About this file
This is a self-scoring worksheet template for the FARM III (Facilities Acquisitions for Restoration and Modernization) Draft RFP evaluation criteria. The scoring matrix is designed for offerors to submit self-assessed points across six primary evaluation categories with a maximum total weighted score of 3,000 points before CPARS bonus considerations.
The first three categories—Mechanical Systems (3.2.2.1), Electrical Systems (3.2.2.2), and Facility Renovations (3.2.2.3)—require offerors to document work samples demonstrating experience with specific system types and components. Offerors may allocate up to 20 points combined across these three categories, with individual line items valued at up to 250 points each and weight factors ranging from 3 to 9. Mechanical Systems includes ten specialized subsystems such as high-pressure piping, large pressure vessels, cooling water pumps, and steam boilers. Electrical Systems encompasses eight medium and high-voltage components including switchgear, transformers, and circuit breakers. Facility Renovations requires work samples executing at least five of seven construction elements (HVAC, fire protection, electrical, exterior components, plumbing, communications, and interior finishes). The remaining two categories evaluate Project Management and Design (3.2.2.4 and 3.2.2.5) with maximum scores of 60 and 20 points respectively, based on total DoD construction project value and single-contract design/construction service experience. The worksheet includes provision for up to five CPARS or CPARS-equivalent project submissions (3.2.2.6) that generate bonus points calculated as a percentage of the base score, with an additional double-bonus multiplier available if offerors submit at least one CPARS entry for each of the first three categories.

Instructions
Instructions
1. Only input data into the yellow cells of the Self Score Sheet tab.
2. Please insert Offeror's name on Row 3.
3. Offeror shall fill in Column D "Offeror Proposed Self-Score Points" for the first 5 Categories, 3.2.2.1-3.2.2.5, with a value equal to or less than the max score allotted for that line (column C).
4. An Offeror may account for a sum of 20 points across the first three Categories: 3.2.2.1 Mechanical Systems, 3.2.2.2 Electrical Systems, and 3.2.2.3 Facility Renovations. Please note that points are referencing the value placed by the Offeror in column D, not the score calculated from those entries.
5. All Offerors are to fill in the Project Management, Development, and Design Category (3.2.2.4 and 3.2.2.5) based off the projects/work samples submitted for sections 3.2.2.1-3.2.2.3.
6. All Offerors are encouraged to fill out the CPARS/CPARS Equivalent Data Submission Category(3.2.2.6), columns A-I, based off the projects/work samples submitted from Categories 3.2.2.1-3.2.2.3.
7. Offerors are allotted a maximum of five CPARS/CPARS Equivalent submissions (3.2.2.6). It is up to the Offeror to determine best score feasible with the available five submissions.
8. Points allotted from CPARS/CPARS Equivalent data are considered a bonus score; each entry will produce a specific percentage of points added for the specific Subcategory it is associated with and the Offeror's ratings.
9. If an Offeror is able to submit at least one CPARS/CPARS Equivalent response for each of the first three Categories (3.2.2.1 Mechanical Systems, 3.2.2.2 Electrical Systems, 3.2.2.3 Facility Renovations), then the Offeror will receive a double bonus, doubling the points received for all CPAR submissions.
