Renovate B20202

1. General Project Information

1.1. Existing Building Conditions and Scope

Building 20202 is located on the southwest corner of Gibson Blvd. and Wyoming Blvd. at Kirtland AFB, NM. The building was commissioned in 1949. It consists of three linear segments which include the following: three-story center segment which is flanked by "C" and "D" wings and includes a basement area, two 2-story segments with one to the west (C-wing) and the other to the east (D-wing).

The existing building is constructed of concrete spread footings and a reinforced slab-on-grade with perimeter and interior beams supporting a slab that spans continuously over the supports in the short direction of the building. The second floor is constructed of a 4-1/2” reinforced concrete slab. The roof structure is concrete roof decking. Roof top unit HVAC system includes water-cooled heat pumps, air distribution, and exhaust fans. There is a water fire suppression system. The interior partitions are metal studs with gypsum wallboard (GWB) and various finishes.

This project will not construct any additional area under the roof and will only renovate the interior of the second floor of the east wing consisting of approximately 5,500 SF. This area provides workspaces daily between the hours of 6 a.m. to 6 p.m.

except for federal holidays, for Security Forces Group (SFG) who oversee the Security Forces Squadron (SFS).

The renovation entails abatement and removal of existing flooring and demolition of selected existing walls. New work consists of installing walls to create new office, meeting room, interview room, and storage spaces, installing new carpeting, updating communication wiring to new Cat 6 standards, painting, and installation of new security doors.

1.2. Design/Build (D/B) Delivery

This Design/Build Statement of Work (SoW) identifies the design and construction services required of the D/B team to complete a turnkey project as further described in the succeeding paragraphs. Provide the professional design services of licensed architects and engineers necessary to develop drawings and specifications in compliance with the requirements of this SoW for review and approval by the government. The plans and specifications constitute the construction documents (CDs).

UFC 3-600-01 Fire Protection Engineering for Facilities 2-1.20 defines the required scope of work as a major project which requires the services of a Qualified Fire Protection Engineer (QFPE) listed in 1-7 Fire Protection Engineering Services.

2. Scope Requirements The following requirements identify the D/B team objectives throughout design and construction of the project to provide a turnkey project:

2.1. General and Miscellaneous Requirements

2.1.1. The renovation is only the interior and shall include all rooms and corridors on the

2nd floor, east wing of B20202 except for the stairwells, restrooms, the north-south main corridor to the remainder of the floor, corridor 2137, and Comm/Server room 2136.

2.1.2. Provide all labor, tools, materials, equipment, means, and methods required to complete this turnkey project in accordance with the CDs. The work will comply with the requirements of all codes, references, and standards applicable to this project as listed below.

2.1.3. The D/B team’s management shall ensure that all employees, to include any sub-contractor employees, performing work on this effort receive an Operations Security (OPSEC) Awareness Briefing/Training prior to starting work and the briefing/training must be accomplished prior to entering the work site on day one of the work effort. The government (i.e., Contracting Officer (CO), Unit Security Assistant, Unit OPSEC Coordinator, or 377 Air Base Wing OPSEC/Signature Manager (377 ABW/SMO)), can provide a tailored lesson plan for use by the D/B team’s management to ensure training requirements are met. It is critical that D/B team’s employees fully understand the importance of not discussing work details associated with this work effort, such as: actions or activities observed, the statement of work, as-built or other drawings, etc., in public or outside of the worksite. The D/B team’s management will track all training by employee name and date the training was provided using a spreadsheet or similar document, such as a sign-in roster, and provide a copy to the assigned CO.

2.2. Demolition

2.2.1. Provide miscellaneous demolition as needed for renovation. Demolished materials shall become the property of the D/B team and shall be removed from Kirtland AFB.

2.3. Abatement

2.3.1. CE has sampled the B20202 2nd floor east wing for Asbestos Containing

Materials (ACM) in 1991, 1992, 1993, and 2024. Much of the 2nd floor has been abated except for the existing floor tile and mastic which has tested positive for ACM. (Attachment E).

2.3.2. This scope requires removal and replacement of the existing carpeting. It has been determined the removal of this carpeting will require abatement of ACM since it is glued to the existing floor tile and therefore the flooring must be abated per Abatement Plan Area. (Attachment D).

2.3.3. Flooring under existing walls shall not be disturbed with exception of the existing east walls of room 2133 which are constructed on top of existing carpeting. Walls shall be demolished prior to flooring abatement.

2.3.4. If hazardous material is encountered, the D/B team is to immediately notify the Kirtland AFB Contracting Division Contract Officer (CO) and Civil Engineer Project Manager (PM). A suspension of work may or may not be issued by the CO. If sampling and testing are required, it will be performed by Kirtland AFB Environmental Division and results added to this contract. A formal, funded modification will definitize the necessary contract action.

2.4. Interior Renovations-Architectural

2.4.1. New steel stud and gypsum board walls shall be provided in accordance with new design layout and finished in accordance with the design. A Conceptual Plan is included in the CD’s. (Attachment B). See Structural, for additional requirements for bracing of walls.

2.4.2. Room 2133, Battle Staff Conference Room, shall be sound rated to STC 44 and walls shall extend to the bottom of deck. The existing walls are framed from floor to the bottom of deck, with insulation and gypsum board extending to the lay-in ceiling. For the west and north walls, modify the existing wall assembly to achieve a sound rating of STC 44. The room is to be enlarged such that the east wall will be relocated close to Grid Line “N” shown on the Conceptual Plan (Attachment B). The location of the wall shall not conflict with the location of the existing heat pump and access to the shut off valves.

2.4.3. Rooms 2142, 2144, and 2147 shall have sound insulation for new walls with sound batts laying on ceiling above walls extending out three feet to each side.

2.4.4. Room 2141, Evidence Room, shall be a secured room. Windows shall have interior mounted security bars. Provide a wall assembly that prevents unauthorized entry through the walls and ceiling plenum space while keeping in compliance with the structural requirement section of this SoW. The east wall shall be located close to Grid line “O” on attachment B and shall not conflict with the location of the existing heat pump and access to the shut off valves.

2.4.5. All existing doors shall remain, except the door in room 2133. D/B team shall remove fire-labels on existing doors. New interior doors shall match existing and be provided with hollow metal frames grouted solid and solid core wood.

Doors’ veneer finish shall be as selected from manufacturer’s standard door finishes to be as close to existing and provided on the color board. Hardware shall be required for security and in accordance with code requirements.

2.4.6. All existing doors locks shall remain. Replace door to room 2133 with sound rated STC 44 door assembly and an added cypher lock pre-wiring. New doors in hall 2139 shall have cypher lock pre-wiring as indicated on the Conceptual Plan. All other added doors to have standard keyed locks.

2.4.7. Demo existing suspended-ceiling grid per Ceiling Demo Replace Plan (Attachment C), remove and store existing 2’x 2’ ceiling tiles for selected re-use in remaining existing ceilings, mechanical diffuser supply and return air grills and lighting and protect all from damage. Provide temporary supports for existing flexible ducting. Install new 2’x 2’ suspended ceiling grid system and carrier channels. Reconnect and reinstall mechanical components and lighting at the original locations where possible, avoiding interference with new walls or existing overhead fire suppression sprinklers.

2.4.8. Remove existing public announcement speakers in room 2133. These will not be reinstalled.

2.4.9. Paint all walls within the limits of the renovation. D/B team shall recommend color scheme in conjunction with the color boards required per the Kirtland AFB Installation Facilities Standards (IFS). Paint door frames to match existing.

2.4.10. Install new carpet tiles and cove base within the limits of the renovation. D/B team shall recommend color scheme in conjunction with the color boards per

IFS.

2.4.11. Paint and carpet tiles shall comply with the requirements of the IFS which references applicable UFCs. Per IFS submit color board for Architectural Compatibility Review Board (ACRB) and user review. 2.4.12. All existing openings, as well as openings caused by removals incident to renovation construction activity, shall be infilled and patched as required to match adjacent surface(s).

2.4.13. Provide all office spaces with signs complying with Department of Defense (DoD) Sign Standard UFC 3-120-01 Interior Signs and IFS. Submit a signage schedule and sample(s) for ACRB review and approval.

2.5. Modifications and Upgrades

2.5.1. Communications (Comm)

2.5.1.1. Design, provide, and coordinate installation with Base Comm for CAT6 Comm ports, cables, patch panels, and other components within the limits of renovation.

2.5.1.2. In 2022-2023 a previous Comm project was performed on the 2nd Floor, east wing as shown. (Attachment G). New CAT 6 cables were installed to the existing columns shown on the Conceptual Plan. A new 42 port CAT6 patch panel was installed in the existing COMM/Server Room.

2.5.1.3. D/B team shall perform a survey of the existing comm system and identify components that are re-usable. Survey to include comm drops and other components such as cable trays, patch panels, and switches that are currently in place for review and approval before installation.

2.5.1.4. Submit a layout plan showing the survey performed. Show what components will be demolished. If some cables and components are re-usable, show where existing cables and components will be relocated.

2.5.1.5. Demolish any CAT3, CAT5, and all unused cabling back to the patch panels in the COMM room while protecting any cables that remain. Re-use comm jacks as needed. Existing Cat 6 comm drops utilize surface mounted materials on columns, re-locate to new walls per attachment A as needed. Some existing surface-mounted comm drops to remain as will be determined in the field.

2.5.1.6. Install new Cat 6 comm drops near locations indicated on the Conceptual Plan including rough-ins for new cameras and cypher-locks to Comm/Server room. Install comm from Comm/Server room to viewing room/equipment storage labeled as P-1.

2.5.1.7. All work, new materials, and reused materials and components shall comply with KAFB Communications Specifications 2023 edition (Attachment H).

2.5.2. Structural

2.5.2.1. Seismic load analysis and bracing requirements will be based on a Seismic Design Category C facility.

2.5.2.2. Provide bracing of non-structural components per ASCE 7-16 chapter 13.

This is to include, but not limited to the following non-structural components:

2.5.2.3. The elevated floor slabs of the facility have been previously evaluated as part of an AIMT Engineering Service Inc. (AIMT) capacity check evaluation (Attachment I) to determine the floor live load capacity in its current as-built condition. Therefore, the walls that are slab supported (not directly bearing on a concrete beam line) are subjected to a total wall system weight limitation not to exceed 7psf. The partition walls are to be constructed per ASCE 7-16 13.5.8. If this load is exceeded the wall will need to be supported by a structural load-carrying element or the slab will require support directly under the new wall. The support loads would then need to be directed to the adjacent beams and/or columns with the adequate connections to support the required reactions. Each partition type will need to be submitted for review and approval by PM and CO prior to installation. The submittal will need to include the total weight of the wall system and will need to be listed in the construction documents.

2.5.2.3.1. The results of the (AIMT) floor slab evaluation yield a maximum uniform distributed live load of 50 psf for the elevated floor slabs in their as-built condition. The floor slabs have floor deflections beyond the serviceable limit state per the IBC 2021. Therefore, the use of the floor slabs adjacent the corridor is not permitted for bulk material and heavy equipment storage during construction. The corridor slab is permissible for light material storage during construction and the D/B team is to provide a material laydown plan for review and approval prior to utilizing the corridor slab for material storage. The material laydown plan will need to include the material, material weight, quantities, dimensions, etc. and is not to exceed a uniform distributed load of 40 psf.

2.5.2.4. Provide seismic bracing of mechanical, electrical, and plumbing (MEP), modified fire sprinkler suppression sprinkler head relocations, and architectural components, etc. per ASCE 7-16, chapter 13 utilizing sections 13.1.3 and 13.1.4 for determination of importance factors and exemptions.

2.5.2.5. Attach all seismic bracing for the non-structural components to the underside of the slab or to the upper third of the concrete beams to distribute the vertical and horizontal component loads into the structural diaphragm. The loads are to be determined by the requirements of ASCE 7-16, chapter 13.

2.5.3. Mechanical

2.5.3.1. Provide calculations to verify heating and cooling loads are adequate for each occupied space per ASHRAE 55.

2.5.3.2. Ensure minimum ventilation rates and outside air requirements are in accordance with ASHRAE standard 62.1.

2.5.3.3. The final wall layout is to be coordinated such that the existing heat pumps, associated piping, and valves are accessible and do not require modification.

2.5.3.4. In the event a heat pump needs to be relocated, remove, and replace existing heat pump unit and associated ductwork. If a unit is moved toward the interior of the space, slope on the condensate drains should be verified to be adequate for proper operation. Include any replacement tubing, disconnecting/reconnecting regular and low voltage, and clevis hangar assemblies as per type L at 10' on center.

2.5.3.5. Remove, store, and reinstall air supply diffusers and return air grilles as needed per the new wall installations and ceiling demo and replacement to be adjusted for optimum distribution utilizing drop-down hangars.

2.5.3.6. The outside air is coming from a different fan system, a relief vent is required to maintain equal pressurization.

2.5.3.7. Add 6” diameter spin-in outside-air duct with spin-in with balance damper as needed.

2.5.3.8. Commission the completed HVAC system and provide test and balance for HVAC areas modified in this project.

2.5.3.9. Replace existing thermostats and install additional thermostats of similar design to accommodate final wall layout with one per room.

2.5.4. Electrical

2.5.4.1. Building 20202's main electrical service consists of two service transformers: 500 kVA, 208Y/120V and 750 kVA, 480Y/277V. The two main distribution panelboards, DPL and DPH, are in the electrical room on level 1. They are both fully loaded.

2.5.4.2. The existing distribution panelboards in the electrical rooms appear sufficient for the electrical work in the areas/rooms on the second floor. If necessary, rewire the existing feeders and branch circuits, and provide a new subpanel IAW applicable UFC, codes, and standards. Balance existing and new loads between circuits and phases in the panelboards.

2.5.4.3. Selective lighting and power demolition is required in the areas/rooms.

The D/B team is responsible to ensure that electrical demolition does not affect the continued electrical service in the occupied areas of the building.

2.5.4.4. Design and coordinate new and existing circuits. Existing luminaires shall be reused. Relocate luminaires and associated new switches as necessary to accommodate new wall layout. Install new switches and wiring for lighting to ensure each new room is independent of shared spaces.

2.5.4.5. Provide new receptacles where necessary to accommodate new wall layout. Relocate existing receptacles as necessary.

2.5.4.6. Provide a duplex receptacle for each wall-mounted TV as shown on the Conceptual Plan.

2.5.4.7. Other electrical work is required to support mechanical HVAC and telecom work. Accommodate for new systems furniture, locations to be determined in the field by user.

2.5.5. Fire Protection

2.5.5.1. Submit and provide a Life Safety Plan, showing occupancy type, occupant loads, required widths, and means of egress paths with occupant load.

Include other contributing spaces to occupant load(s).

2.5.5.2. Provide location of all required way-finding signage.

2.5.5.3. If there is down-time during the work, submit in writing when down time is anticipated (from start date to finish date) 21 days prior to the anticipated start time for approval. Notify the alarm-room 3 days prior, and facility manager during this time as well. Place a sign on each of the normal entrance and exit doors indicating these dates. Mark off the area being worked on so the section supervisor can brief his personnel on an alternate escape route.

2.5.5.4. Design, provide, and install modifications to the existing wet-pipe fire sprinkler system IAW applicable UFC, codes, and standards. Include the addition or realignment of sprinkler heads to accommodate new wall layout.

2.5.5.5. Design, provide, and install modifications to the existing fire alarm system IAW applicable UFC, codes, and standards. Fire alarm plan shall show any modifications to the existing FACP, pull stations, horns, and strobes.

2.5.5.6. Field Verification- Descriptions of the project scope provided in this SoW, are solely for the purpose of revealing the general nature of the work involved. The D/B team is responsible for providing any or all field investigation needed to confirm existing conditions and ensure the proposed design has accounted for current field conditions.

3. D/B Requirements All work shall be in strict compliance with all applicable requirements as adopted and modified by the Department of Defense. Listed below, but not limited to these, are emphasized criteria, codes, regulations, and standards:

3.4. Period of Performance

3.4.1. The start of design and construction shall be approved by the CO in writing as two separate Notices to Proceed (NTP). Calendar days are provided in the Table

4.1 the Period of Performance (PoP) timeframe with milestones to accomplish submittals and final inspection and acceptance.

3.4.2. Provide an overall schedule to meet the PoP from start to final completion date.

Completion will include acceptance of all closeout documents.

3.4.3. The PoP for this project shall be 323 calendar days of contract award. Table 4.1 shows the timeframe of each government response requirement.

4. Drawing and Specification Standards

4.1. Contract Drawings

4.1.1. Preliminary Design (50%): Submit a preliminary design package that will include, as applicable, deliverables described and incorporating responses to all government review comments.

4.1.2. Pre-Final Design (95%): Submit a pre-final design package that includes applicable deliverables described and incorporating responses to all government review comments.

4.1.3. Final 100% Design: The final 100% design shall include all drawings and specifications with all comments and corrections incorporated.

4.1.4. Drawings shall be prepared and submitted in AutoCAD DWG format. Plot configuration files shall be submitted.

4.1.5. All applicable base activity representatives shall sign final drawings as having been reviewed. Full size drawings shall be 24" x 36". Reduced drawings shall be 11" x 17" for the final-stamped 100% drawings.

4.2. Specifications

4.2.1. Specifications shall be delivered at each design submittal stage in accordance with

Table 4.1.

4.2.2. Provide technical specifications that are definitive and free of ambiguities. Use of the Unified Facilities Guide Specifications (UFGS) system is mandatory in preparing specifications for D/B projects. UFGS are available at the Whole Building Design Guide website (http://dod.wbdg.org/).

4.2.3. The UFGS are published only in electronic format and are intended to be used with Specs Intact software. The UFGS Master complies with UFC 1-300-02.

Updated UFGS are posted quarterly in February, May, August, and November.

4.2.4. D/B team shall avoid the use of "proprietary type" specifications, except to set an acceptable standard, or when otherwise not available, and shall be re-written in UFGS format prior to inclusion in contract documents in accordance with UFC 1- 300-02 UFGS Format Standard. When necessary to use a manufacturer's name to describe a type of product, at least three manufacturers shall be named, if possible, and shall include the words "or approved equal".

4.3. Design Analysis (DA)

4.3.1. DA shall be provided bound on 8-1/2” x 11” paper and on approved electronic media as approved by the Base Civil Engineer. It shall be arranged by discipline, meet all division specific UFC requirements, and support design decisions made throughout the design. The DA shall provide a narrative description of the key elements and features of the design. It shall include calculations, material cut sheets, and explanations of options considered. The DA shall also contain, as a separate section, all previous design review comments annotated with action taken, cost estimate, and a listing of any outstanding issues requiring resolution.

Drawings shall be submitted under separate cover.

4.3.2. The information provided in the paragraphs that follow shall be used at every submittal point to produce the various elements of a complete DA.

4.3.3. Purpose: The DA contents shall be used 1) to provide a narrative description of the key elements and features of the design; 2) to provide a basis for design, including assumptions and design criteria such as temperature, humidity, and loads; 3) to provide the information, justification, and calculations of the design;

and 4) to provide the designer’s thought process in the development of the design.

4.3.4. Specific Content: The submittal topics to be discussed in the design analysis are shown below. The format and organization within the outline are at the discretion of the D/B team. Include catalog cuts of proposed products where applicable. Update the contents of the DA at each required submittal point to account for changes and corrections since the last submittal. Calculations need not be resubmitted at each point if the previously submitted calculations were complete and no corrections or changes were required. However, if calculations are not included with a subsequent submittal, the contents shall reference the submittal date, submittal title, volume number, and pages where the calculations may be reviewed. All calculations shall be included in the final design submittal.

4.3.5. Criteria: List all Air Force manuals, regulations, instructions, military handbooks, UFC, codes, design guides, Comprehensive Plans (CPs), and CP bulletins used by the D/B team for guidance in the project design.

4.3.6. Analysis Summary:

4.3.6.1. Describe the scope of work.

4.3.6.2. Identify any violations of criteria that would occur if the project is constructed as described in the design documents.

4.3.6.3. Recommend phasing of construction if required to meet user needs.

4.4. Delivery Schedule

4.4.1. The PM is to schedule the 50% and 95% review conferences at Kirtland AFB after government comments have been provided to the D/B team and the D/B team has prepared responses. See Table 4.1 for the review timeline.

4.4.2. Upon government approval of the 50% and 95% design, provide the 100% design submittal.

4.4.3. Upon government approval of the 100% design, provide the final 100% stamped documents submittal.

4.4.4. The 100% final design shall be manually signed, dated, and sealed by the licensed design professionals responsible for the design. Final drawing and specification set submitted shall be an original signed set; copies will not be accepted.

5. General Construction Information

5.1. Before the commencement of construction activities, the user shall remove from the entire facility furniture, fixtures, equipment, and appurtenances in second floor east wing area to include: All hallways and rooms except bathrooms and the Comm. room as seen in attachment B. It is planned that all the staff will be relocated to other parts of the building not occupied by construction activities until these activities are complete.

5.2. The D/B team shall schedule appropriate construction scheduling with the CO. The D/B team shall provide a three week look ahead update with each week plus a 21-day look-ahead schedule.

5.3. The PM will notify the facility manager (FM) who shall notify other building users of the upcoming schedule events. Any outages of electrical, comm, water, sewer, or gas shall be comprehensively scheduled and coordinated with the CO, PM, and FM. D/B team shall fill out an outage form and submit it thirty days in advance. At no time shall the user in adjacent facilities affected by the outage be without such services.

5.4. Access - The D/B team shall not use the existing elevator during construction activities. The east stairs shall be used for access to the second floor. The building entrance leading to these stairs is located on the southeast corner of the facility.

Protect existing stair construction and restore any damage to preconstruction condition at conclusion of construction. Access to the remainder of the facility outside the scope of work of the project is prohibited.

5.5. Minimize interference with regular Kirtland AFB activities: The D/B team shall plan construction activities to minimize interference with the normal activities of base occupants in the remaining areas of the facility, neighboring facilities, surrounding streets, and parking areas.

5.6. The D/B team will not have access to the facilities restrooms. Provide alternate facilities to accommodate the construction workforce.

5.7. The project shall be scheduled and executed to minimize conflict and to facilitate user's operations. Changes to the approved schedule shall be vetted through the CO to support minimal operation impact.

5.8. Staging, Storage, and Parking

5.8.1. The government will designate an area (attachment A) for construction staging and storage. The exact location will be discussed and determined in the pre-construction meeting. The location shall not impede access for emergency response vehicles and personnel.

5.8.2. Parking shall be in designated parking spaces and in designated parking areas.

Exact areas designations to be discussed during the pre-construction meeting.

Parking will not be allowed on sidewalks or grass areas. (attachment A)

5.9. Availability of Site

5.9.1. Workdays are normally defined as 8-hour duty days of Monday through Friday, excluding federal holidays and family days. Normal hours of operation are 7:00 AM to 4:00 PM. Outages to the facility shall be kept to a minimum, confined to the weekends, and scheduled 30 days in advance.

5.9.2. Work after business hours, on weekends, or federal holidays will be allowed if requested 10 days in advance to the CO in writing and not interfere with base missions. The PM shall coordinate with the user if approved by the CO.

5.10. Schedule and Planning Requirements

5.10.1. Submit three week look ahead schedule to show how work is to progress from start to completion, if warranted, also submit a drawing to visually depict areas of work during the same period. Three week look ahead schedule shall be submitted every two weeks unless major changes happen between the two weeks that can affect coordination with Kirtland AFB. See Spec. Section 01 32

16.00 20 for details.

5.11. Meeting and Conference Requirements

5.11.1. Meeting/Teleconference Support – PM shall schedule meetings necessary to support design coordination and each design review. Lead and support weekly meetings and/or teleconferences throughout the construction phase of the contract. Prepare and maintain the weekly meeting agenda report which shall include the record of new and old discussion of technical issues, percent complete actual versus planned, and tabular report for Requests for Information (RFIs) and submittals (open and upcoming). Submit the meeting agenda in writing to the CO, PM, and CE prior to the weekly meeting. Prepare, update, and submit in writing to the CO, PM, and CE on a weekly basis, a three week look ahead schedule to demonstrate construction progression from start to completion.

5.11.2. Prepare minutes for meetings, including teleconferences, design reviews, weekly meetings, and Red Zone meetings, and distribute minutes to attendees within three working days.

5.11.3. Red Zone meeting shall commence at 80% of construction completion as scheduled by the PM weekly. At these meetings, the D/B team and the project stakeholders shall discuss, define, and achieve consensus on actions necessary to complete construction and support user occupancy. Meetings at the beginning of the Red Zone period will be held every two weeks then increase to weekly towards the final month of the project.

5.12. Notification Requirements

5.12.1. The D/B team is required to notify the CO and PM of anything they believe is outside their scope of work and/or beyond their control that could impact cost, schedule, or quality. Additionally, the D/B team is required to notify the CO and PM of critical issues that may affect the contract performance and/or human health and the environment. The types of issues that require notification include, but are not limited to, health risks, spills, and changes in critical or key personnel. If encountered. immediately stop work, report the discovery to the CO and PM, and implement the appropriate safety precautions. Field activities shall not resume until clearance is received from the CO. On critical issues, verbal notification shall be made immediately, followed by written notification as soon as practical within the succeeding 48 hours.

5.12.2. Utility outages: Outages require 30-day notice to schedule and shall not take place during hours of operation.

5.13. Permits/ Environmental Requirements

5.13.1. The following permits are applicable, and approvals are required by Kirtland

AFB prior to construction start, which includes Form 103 Utility Outage Request Form.

5.13.2. Obtain the following additional permits:

5.13.2.1. KAFB Landfill Permit

5.13.2.2. Hazardous Waste Disposal Permit

5.13.2.3. Abatement Permit

5.13.2.4. Hot Work Permit- obtained at Kirtland AFB Fire Dept.

5.13.3. Other permits may be required on this project, pay particular attention to the requirements specified in the Kirtland AFB Checklist of Environmental Requirements for this project, which detail regulations with which those executing the work of the contract shall comply.

5.13.4. If changes are made to the project, or new laws or policies are enacted, there may be additional permits required for construction. Prior to work starting, verify that all current permit requirements are being met.

5.14. Photo Documentation

5.14.1. Prepare digital photo documentation of field activities and progress. Digital photos shall be submitted using a minimum five (5) mega pixel photograph in JPEG format. Provide an index for each set of photographs submitted, identifying the project number, the D/B team, and a brief description.

Photography of any kind shall be coordinated through the CO, PM, and CE. The PM shall coordinate with facility point of contact (POC).

5.15. Accident Prevention Plan (APP) and Checklist - provide an Accident Prevention Plan (APP) and checklist in accordance with EM 385-1-1.

5.16. Site Protection, Cleanup, Repair or Replace, and Differing Site Conditions

5.16.1. Ensure that all adjacent property, buildings, and contents are protected from dust, dirt, runoff, debris, and other damages resulting from project activities.

5.16.2. Provide plastic sheeting, covering of ventilation, and the use of sticky mats to minimize the transfer of construction dust and debris to the occupied areas of the building during construction. Ensure that all trash and debris shall be disposed of daily.

5.16.3. Blockages or obstructions of any doorway or building egress shall be coordinated with the PM.

5.16.4. No food shall be prepared on the facility premises. Coordinate with the user to designate a location outside of the work area where breaks will be taken.

5.16.5. Replace or Repair: Replace or repair to match original existing conditions, all existing adjacent finished surfaces, utilities, equipment, products, vegetation, and structures or parts thereof that have been damaged or removed to perform the work. Inform the CO, PM, and CE in sufficient time to allow scheduling inspection of all work prior to being enclosed.

5.16.6. Daily Cleanup: Ensure that all non-recyclable trash, debris, refuse, garbage, etc.

is disposed in a safe and legal manner. The Kirtland AFB landfill will be available for disposal of non-hazardous construction and demolition waste.

5.16.7. Final Cleanup: Ensure that the project site is completely clean before the project final walk-through. All new materials shall be properly cleaned and shall be free of scuffs, dirt, and dust. Areas surrounding the construction site shall also be cleaned of any dirt or dust. All unused construction materials shall be removed from the site.

5.17. DD1354

5.17.1. D/B team is required to provide information for PM and CO to fill out a

DD1354 for real property records and a component and subcomponent list for new and previous equipment, to include, but not limited to model, serial numbers, and manufacture specification, locations, and before and after pictures. D/B team shall contact real property department for any questions and concerns. DD1354 and component list are to be completed, submitted, and accepted before any pre-finals and finals are scheduled.

5.17.2. There can be only one final version of a DD Form 1354 for a single construction project. The D/B team is to update a draft version on a daily or semi-daily time frame. The D/B team may simply have an adjusted draft version with a later date prepared (block 2) and the same serial number (block 4). However, the D/B team may have multiple valid interim DD Form 1354’s for a phased turnover of facilities, each with its own serial number (block 4). This will be reviewed by the government at the 65% completion of the construction project.

5.17.3. FINAL Form DD 1354 to include approved final invoice with checklist including key supporting documents shall be submitted with approval of final invoice.

5.18. Warranty – Furnish an overall warranty for one year upon Beneficial Occupancy Date (BOD) by the government.
