1) Please provide make and model number(s) of the existing ligature faucets in the Mental Health Ward. This will assist in estimating the modifications that will be needed, if any, to the top of the counter to fit the new faucet.

2) Please provide existing paint manufacturer, sheen, and color of walls for each room section.
Inpatient & TL by the handwashing sinks/ PROVIDER AREA:

5D sink wall:

Sherwin Williams high gloss Water base Epoxy Pro Industrial Interior /Exterior AF-100 Pashmina. The adjacent wall to the sink is the Sherwin Williams Interior/ Exterior water base high gloss Epoxy Pro Industrial OC-17 White Dove.

3) Please provide existing tile manufacturer, sheen, and color for each room section.

4) Please advise area/specific location where Contractor can place dumpster and conex for material storage.

5) How many rooms will we be allowed/opened to the contractor to be renovated at one time?
Refer to Section 7 in the SOW
6) With over 377 sinks in 197 different rooms, 180 days for completion seems to be an unattainable goal. Is there any liquidated damages if the work takes longer than 180 days?
Liquidated damages are not associated with this action. The period of performance is mandatory and remains unchanged.
7) Will we be required to fully finish each room before moving on to the next room? Including demo, new electrical, new sink faucet, patch and paint drywall, replace wall tile.
Yes. In the event that the hospital is experiencing a high patient census, the room will need to be complete, cleaned IAW provided ICRA requirements, and inspected by Engineering staff (and other VA Officials) in order for a patient to be moved into that completed room and opening up another room to be worked on.
8) Can the contractor replace damaged and removed tile with a close match since most likely the exact tiles are no longer produced?
If the contractor can prove that the exact tile is no longer produced, we will consider an alternate tile after approval from our Interior Design team.
9) What special requirements will we have to follow when doing work in the mental health ward?
· All locked doors must remain closed and locked.

· Be mindful when coming in the door to the unit. The door should be closed quickly and not held open.

· Notify nursing if someone wants to open a door.

· Only clinical unit staff may allow people in or out of the unit.

· If you are working a room, ensure the door is locked while you are in the room and if you leave the room.

· Working outside of the rooms in the common area is at least a two person job: One person doing the job and the other watching the tools and equipment for safety.

· Never leave tools, equipment, gloves, plastic bags, cords etc. unattended.

· Always keep any keys in your possession.

· Clean up all tools, nails, screws, washers, bolts and other remnants of your job before you leave.

· Please do not exchange money, goods, or food with patients.

· Do not engage patients in conversation. Refer them to staff on the unit.

· All information is confidential, even the fact that someone is on the psychiatric unit. If you see someone you know, you cannot talk about this after you leave the unit.

· Add Special Paragraph below to the original Scope of Work.

1. COVID PROCEDURES

0. Transitional Living Building

0. The contractor shall present an initial negative COVID PCR (Polymerase Chain Reaction) test to VA Employee Health, not older than 4 days, prior to starting work in this building.

0. The COVID PCR test must be taken at a facility/organization that is authorized to administer the test.

0. The contractor will be required to be COVID PCR tested twice weekly while working in this building

2. The first test is to be administered Monday thru Wednesday

2. The second test is to be administered Thursday thru Saturday.

0. The contractor will provide the results of the COVID PCR tests to VA Employee Health upon the return of the test results.

0. If a COVID test comes back with a positive reading for COVID, the contractor must immediately notify VA Employee Health and exit the Transitional Living Building.

0. The contractor may return to this building after VA Employee Health has determined it is appropriate.

0. Inpatient Wards 2C and 3C – these two units house our COVID positive patients
