PWS DESCRIPTION SUBMISSION FREQUENCY
1.7 d. Customer service training program for all employees.
Complete the training within thirty (30) business days of the contract start. After the initial training, the Contractor shall ensure any new personnel complete the training within fourteen (14) business days.
1.9 Employee Roster
Ten (10) business days after the start of the contract, at execution of option periods, and/or when changes occur.
3.1 e. Joint Inventory The Contractor and the COR shall conduct a joint inventory at start of the contract and at exercise option periods.
3.5 a. Joint Inventory Be conducted by the incumbent Contractor, the Contractor, and the COR during phase-in.
3.5 a. Joint Inventory Report The Contractor shall submit to the CO/COR no later than ten (10) business days after completion.
3.5 a. Inventory Property
The Contractor shall inventory all property (accountable and non-accountable) annually at the end of each fiscal year or at any time specified by the Government.
3.5 b. Monthly Report
Monthly reporting shall reflect activity through the last day of the month. Reports must be submitted in a manner to reach the CO by midnight, Eastern Standard Time on the fifth calendar day after the end of the month or as specified within the contract.
3.6 a. (1) IT Security Awareness Training Within three (3) business days of an employee being provided access to the Government furnished computer.
3.6 a. (2) Active Shooter Threat Training At least annually, for Contractor and subcontractor personnel, and applicable representatives requiring access to the FLETC.
3.6 a. (2) (a) Active Shooter Threat Training
All Contractor and subcontractor personnel requiring access to the FLETC shall be trained within the first thirty (30) business days of reporting for duty.
3.6 a. (2) (b) Active Shooter Threat Training Reporting
The Contractor shall deliver to the CO within the first thirty (30) business days of performance and semi-annually on the first day of October and April.

4.5 a. Safety Data Sheet(s) (SDS)
The Contractor shall submit to the CO no later than the pre-performance conference for all cleaning supplies and other chemicals that will be used on this contract.
4.6 a. Employee Uniforms A sample uniform shall be submitted for acceptance by the CO/COR ten (10) business days after award.
4.7 Emergency Phone Number
The Contractor shall provide a telephone number at the time of award and at any time the phone number is changed.
4.8 a. Contractor Vehicles
At the pre-performance conference, the Contractor shall provide the CO a list of all Contractor-assigned company vehicles. Any changes during the performance of the contract shall be submitted in writing to the CO within five (5) business days of the change.
5.10.1 Maintenance Agreements The Government will provide this information to the Contractor ten (10) business days after award.
5.11 c. ESPC Deficiency Report Form
The Contractor shall maintain documentation of all repairs and/or replacement parts (warranty & non-warranty) associated with the ECMs and provide a monthly report to the Government by the fifth (5th) calendar day following the end of the reporting period.
5.16 a. Preventive Maintenance Program (PMP) Schedule
The Contractor shall establish an annual schedule for PMP of equipment and be submitted to the CO for approval fifteen (15) business days from date of award and fifteen (15) business days prior to any option renewal.
5.16 d. Monthly Report
Monthly reporting shall reflect activity through the last day of the month. Reports must be submitted in a manner to reach the CO by midnight, Eastern Standard Time on the fifth calendar day after the end of the month or as specified within the contract.
5.25 Pest Control Plan
Contractor shall provide an annual pest control plan that covers all areas of this contract fifteen (15) business days after date of award and fifteen (15) business days prior to any option renewal.

5.25.2 Pesticide Safety Data Sheet
A copy of the SDS shall be provided to the CO at the time of award and fifteen (15) calendar days prior to any option awarded and/or any time during the contract life prior to use of a different pesticide.
5.31 a. Work Order Maintenance System The Contractor shall submit within fifteen (15) business days after contract award to the CO for approval.
5.31 b. Quarterly Report
The Contractor shall provide an email on the fifth day of each quarter (Oct., Jan., Apr., Jul.) to the CO/COR that shows the amount of Freon bought and used during the previous quarter and the cumulative yearly amount as well as the amount currently on hand.
6.1 Service Plan
The Contractor shall submit to the CO for approval ten (10) business days after award, at exercise of option periods, and when changes occur.

6.3 Contractor Quality Control Plan (QCP)
The Contractor shall submit to the CO for approval at the time of Proposal submissions, at exercise of option periods, and when changes occur.
6.3.1 a. Inspection Records
The Contractor shall maintain complete inspection records for the life of the contract. The file shall be turned over to the CO within ten (10) business days after completion or termination of the contract.
6.3.1 b. Quality Control Deficiency Report The Contractor shall submit to the COR/CO within five (5) business day of the month following the reporting period.
6.3.1 c. All Follow-Up Inspection Report
The Contractor shall maintain the Technical Library no later than five (5) business days following the inspection date and available for review by the CO/COR upon request.
6.4 Key Control Plan
The Contractor shall submit to the CO for approval ten (10) business days after award, at exercise of option periods, and when changes occur.

6.5 Safety Plan & Designated Safety
Representative
The Contractor shall submit to the CO for approval ten (10) business days after award, at exercise of option periods, and when changes occur. The Contractor shall provide the CO, in writing, the name of the designated safety representative, pertinent telephone numbers, and normal area of assigned responsibility. The safety representative shall be identified in the Safety Plan and is subject to the CO/COR approval.
6.6 Risk & Contingency Plan
The Contractor shall submit to the CO for approval ten (10) business days after award, at exercise of option periods, and when changes occur.

Hazardous Waste Management Plan
(HWMP)
The Contractor shall submit to the CO for approval at the time of Proposal submissions, at exercise of option periods, and when changes occur.
6.7 b. Hazardous Waste Training
The Contractor shall coordinate with the COR who will contact the FLETC ESD within fourteen (14) business days of award to schedule Hazardous Waste Training.
6.8 Information Technology (IT) Security Plan
The Contractor shall submit to the CO for approval ten (10) business days after award, at exercise of option periods, and when changes occur.
6.9 Property Control Plan
The Contractor shall submit to the CO for approval ten (10) business days after award, at exercise of option periods, and when changes occur.
6.10 Phase-In Plan The Contractor shall provide a Phase-In Plan with proposal submission.
6.11 Phase-Out Plan
The Contractor shall provide a phase-out plan to the Contracting Officer for approval 120 calendar days prior to contract completion.
8.23 Insurance
The Contractor shall furnish to the CO a certificate of insurance as evidence within fifteen (15) business days after the award of the contract.
