About this file
This is a Past Performance Rating Form (FORM III FA9101-26-R-B011, RFP Attachment 6, L-5) used as a template for evaluating contractor past performance on previous federal contracts. The form is classified as source selection sensitive information per FAR 2.101 and 3.104.
The form requires offerors to provide information about a past performance project, including the contractor name, contract or order number, project title, and total period of performance including options. A designated rater (typically a government representative or customer from the previous contract) completes the evaluation section and returns it to the offeror for inclusion in their proposal response. The form evaluates contractor performance across six criteria using a standardized five-level adjectival rating scale: Exceptional, Very Good, Satisfactory, Marginal, and Unsatisfactory. Each rating level includes specific performance definitions. The six evaluation criteria are: (1) Quality, (2) Schedule, (3) Cost Control, (4) Management, (5) Small Business Subcontracting, and (6) Regulatory. For any rating marked as Marginal or Unsatisfactory, the rater must provide additional substantive comments to justify the rating. The rater completes fields for their name, title, agency or customer, phone number, and email address. This template serves as a standardized mechanism for collecting objective past performance data to support proposal evaluation during the federal contract selection process.

1. QUALITY
RATING Adjectival Rating
Definition
Exceptional Performance meets contractual requirements and exceeds many to the Government’s benefit. The contractual performance of the element or sub-element being evaluated was accomplished with few minor problems for which corrective actions taken by the Contractor were highly effective.
Very Good Performance meets contractual requirements and exceeds some to the Government’s benefit. The contractual performance of the element or sub-element being evaluated was accomplished with some minor problems for which corrective actions taken by the Contractor were effective.
Satisfactory Performance meets contractual requirements. The contractual performance of the element or sub-element contains some minor problems for which corrective actions taken by the contractor appear, or were, satisfactory.
Marginal Performance does not meet some contractual requirements. The contractual performance of the element or sub-element being evaluated reflects a serious problem for which the Contractor has not yet identified corrective actions. The Contractor's proposed actions appear only marginally effective or were not fully implemented.
Unsatisfactory Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance of the element or sub-element contains a serious problem(s) for which the Contractor's corrective actions appear or were ineffective.
