The Offeror shall leave these envelopes sealed and submit them with the proposal. Only envelopes that are sealed and have been postmarked by the United States Postal Service (USPS) or date stamped by another acceptable delivery service will be considered by the Government.

L.4.3.4.3 Additional References

The Government may or may not choose to contact these additional references.

L.4.4 Volume IV – Price

L.4.4.1 Section 1 – Price Proposal

L.4.4.1.1 General

The price proposal submitted by the Offeror shall be in a separate binder, plainly marked “Price Proposal.”

Offerors are cautioned that their proposed prices should be consistent with the proposed technical approach and reflective of the effort required by the solicitation. Unrealistically low proposed prices may be grounds for eliminating a proposal from the competition on the basis that the Offeror does not understand the requirement.

Proposed burdened hourly rates shall be rounded to the nearest whole cent, and final monetary extensions (using the CLIN structure in Section B) shall be rounded to the nearest whole dollar.

All five (5) years of the contract (base and option periods) and each CLIN must be priced. The Government will include the price of options in arriving at the total evaluated price for each Offeror.

L.4.4.1.2 CLINs 0001, 1001, 2001, 3001, and 4001

Performance under CLINs 0001, 1001, 2001, 3001, and 4001 shall be reimbursed based upon fixed unit prices for each procedure performed. The Offeror shall propose a fixed unit price for each procedure listed on Attachment 1 to the RFP. The fixed unit price shall consist of a single fixed price rate (inclusive of cost, burdens and profit) for each procedure. The Offeror shall complete Attachment 1 as instructed herein without making any additions, subtractions, and/or alterations to the content or format of the attachment. The Offeror shall populate the “Fixed Unit Price” column within Attachment 1 for the base year and each option year. The “Fixed Unit Price” column will be multiplied by the “Historic Quantity” column to arrive at an “Extended Amount” for each procedure. The “Extended Amount(s)” will then be summed to arrive at a “Total” amount for the applicable period. The “Historic Quantity” column was populated by the Government based upon the anticipated number of procedures that may be required during the applicable period. However, during the performance of the contract, the Government may not order the entire estimated quantity for each procedure. Services/procedures are to be coordinated within a fifty (50) mile driving distance of the service member’s home, service member’s work, or service member’s place of duty depending upon the service member’s request. Therefore, the prices for the services/procedures shall be reflective of performance throughout the United States including Puerto Rico, Guam, American Samoa, Virgin Islands, and Germany. Additionally, the Offeror may be requested to perform services on-site at unit locations or other designated locations by the Reserve Components.

L.4.4.1.3 CLINs 0002, 0003, 004, 1002, 1003, 1004, 2002,2003, 2004, 3002, 3003, 3004, 4002, 4003, and 4004

Performance under CLINs 0002, 0003, 0004, 1002, 1003, 1004, 2002, 2003, 2004, 3002, 3003, 3004, 4002, 4003, and 4004 shall be on a Cost Reimbursable basis. The Contractor will not receive any fee for the performance of services under these CLINs. Costs for these CLINs shall not be proposed by the Offeror.

L.4.4.1.4 CLINs 0005 and 4005

Performance under CLINs 0005 and 4005 shall be on a Firm Fixed Priced basis. The Offeror shall propose the number of labor hours, estimated costs, and fixed fees for providing transition and outgoing transition support.

L.4.4.1.5 Hypothetical Examples – Attachment 1A
