PRODUCTION FACILITY INSPECTION REPORT (PFIR)
The purpose of this form is to record data and information obtained during the field inspection of a proposed production facility during “Stage Two” of the Source Selection process. The “Production Facility Inspection” assists FEMA in determining the readiness of the listed facility to meet FEMA’s needs to produce manufactured homes to be used as temporary housing units.
The release of this completed form is governed by the Federal Acquisition Regulations Part 15.3. Its release can only be authorized by the Procuring Contracting Officer (PCO).

The Offeror is requested to provide access to all information requested in this report to a designated Government Official. The information contained herein will be used during the source selection process where it will be reviewed by appropriate personnel.
The following assessments are requested or questions answered:
Examine the manufacturer’s production rate schedule and delivery schedule:
Production readiness:
1. Is the production facility adequate for the potential production rate?
a. Is the facility currently producing homes? YES_____ NO_____
b. How many days per week are is the facility operating? ______

d. What is the current number of staff working on the production line? _______
e. How many employees do you need to operate at full productions for a single shift? _____

i. Is there adequate labor to build the potential production, if not, how many workers will need to added? __________
j. How will labor be trained, do they have a formal training program? _________________________________
k. Does the manufacturer have sufficient personnel of requisite skill on hand to produce the MANUFACTURED HOME?
l. What are the plans to ramp up production if a contract is awarded to the manufacturer?
2. Does the facility have required tooling, test, and inspection equipment in place to support full rate production (FRP)?
a. Does the facility have the proper construction jigs in place, i.e., wall and roof jigs to construct walls and roofs in parallel to the assembly line? _________
b. Does the facility have hoist systems to pick up and move the walls and jigs? ___________
c. Does the facility have adequate space for receivers to pick up or deliver material? ___________
d. Are there other items that cause concern with the facility’s ability to produce manufactured homes? _____________
3. Does the facility have a production manager for the manufactured homes who has experience in producing manufactured homes? __________
4. What methods does the production facility use to ensure that its quality control program works? __________
5. Examine the work instructions at work stations to see what material is provided to the individual working on the production line for quality and completeness? ____________
6. Has the manufacturer developed a bill-of-materiel (BOM) required for production of the MHs? Inspect the BOM, quantities and deliveries and determine its adequacy to meet the production schedule and the delivery schedule.
7. Examine the facilities material storage space to ensure that there is adequate space for materials at full production. ___________________
8. Has the manufacturer identified any long lead items required for the manufacture of the MH? _____________________
9. Discuss the manufacturer’s plans to acquire the necessary long-lead items. Does the facility have a plan to ensure that they have adequate stock of long-lead items for FEMA production? ____________
10. Determine if the manufacturer has agreements with vendors of specialized components such as HVAC or Furnishings and how the manufacturer will acquire these components or Furnishings.
11. Learn from the management team how the facility plans to be able to respond to FEMA requirements that can be issued at any time of the week including hours beyond the traditional work week.
