Mr. John Miller Coast Guard - Shore Maintenance Command 915 Second Avenue, Room 2664 Seattle, WA 98174-1011

Subject: Results of Asbestos Materials Survey in Building 54 (Barracks 7), Base Support Unit Kodiak under

Contract No. HSCG50-08-D-643DH4

Dear Mr. Miller, North Wind, Inc. (North Wind) very much appreciates the opportunity to perform the asbestos survey of Building 54 (also known as Barracks 7) for the U.S. Coast Guard under Contract HSCG50-08-D-643DH4. On 21 June 2012, North Wind and Absolute Services personnel conducted a site visit of the building and Absolute then collected 22 bulk samples of suspected asbestos containing materials (ACM) in the building.

The bulk samples were collected and analyzed for the presence of asbestos by Polarized Light Microscopy (PLM), the method of analysis recommended by the U.S. Environmental Protection Agency (EPA) to determine the composition of suspected asbestos-containing materials. White Environmental Consultants Inc, of Anchorage, Alaska, performed the analysis of the suspect bulk material.

Only materials containing more than 1% total asbestos were classified as "asbestos -containing" based upon EPA and the Occupational Safety and Health Administration (OSHA) criteria. The following building materials were found to be asbestos-containing:

12-in by 12-in Floor tiles

Joint compound (mud) located at the entrance at the first floor

The cove base mastic located in Room 113 and in the hallway at the Sea-bag locker room

The Cement Asbestos Board (CAB) located in the Mechanic Room

The pipe insulation located in the crawlspace

The Asbestos Survey Report prepared by Absolute Services provided in Attachment 1 contains more detailed information on the survey and the survey results.

1.0 INTRODUCTION

Absolute Services Inc. (ASI) was retained by North Wind Inc. to perform an inspection for the presence, extent, and condition of possible asbestos-containing materials (ACM) at Building 54 located at the US Coast Guard Base , Kodiak , Alaska. ASI conducted the investigation on June 21, 2012. The inspection was conducted in compliance with contract specifications requirements.

2.0 SITE LOCATION AND DESCRIPTION

Building 54, also referred to as Barracks 7, is an approximately 16,900 square foot, two-story, wood framed building with a steel moment frame substructure originally built in the 1950's. The building houses the CPO lounge, several offices, and living units including a number of single and multi-bedroom apartment units. The building is in poor condition and is currently unoccupied with the exception of the CPO lounge. The rehabilitation of the building is being considered.

3.0 SAMPLING AND ANALYSIS

During the inspection of Building 54, ASI collected 22 bulk samples from areas and materials suspected of containing asbestos. Two samples were collected from the 12x12 vinyl mastic, four samples were collected from the suspended ceiling tiles, four samples were collected from Gypsum joint compound, two samples were collected from the inner chimney black and mastic, four samples were collected from the cove base mastic, and three samples were collected from the pipe insulation.

The bulk samples were collected and analyzed for the presence of asbestos by Polarized Light Microscopy (PLM), the method of analysis recommended by the U.S. Environmental Protection Agency (EPA) to determine the composition of suspected asbestos-containing materials. White Environmental Consultants Inc, located at 383 Industrial Way, Suite 300, Anchorage, Alaska, performed the analysis of the suspect bulk material.

Only materials containing more than1% total asbestos were classified as "asbestos -containing" based upon EPA and the Occupational Safety and Health Administration (OSHA) criteria. Table 1 in Appendix A contains a summary list of the bulk asbestos samples and analytical results.

Field survey data sheets and laboratory reports are included in Appendix B.

4.0 SURVEY RESULTS

12 X12 Floor Tile The 12x12 floor tile with mastic contains Chrysotile at 4%-tile, Chrysotile at 10%-mastic.

Joint Compound The joint compound (mud) located at the entrance at the first floor contains Chrysotile at 4%.

Cove Base Mastic The cove base mastic located in room 113 and in the hallway at the Sea-bag locker room contains Chrysotile at 3%.

Cement Asbestos Board The Cement Asbestos Board located in the mechanic room contains Chrysotile at 25%.

THERMAL SYSTEM INSULATION

The pipe insulation located in the crawlspace contains Chrysotile at 10% and Amosite at 15%.

5.0 REGULATORY CONSTRAINTS

The Federal Occupational Safety and Health Administration (29CFR 1926.1101) and the State of Alaska Department of Labor (8 AAC 61) have promulgated regulations requiring testing for airborne asbestos fibers; setting allowable exposure limits for workers potentially exposed to airborne asbestos fibers; establishing contamination controls, work practices, and medical surveillance; and setting worker certification and protection requirements. These regulations apply to all workplace activities involving asbestos.

The EPA regulations, issued as Title 40 of the Code of Federal Regulations , part 61 (40CFR61) under the National Emission Standards for Hazardous Air Pollutants (NESHAP) established proper handling ACM during asbestos removal and waste disposal. These regulations require an owner (or the owner's contractor) to notify the EPA of asbestos removal operations and to establish responsibility for the removal, transportation, and disposal of asbestos.

The disposal of asbestos waste is regulated by the EPA, the Alaska Department of Environmental Conservation (ADEC), and the disposal site operator. Wastes being transported to the disposal site must be sealed in leak tight containers prior to disposal and must be accompanied by disposal permits and waste manifests

6.0 LIMITATIONS

The conclusions and recommendations contained in this report are based upon professional opinions with regard to the subject matter. These opinions have been arrived at in accordance with currently accepted environmental consulting and engineering standards and practices and are subject to the following inherent limitations:

1. Accuracy of the Information.

The laboratory Reports utilized in this assessment were provided by the accredited laboratories cited in the report. 2. Site Conditions.

This survey was conducted on June 21, 2012. The site was vacant at the time of the survey, with the exception of the CPO lounge, which is still in use. Other asbestos-containing or hazardous materials may be present in the facilities that were concealed by structure members, walls, ceilings, or floor coverings.

3. Changing Regulatory Constraints.

The regulations concerning hazardous materials are constantly changing, including the interpretations of the regulations by the local and national agencies. Should the regulations or their interpretation be changed from our current understanding, Absolute Services, Inc. reserves the right to amend or revise its conclusions, opinions, and/or recommendations.
