SOUTHEAST LOUISIANA VETERANS HEALTHCARE SYSTEM
NEW ORLEANS, LOUISIANA
Project Number: 629-18-140
Project Title: Install Auto-Operated Faucets
SCOPE OF WORK
1. GENERAL
1.1. The Southeast Louisiana Veterans Health Care System located at 2400 Canal Street, New Orleans, LA 70119 has a requirement for installation of 377 auto-operated sink faucets throughout the campus.
2. SCOPE OF WORK
2.1. The contractor will provide all materials, supplies, tools and manpower to install 377 auto-operated sink faucets in 197 patient rooms located in 3 buildings throughout the campus.
2.2. The new sink faucets will be installed in patient rooms located in the following buildings: Inpatient
Bldg. floors 2-4 (240 faucets in 120 rooms); Transitional Living Bldg. floors 2 & 3 (120 faucets in 60 rooms); Diagnostic and Testing Bldg. floor 5, Mental Health Ward (17 faucets in 17 rooms).
2.3. The existing sink faucets in the specified rooms will be removed, turned over the VA, and replaced with auto-operated fixtures with the following features:
2.3.1. Motion sensor activated
2.3.2. Programmable automatic hygiene flush function
2.3.2.1. Hygiene flush interval – flush can automatically activate after 1 to 168 hours of inactivity
2.3.2.2. Hygiene flush duration – flush can operate between 3 to 180 seconds for each interval
2.3.2.3. Programmable features – programming device uses point-of-use infrared communication
2.3.2.4. Adjustable temperature – faucets must have hot and cold temperature controls
2.3.3. The fixtures to be placed in the Mental Health Ward will be rated as ligature resistant
3. PERIOD OF PERFORMANCE
3.1. The contractor shall complete the task within one hundred eighty (180) calendar days after receipt of the Notice to Proceed.
4. PLACE OF PERFORMANCE
4.1. The work shall be performed at the New Orleans VA Medical Center 2400 Canal St., New Orleans, Louisiana. Work shall occur at the following buildings: Inpatient (Floors 2-4), Transitional Living (Floors 2-3), and Diagnostic and Testing (5D Mental Health Ward).
5. COR AUTHORITY
5.1. In no event is the Contracting Officer’s Representative (COR) empowered to change any of the terms and conditions of the contract. All changes, to any section of this contract, shall be made only by the Contracting Officer pursuant to a properly executed modification. The types of actions within the purview of the COR’s authority are to ensure that the Contractor performs the technical requirements of the contract, and to notify the Contracting Officer of any deficiencies observed. A Letter of Designation shall be issued to the COR and a copy shall be sent to the Contractor at the time of contract kickoff meeting or post-award conference setting forth in full the responsibilities and limitations of the COR.
6. HOURS OF WORK
6.1. Normal operational hours for the VA facilities are from 7:30 a. m. – 4:30 p. m, Monday through
Friday, exclusive of Federal Holidays. All work can be included during this time. The contractor can be allowed to work on weekends and/or after hours with special permission. Requests must be made at least 7 days in advance.
7. SCHEDULING OF WORK
7.1. All the patient rooms needing sink fixture replacement are in active wards. The VA will make every effort to vacate entire units or sections of units so that all work can be completed in these areas before moving onto another area.
7.2. Due to variable and unpredictable patient census, the VA cannot guarantee that entire units or sections of units can be vacated for work to be done. The contractor will need to be flexible when it comes to scheduling work.
7.3. The Mental Health Ward, Diagnostics and Testing Building 5D, cannot be vacated for this work. The contractor will need to be able to work on those 17 rooms on short notice while working in the other two buildings.
8. FAUCET INSTALLATION
8.1. Inpatient Building, Floors 2-4
8.1.1. Remove existing sink fixtures and replace with Auto-operated fixtures in 120 patient rooms per specifications and drawings
8.1.2. Each patient room has two (2) sink fixtures, one each in the patient room and the bathroom
8.2. Transitional Living Building, Floors 2-3
8.2.1. Remove existing sink fixtures and replace with Auto-operated fixtures in 30 patient rooms per specifications and drawings
8.2.2. Each patient room has two (2) sink fixtures, one each in the patient room and the bathroom
8.3. Diagnostic and Testing Bldg. Floor 5, Mental Health Ward
8.3.1. Remove existing sink fixtures and replace with a Ligature Resistant Auto-operated fixture in 17 patient rooms per specifications and drawings
8.3.2. Each patient room has one (1) sink fixture in the bathroom
9. SUBMITTALS
9.1. Contractor shall submit to the Contracting Officer and COR the following prior to commencing work:
9.1.1. Site specific safety plan. The contractor shall provide a detailed site-specific safety plan electronically to the VA for approval prior to the commencement of work. The plan must be specific to the work to be completed and shall include at a minimum: a job hazard analysis, plan for confined space work/entry, hot work, fall prevention and lock out/ tag out.
9.1.2. Product Data. Submit manufacturer’s current specifications and installation instructions. All product submittals must be approved by the VA prior to purchase and implementation. See associated specification section for more information. All product data shall be submitted within thirty (30) days of the Notice to Proceed.
9.1.3. Schedule for installation. The contractor shall provide a detailed listing of tasks to be accomplished (milestones) and projected start and completion dates for each task. This shall include all pre and post work items.
9.1.4. Schedule of Values. The contractor shall provide a schedule of values for each major task associated with the completion of this project. The line items on the schedule of values shall match the line items noted under the construction schedule. Billing will be initiated in accordance with percentage of completion as it relates to the submitted and approved schedule.
9.1.5. OSHA Competency Paperwork. Prior to commencing work, contractor shall provide proof that an OSHA designated “competent person” (CP) (29 CFR 1926.20(b)(2) will maintain a presence at the work site whenever the general or subcontractors are present.
9.1.6. Certified payrolls. Certified payrolls shall be submitted within 7 calendar days after payroll period ends with schedule of values for invoicing. Payrolls shall be submitted electronically to the COR and Contracting Officer for review.
9.1.6.1. All resubmission of payrolls shall be completed within seven (7) calendar days of inquiry or rejection.
9.1.7. All submittal resubmissions must be made within fourteen (14) calendar days of VA returned and rejected submittals.
9.1.8. All other items listed in the attached specifications
10. CODE REQUIREMENTS
10.1. The VA will be responsible for obtaining any necessary station level approvals as required.
10.2. All work performed under this contract shall be in accordance with the VA Master Specifications listed below and attached as part of this SOW:

10.4. All work shall comply with all local, state, and federal rules and regulations. In the event of a code discrepancy, the stricter of the ordinances shall prevail.
11. WARRANTIES
11.1. A one-year warranty on material and workmanship will begin from the date of final acceptance of the work.
12. PARKING
12.1. Parking is rigidly controlled throughout the Medical Center. Contractor parking is only allowed in authorized areas. Contractor shall abide by all police requirements. Parking for unloading equipment necessary to perform the work will be authorized in advance of starting the project.
13. STORAGE OF EQUIPMENT & MATERIALS
13.1. The contractor shall arrange with the COR for allocation of required workspace and for the storage of equipment and material to be used for this project. Storage space is very limited. There are no exclusive areas within the campus that can be given to the contractor for their storage needs.
13.2. Additionally, no space will be made available for the placement of a contractor trailer for this project.
The Contractor shall schedule delivery of materials to limit the amount of storage space and time.
14. DEBRIS REMOVAL
14.1. Contractor shall remove waste materials and debris daily from the campus. All waste material and debris shall be removed off the site by the contractor and shall be disposed of in accordance with applicable State and Federal regulations. All existing fixtures that are removed will be turned over to the COR.
15. SITE UTILITIES
15.1. The Contractor shall note this scope of work does not detail all existing structures, utilities, or components that may potentially interfere with the contract work required. All proposed utility relocations, reallocations, and shutdowns shall be approved by the COR prior to commencing such work. The contractor shall field verify all existing utility installations and take appropriate action prior to working around any potential utility installation.
15.2. In the event a shutdown, restriction, or interruption of any utility services is required, a written request shall be submitted to the COR with a minimum of 7 calendar days’ notice. Requests shall be approved by the COR prior to commencing work.
16. PROJECT SAFETY
16.1. Contractor shall comply with the following and keep a copy of the attached on site at all times:

17. INTERIM LIFE SAFETY PROTECTION MEASURES
17.1. Contractor is not required to prepare a formal interim life safety measures plan for this project. At a minimum, contractor shall comply with the following requirements of an interim life safety measures and fire protection plan:
17.1.1. Maintain the construction area to minimize the potential for fire or safety hazards resulting from storage of construction material, construction waste and debris during construction operations.
18. MATERIAL & WORKMANSHIP QUALITY
18.1. Install all equipment to manufacturer’s specifications maintaining Federal and Local safety standards.
All work shall be done according to the best trade practices and in accordance with the terms and conditions of the contract.
18.2. The Contractor shall provide VA personnel with the necessary training, as needed, to properly operate all provided equipment and systems, within period of performance time frame.
Additionally, the Contractor shall provide complete maintenance service support during the warranty period. All maintenance service shall be performed by factory trained, English speaking, technically qualified servicemen and shall be responsible for recording all work actions in the customer’s maintenance logbook. Contractor’s warranty maintenance service support shall include all parts, service, labor, travel time, and expense to perform corrective maintenance on the new turnkey installation.
19. SECURITY
19.1. All contractor personnel are subject to inspection of personal effects when entering or leaving the project site.
19.2. C&A requirements do not apply, and a Security Accreditation Package is not required.
