About this file
This document is a template form for submitting work samples as part of the FARM III RFP (FA9101-26-R-B011). The Work Sample Cover Sheet - Prime Contract Effort is used by offerors to document and provide details about relevant past contract performance.
Part I requires offerors to identify the work sample by entering the contractor name, contract number, task order number (if applicable), customer/office symbol and program name, total period of performance in MM/YYYY format, final or most recent contract value, and a unique identifier for each work sample (e.g., WS-1, WS-2). Part II mandates provision of contact information for at least two Points of Contact (POCs) per work sample, such as the Program Manager, Contracting Officer, Contracting Officer's Representative, or other customer representatives, including name, title, agency/customer affiliation, phone, and email. For Joint Venture work, the offeror name must be included in parentheses after the JV name. Part III allows offerors to provide a brief synopsis of the work sample limited to 900 characters, though this descriptive information will not be evaluated. The instructions clarify that POCs should be currently working on the contract/task order or the most recent contacts if the contract has ended.
