C.1.1It is understood that additional Service Members may show up at Group Events on the day of the event that were not previously verified with the SC/Unit Group Event Point of Contact and the Contractor (i.e. add-ons to roster for the event). It is our assumption that these individuals should be included in the event to optimize convenience and timely service to the Service Member. Is this assumption correct? Please clarify the requirements for this situation. If the assumption is not correct, please provide clarification. If additional Service members show up at group events for a service that is being provided, the expectation is that those Service members will receive services unless directed otherwise by the SC Group Event Point of Contact or guidance by the SC Point of Contact.

The contractor shall be able to meet the surge of demand up to the range provided. There are some weekends where there are few to no events required, e.g., over holidays.
46C.1.1In order that we may estimate equipment and supply acquisition and shipping costs with some accuracy, information on the types of services at a single group event is requested.
Please provide information on the typical combination of services at group events, i.e. PHAs combined with audio, vision and dental; PDHRAs combined with any other service; audio, vision, and immunizations. What percent of the 351 maximum requirement locations required dental only? What percent required medical only? What percent required both dental and medical services for the same location?Information regarding the breakout of the 351 maximum requirements is not available.

60C.2.2.4.1If linking the referral to the SM immediately, does that mean that a referral does not need to be approved? The 4th and 6th bullets in the second section of 2.2.4.1 are contradictory and confusing in this regard. Please clarify. The sentence regarding immediate linkage to the referral resource is deleted.
61C.2.2.4.1Regarding completion of the referral forms, could the government please provide copies of the DA 2173 (Army) and MMSO-1 (Navy, Marines, and Coast Guard) in its most current versions? The MMSO-1 referral form can be can be found at http://www.tricare.mil/tma/MMSO/pdf/MMSOFormMedicalEligibility.pdf. 79C.9.2.3The RFP states that “The mitigation plan shall be forwarded to the Contracting Officer with the Offeror’s proposal.” It is our assumption that the mitigation plan can be submitted as an attachment to Volume IV, Price. Is this assumption correct? If not, please clarify submission requirements.C.9.1 and C.9.2.3 will be revised to address submittal.
80C.10.1Please provide URLs for or copies of the “Compliance Documents.”The URLs are now provided within C.10.1
81ECLINS 0001, 1001, etc. are referred to elsewhere as firm fixed unit price. Why are they called “Cost type” here?CLIN 0001, 1001, 2001, 3001, and 4001 are fixed unit price. Section E has been revised.
82EIn the Fixed Price Inspection paragraph, the RFP states that CLINs 0001, 1001, 2001, 3001, and 4001 are “designated as Cost type CLINs in Section B”. However, Section B, and all other sections of the RFP, states these CLINs are Firm Fixed Price. Can you please confirm that these CLINs are Firm Fixed Price, and that this contract will be considered a Firm Fixed Price contract?CLIN 0001, 1001, 2001, 3001, and 4001 are fixed unit price. Section E has been revised.
83EPlease list the CDC guidelines documentation for the handling and administration of all vaccines as referenced in (3) Performance Standards Summary Matrix. This information is available on the CDC website.
84F and L.4.3.3 and C.2.1.2.6Section C.2.1.2.6: The RFP states that “The Contractor shall provide for the Government’s approval materials to achieve the training required within this PWS as part of the Transition Plan (Deliverable 3) and not later than 10 working days prior to implementation of new service processes or requirements (Deliverable 50).” Our understanding of this requirement is that Deliverable 50 contains training material related to changes during contract operations, and not training materials for use during the transition in period.

Section F: The initial version of Deliverable 50, Training Material, is identified as to be provided “with Management Proposal.”

Section L.4.3.3: Deliverable 50 is not identified as a component of the proposal in the instructions for Volume III.

It is our assumption that the initial version of Deliverable 50 is not required to be submitted with the management proposal. Is this assumption correct? If not, please clarify. This language has been revised. The C.2.1.2.6 reference to training has been deleted and the initial due date on Deliverable 50 from with management proposal changed to NLT 60 DACA.
85FPlease confirm that the Government will honor the contractor’s interest in proprietary information contained in various deliverables, such as the identities of the contractor’s network providers, the contractor’s proprietary processes and methods, etc. Please refer to Section C.2.2.8
86GThe Contractor has an in-house travel agency that supports other government contracts and adheres to the FTR in its processes, booking travel at the lowest logical fare and at rates at or below per diem. Please clarify what "conduct appropriate competition" means for travel above $2,500. If one travel agency is comparing costs across various airlines, hotels, etc, and travel is booked at the lowest logical fare and at or below government per diem rates, is that sufficient competition and does that satisfy the government's requirement?Appropriate Competition means to obtain a minimum of three quotes/prices, the vendor has to provide those three quotes/prices for airline and hotel to the Government Representative.
87IFAR 52.216-7 Allowable Cost and Payment clause is included in the RFP. However, the CLIN 0001, 1001… in the RFP are identified as being Firm Fixed Price, and will therefore be reimbursed by the fixed price and/or fixed unit prices established in the contract and will not be reimbursed based on actual cost incurred. Can you please confirm that FAR 52.216-7 is not applicable to the fixed price CLINs per 16.307(a)(1)?
FAR 16.307-(a)(1) is applicable to cost-reimbursable or T&M contracts.FAR 52.216-7 is not applicable to the fixed price CLINs per 16.307(a)(1)
88IWould the Government consider a commercial item procurement? Many other Government agencies procure medical exams as commercial items. The vast bulk of the contract involves fixed price activity. Reimbursement of travel costs under FTRs is routinely accomplished in fixed-price contracts without incorporating FAR Part 31 cost principles. Moreover, many commercial item and other fixed price contracts include the Continuity of Services clause (FAR 52.237-3), which provides for transition out activities.No.
89IPlease confirm that clauses dealing with cost-type contracts such as FAR 52.216-7, -8, -11 and DFARS 252.232-7000, relate only to the CLINs for travel costs (XXX2) and transition (0003 & 4003).FAR 52.216-7 and 52.216-11 apply to Cost Reimbursment CLINs.
90IIt is our assumption that the No Fee (FAR 52.216-11) clause only applies to the travel CLINs (XXX2). Is this assumption correct? If not, please clarify.FAR 52.216-11 applies to Cost Reimbursment CLINs
91IWhy does the contract include the clause FAR 52.222-20 Walsh-Healey Public Contracts Act? Please clarify or remove. Please refer to FAR 52.222-20
92IFor FAR 52.228-7, what is the amount and types of insurance that will be required for performance? Please refer to FAR 52.228-7
93IDFARS 252-242-7005 Material Management and Accounting System. Please clarify the applicability of these clause to the fixed price CLIN 0001,1001.. CLINS and CLINS, 2002.. travel reimbursement CLINS?Please refer to DFARS 252-242-7005
94IDFARS 252-242-7004 Material Management and Accounting System. Please clarify the applicability of these clause to the fixed price CLIN 0001,1001.. CLINS and CLINS, 2002.. travel reimbursement CLINS?Please refer to DFARS 252-242-7004
95I
252.227-7014Why is the clause “Rights in Noncommercial Computer Software and Noncommercial Computer Software Documentation” included? There are no contract requirements to create or deliver any computer software. Please refer to DFARS 252.227-7014
96L.4.3.4The second paragraph reads “Additionally, the Offeror shall address whether or not it has met its goals for small business participation in contracts over the past three (3) years that required subcontracting plans…” It is our assumption that this information is to be provided only for the at least three primary references. Is this assumption correct? If not, please clarify.Yes, this is correct
97L.4.4.1.3The RFP states there are “values included in Section B for each of these CLINs.” We understand that offerors should not fill in any amounts for the travel CLINs in their proposals, since there is no basis provided to estimate these amounts. Is this correct?Yes, this is correct
98L.4.3.4.2.4Past performance questionnaires are to be submitted in “only envelopes that are sealed and have been postmarked by the United States Postal Service (USPS) or date stamped by another acceptable delivery service will be considered by the Government.” Most common other delivery services are Federal Express (FedEx) or UPS shippers. These companies usually do not provide a date stamp on packages. If past performance questionnaires are returned using FedEx or UPS may we submit them with a hard copy print out of the tracking information for each respective package that shows the date and time returned to the bidder?Yes.
99Section L,
Section MItem L.4.4.1.6 indicates that pricing support is required in accordance with Table 15-2 of FAR Section 15.408. This section also states that the Offeror is not required to certify this pricing support. We understand based on Section M of selection criteria, the award will be based on price competition, without certified cost or pricing data. Please confirm.Award will be made to that responsible Offeror whose proposal is determined by the Government to offer the BEST VALUE to the Government, price and other factors considered.
100M.6.4“Providing points of contact that cannot be contacted by the Government may have an adverse impact on the past performance rating.” Does this mean that an offeror will be downgraded if its references do not respond, or that fewer references may result in a less favorable evaluation? The evaluation factors are clearly explained in Section M.
101M.6.5The first paragraph of this section states that “To arrive at the “Total Proposed Price,” the Government will sum the proposed prices of the extended amounts for each line item included on Attachment 1 for the base year and each option year. CLINs 0002, 1002, 2002, 3002, and 4002 are estimated amounts set by the Government and will not be included in the “total proposed price.”

The third paragraph then reads “The Government will evaluate the fixed unit prices for “transition-in” and “transition-out” proposed under CLINs 0003 and 4003.” It is unclear whether this evaluation is separate from, or included in, the Total Proposed Price.

Will the government clarify that Total Proposed Price excludes CLINS 003 and 4003, to enable a level-playing field for all Offerors?Total Proposed Price evaluation includes Transition-In and Transition-Out.
102M.6.5“The cost analysis evaluation techniques described under FAR 15.404-1 I through (g), as determined appropriate, may also be performed in further determining the reasonableness of the price.” FAR 15.404-1(d) describes a cost realism analysis. Will the proposals be evaluated for realism in addition to reasonableness, and if so, what costs and/or prices will be so evaluated, and on what basis? Those techniques will be used as appropriate for any appropriate cost and/or price.
103Attachment 1New Procedure Number PP3: Please define the following: “Prescription Authorization for Dental Restorative Services.” We could not find a reference to this terminology in the RFP.These are prescriptions required for the Service member who received dental treatment, e.g., pain medication, antibiotics, etc.
104Attachment 1Column B, New Procedure Number PP1 and PP2, Element description is “Mobile vehicle, dental treatment (limited to dental treatment services listed in Attachment 1; price per vehicle per day).” Based on the requirements in the PWS, it is our assumption that group event dental treatment services can be performed in many physical environments (i.e. government facility, mobile/temporary buildings) in addition to mobile vans. Is this assumption correct? If not, request addition of other physical environments in order to provide the required flexibility by physical location for group events and cost effective solutions for the government.Yes, dental treatment services can be performed in environments other than the mobile vans.
105Attachment 1Column B, New Procedure Number PP1 and PP2: Does the price for these services include all personnel and all treatment services, or are the services to be billed as if they were accomplished in-clinic (section P)? Please clarify the billing of on-site dental treatment services. The price should reflect only the cost of the unit. The cost of professional staff necessary to perform dental treatment should be included in the pricing for services outlined in section P.
106Attachment 1Column B, New Procedure Numbers S (Cancellations) and T (No shows): Could the Government provide a break out by volume or percentage the types of cancellations and no shows, i.e. MHA, PDHRA, Call Center Events, etc.?The historical quanities outlined in Attachment 1 for individual cancellations and no-shows are relevant.
107Attachment 3This NDA is generic and much of it does not apply to contract performance. For instance, contractor staff will not have access during performance to “cost/ pricing data, Government spend plan data, Planning, Programming, Budgeting and Execution (PPBE) documents and information, as defined in DoD Directive 7045.14; contractor technical proposal data, independent government cost estimates, negotiation strategies and contractor data presented in negotiations, contracting plans and statements of work.” Can the NDA be tailored to something that is pertinent to the contract and will not look strange to health care professionals? We will be using the Non-Disclosure Agreement (NDA) that was provided.
108Attachment 26Please validate the contents of Attachment 26, provided with Amendment 0001. It appears only the front page of the past performance questionnaire is attached. Typical contents include additional sections with the questions/ratings for the references to complete.Attachment 26 has been revised.
