1. General Project Information

1.1. Design-Build (D-B) Delivery

This Design-Build Statement of Work (SoW) identifies the design and construction services required of the Contractor – (KTR) to complete the turnkey project as further described in succeeding paragraphs. Provide the services of licensed professional designers necessary to develop drawings and specifications in compliance with the requirements of this SoW for review and approval by the Government. UFC 3-600-01 Fire Protection Engineering for Facilities 2-1.20 defines the required scope of work as a major project which requires the services of a Qualified Fire Protection Engineer (QFPE) listed in 1-7 Fire Protection Engineering Services. The plans and specifications constitute the Construction Documents (CDs) the KTR is required to comply with to complete the work of the project.

1.2. Existing Building and Proposed Scope

Building 20202 is located on the southwest corner of Gibson Blvd. and Wyoming Blvd at Kirtland AFB, NM (KAFB). The building consists of three (3) segments which include the following: 3-story center segment which is flanked by "C" and "D" wings and includes a basement area, two 2-story segments one to the West (C-wing) and the other to the East (D-wing). This project will not construct any additional area under the roof and will only renovate the interior of the second floor of the East Wing consisting of approximately 5,500 SF. This area provides workspaces daily between the hours of 6 a.m. to 6 p.m. except for Federal Holidays, for Security Forces Group (SFG) who oversee the Security Forces Squadron (SFS). The renovation entails abatement and removal of existing flooring, demolition of selected existing walls, installation of new walls to create new office, meeting room, interview room, and storage spaces, installation of new carpeting, updating communication wiring to new Cat 6 standards, painting, and installation of a new security door.

The existing building is constructed of concrete spread footing and slab-on-grade with interior concrete load-bearing columns. The second floor is constructed of a 6” concrete slab. The roof structure is concrete roof decking. HVAC systems include water-cooled heat pumps, air distribution, exhaust fans and water fire suppression system, The remaining interior partitions are metal studs with gypsum wallboard (GWB) and various finishes. The building was commissioned in 1949.

1.3. Attachments / Reference Documents

The following documents are applicable to this SoW to the extent specified herein.

These documents are provided to assist the KTR to field verify the accuracy of any information that has been provided by the Government.

2. Scope Requirements The following requirements identify the KTR objectives throughout design and construction of the project.

2.1. General & Miscellaneous Requirements

2.1.1. Provide all labor, tools, materials, equipment, methods and means required to complete this turnkey project in accordance with the CDs. The work will comply with the requirements of all Codes, References, and Standards applicable to this project as listed below.

2.1.2. The renovation shall include all rooms and corridors on the 2nd floor, East Wing of B20202, with the exception of the stairwells, restrooms, the North-South main corridor to the remainder of the floor, and Corridor 2137.

2.1.3. The following requirements apply to the KTR throughout design and construction of the project. The renovation will be constructed to replace existing systems to include but are not limited to the following:

2.2. Demolition

2.2.1. Provide miscellaneous demolition as needed for renovation. Any salvage value of materials, not claimed by CE, shall become the property of the KTR, and shall be removed from KAFB. See Section 2.1.3.2. for existing east wall and east door of Room 2133.

2.3. Abatement

2.3.1. CE has sampled the B20202 2nd floor East Wing for Asbestos Containing

Materials (ACM) in 1991, 1992, 1993 and 2024. Much of the 2nd floor has been abated EXCEPT for the existing floor tile and mastic which has tested positive for ACM (Attachment D).

2.3.2. This scope requires removal and replacement of the existing carpeting. It has been determined the removal of this carpeting could likely disturb this ACM since it is glued to the existing floor tile and therefore the flooring must be abated per Abatement Plan Area. (Attachment E).

2.3.3. Flooring under existing walls shall not be disturbed. Exception: existing East walls of Room 2133 which are constructed on top of existing carpeting and shall be demolished prior to flooring abatement.

2.3.4. The KTR shall test for additional undiscovered asbestos, Lead-Based Paint (LBP), or Poly-Chlorinated Biphenyls (PCB) if suspected. In this event the KTR is to immediately notify the KAFB Contracting Division Contract Officer (CO) and Civil Engineer Project Manager (PM). A suspension of work may or may not be issued by the CO. If sampling, and testing are added to this contract, the KTR will be responsible for sampling and testing of suspected materials. A formal, funded modification will definitize the necessary contract action.

2.4. Interior Renovations-Architectural

2.4.1. Limits of renovation: The renovation shall include all rooms and corridors on the 2nd floor, East Wing of B20202, with the exception of the stairwells, restrooms, the main corridor to the remainder of the floor, Corridor 2137, and Comm/ Server Room 2136.

2.4.2. New steel stud and gypsum board walls shall be provided in accordance with new design layout and finished in accordance with the design. For general understanding purposes a conceptual plan is included in this SoW, see (Attachment A). See 2.5.2. Structural for additional requirements.

2.4.3. Room 2133, Battle Staff Conference Room shall be sound rated similar to STC 45, and walls shall extend to the Bottom of Deck. The existing walls are framed from floor to the bottom of deck, with insulation and gypsum board extending to the lay-in ceiling. For the West and North walls, modify the existing wall assembly to achieve a sound rating similar to STC 45. Provide documentation to PM and CO of how this similar rating was achieved. The room is to be enlarged such that the East wall will be relocated close to Grid Line “N” shown on Attachment A. The location of the wall shall not conflict with the location of the existing heat pump and access to the shut off valves.

2.4.4. Room 2141, Evidence Room, shall be a secured room. Windows shall have interior mounted security bars. Provide a wall assembly that prevents unauthorized entry thru the ceiling plenum space. The east wall shall be located close to Grid line “O” on Attachment A but shall not conflict with the location of the existing heat pump and access to the shut off valves.

2.4.5. Doors: All existing doors shall remain. New interior doors shall be provided with hollow metal frames grouted solid and solid core wood veneer finished as the KTR shall establish from the color boards. Hardware and ratings shall be required for security and in accordance with code requirements. In rooms with sound rated requirements, a solid core wood veneer door shall be installed. If one is not available with the required sound rating or sound performance, use a hollow metal leaf of the required sound rating or sound performance.

2.4.6. Door Locks: All existing doors locks shall remain. Modify or replace door to Room 2133 to be complete with a cypher lock. New door in Hall 2139 shall have a cypher lock. All other doors to have standard keyed locks.

2.4.7. Existing suspended ceiling to remain. Repair and modify existing suspended ceiling grid and replace damaged tiles as needed to accommodate new walls, relocated lighting fixtures, relocated HVAC components, and otherwise damaged ceiling tiles. Include standard suspension system and carrier channels with installation.

2.4.8. Remove existing Public Announcement speakers in Room 2133. Replace ceiling tiles in this area to match existing.

2.4.9. Paint: Paint all walls within the limits of the renovation. KTR shall recommend color scheme in conjunction with the color boards required in 2.4.1.1

2.4.10. Carpet and Cove Base: Install new carpet tiles and cove base within the limits of the renovation, KTR shall recommend color scheme in conjunction with the color boards per section 2.4.1.1

2.4.11. Paint and carpet tiles shall comply with the requirements of the KAFB Installation Facilities Standards (IFS) which references applicable UFCs. Per IFS 1.1.1. Submit two color boards for Architectural Compatibility Review Board (ACRB) and User review and approval within 7 business days of receiving submittal. 2.4.12. All existing openings, as well as openings caused by removals incident to renovation construction activity shall be infilled and patched as required.

2.4.13. Signage: provide all office spaces with signs complying with Department of Defense (DoD) Sign Standard UFC 3-120-01 Interior Signs.

2.5. Modifications and Upgrades

2.5.1. Comm

2.5.1.1. Design, provide, and install CAT6 Comm ports, cables, patch panels and other components within the limits of renovation.

2.5.1.2. Demolish any CAT3 and CAT5 and all unused cabling back to the patch panels in the COMM room while protecting any cables that remain. Demolish all Comm drops and minor components that are unusable. Provide cover plates over demolished Comm jacks.

2.5.1.3. In 2022/2023 a previous Comm project was performed on the 2nd Floor, east wing as shown. (Attachment F). New CAT 6 cables were installed to the existing power poles shown on the plan. A new 48 port CAT6 patch panel was installed in the existing COMM/Server Rm.

The existing above-ceiling tray was utilized.

2.5.1.4. KTR shall perform a survey of the existing Comm system and identify components that are re-usable, including the CAT6 cable and patch panels installed in 2022/2023 as described above. Survey to include Comm drops and other components such as cable trays, patch panels, and switches that are currently in place for review and approval before installation.

2.5.1.5. Submit a layout plan showing the survey performed. Show what components will be demolished. If some of the cables and components are re-usable, show where existing cables and components will be relocated. Submit plan to the PM and CO for review and approval prior to installation.

2.5.1.6. All work, new materials, and reused materials and components shall comply with KAFB Communications Specifications 2023 edition.

2.5.2. Structural

2.5.2.1. Seismic Bracing of Non-Structural components to be reviewed.

2.5.2.2. Seismic load analysis and bracing requirements will be based on a Seismic Design Category C facility.

2.5.2.3. Provide bracing of non-structural components per ASCE 7-16 Chapter

13. This is to include, but not be limited to the following non-structural components:

2.5.2.4. The partition walls are to be constructed per ASCE 7-16 13.5.8. The total weight of the wall system is to not exceed 7 psf. If this load is exceeded the wall will need to be supported by a structural load-carrying element or the slab will require support directly under the new wall. The support loads would then need to be directed to the adjacent beams and/or columns with the adequate connections to support the required reactions. The floor slab of the facility has been previously evaluated by AIMT Engineering Service Inc. to determine the live load capacity in its current as-built condition. Therefore, the walls that are slab supported (not directly bearing on a concrete beam line) are subjected to the weight limitation listed above. Each partition type will need to be submitted for review and approval by PM and CO prior to installation. The submittal will need to include the total weight of the wall system and will need to be listed in the construction documents.

2.5.2.5. Provide seismic bracing of modified fire sprinkler/fire suppression sprinkler head relocations.

2.5.2.6. Provide seismic bracing of Mechanical, Electrical, and Plumbing (MEP) components per ASCE 7-16 Section 13.1.3 and 13.1.4 exemptions.

2.5.2.7. Attach all seismic bracing for the non-structural components and loads, that have been determined by ASCE 7-16 Chapter 13, to the underside of the slab or to the upper third of the concrete beams to distribute the vertical and horizontal component loads into the structural diaphragm.

2.5.2.8. Provide structural review and any corresponding structural warranty for new wall load reinforcements as needed.

2.5.3. Mechanical

2.5.3.1. The Mechanical Engineer of record shall confirm functionality and meet all applicable codes and standards.

2.5.3.2. Ensure that each new space contains an existing heat pump unit.

2.5.3.3. The final wall layout is to be placed such that the existing heat pumps, associated piping and valves do not require modification.

2.5.3.4. In the event a heat pump needs to be relocated, then remove and replace existing heat pump unit and associated ductwork. If a unit is moved toward the interior of the space, slope on the condensate drains should be verified to be adequate for proper operation. Include any replacement tubing, disconnecting/reconnecting regular and low voltage, and clevis hangar assemblies as per Type L at 10' on center.

2.5.3.5. Provide and install Air Supply diffusers, dampers, and return air grilles as needed and to be adjusted for optimum distribution.

2.5.3.6. Since the outside air is coming from a different fan system, a relief vent is required to maintain equal pressurization.

2.5.3.7. Add 6” diameter spin-in outside-air duct with spin-in with balance damper as needed.

2.5.3.8. Conduct and provide a completed test and balance report to the PM and CO.

2.5.3.9. Replace existing thermostats and install additional thermostats to accommodate final wall layout with one per room.

2.5.4. Electrical

2.5.4.1. Building 20202's main electrical service consists of two service transformers: 500 kVA, 208Y/120V and 750 kVA, 480Y/277V. The two main distribution panelboards, DPL and DPH, are in the electrical room on level 1. They are both fully loaded.

2.5.4.2. The existing distribution panelboards in the electrical rooms appear sufficient for the electrical work in the areas/rooms on the second floor. If necessary, rewire the existing feeders and branch circuits, and provide a new subpanel IAW applicable UFC, codes, and standards.

Balance existing and new loads between circuits and phases in the panelboards.

2.5.4.3. Selective lighting and power demolition is required in the areas/rooms.

The DOR is responsible to ensure that electrical demolition does not affect the continued electrical service in the occupied areas of the building.

2.5.4.4. Design and coordinate new and existing circuits. Existing luminaires shall be reused. Relocate luminaires and associated switches as necessary to accommodate new wall layout. Provide new receptacles where necessary to accommodate new wall layout. Relocate existing receptacles as necessary. Other electrical work is required to support mechanical/HVAC and telecom work. There will be no systems furniture.

2.5.4.5. Provide a duplex receptacle for each wall-mounted TV. (Attachment A)

2.5.5. Fire Protection

2.5.5.1. Submit and provide a Life Safety Plan, showing Means of Egress and location of signage. If there is down-time during the work, submit in writing when down time is anticipated (from start date to finish date) 14 days prior to the anticipated start time for approval. Notify the Alarm-Room 3 days prior, and Facility Manager during this time as well. Place a sign on each of the normal entrance and exit doors indicating these dates. Mark off the area being worked on so the Section Supervisor can brief his personnel on an alternate escape route.

2.5.5.2. Design, provide, and install modifications to the existing wet-pipe fire sprinkler system IAW applicable UFC, codes, and standards. Include the addition or realignment of sprinkler heads to accommodate new wall layout.

2.5.6. Design, provide, and install modifications to the existing fire alarm system IAW applicable UFC, codes, and standards. Fire alarm plan shall show any modifications to the existing FACP, pull stations, horns and strobes.

2.5.7. The KTR’s Management shall ensure that all employees, to include any sub-contractor employees, performing work on this effort receive an Operations Security (OPSEC) Awareness Briefing/Training prior to starting work on this effort and the briefing/training must be accomplished prior to entering the work site on day one of the work effort. The government (i.e., Contracting Officer, Unit Security Assistant, Unit OPSEC Coordinator, or 377 Air Base Wing OPSEC/Signature Manager (377 ABW/SMO)), can provide a tailored lesson plan for use by the KTR’s Management to ensure training requirements are met. It is critical that KTR employees fully understand the importance of not discussing work details associated with this work effort, such as: actions or activities observed, the statement of work, as-built or other drawings, etc., in public or outside of the worksite. The KTR’s Management will track all training by employee name and date the training was provided using a spreadsheet or similar document, such as a sign-in roster, and provide a copy to the assigned Contracting Officer.

2.5.8. Schedule of the Work- Before the commencement of construction activities, the User shall remove from the entire facility furniture, fixtures, equipment, and appurtenances, in second floor East Wing area to include: All hallways and rooms except Bathrooms and the Comm. Room as seen in Attachment A. It is planned that all the staff will be relocated to other parts of the building not occupied by construction activities until these activities are complete.

2.5.9. The KTR shall schedule appropriate construction scheduling. The KTR shall update each week plus a twenty-one (21) day look-ahead schedule with the CO and PM. The PM will notify the Facility Manager (FM) who shall notify other Building Users of the upcoming schedule events. Any outages of electrical, comm, water, sewer or gas shall be comprehensively scheduled and coordinated with the Contracting Officer (CO), Project Manager) PM and Facilities Manager (FM). KTR shall fill out an outage form and submit it (21) days in advance. At no time shall the User in adjacent facilities affected by the outage be without such services.

2.5.10. Access- The KTR shall not use the existing elevator during construction activities. The East stairs shall be used for access to the second floor. The building entrance leading to these stairs is located on the South-East corner of the facility. Protect existing stair construction and restore any damage to preconstruction condition at conclusion of construction. Access to the remainder of the facility outside the scope of work of the project is prohibited.

2.5.11. Field Verification- Descriptions of the project scope provided in this SoW, are solely for the purpose of revealing the general nature of the work involved.

The KTR is responsible for providing any or all field investigation needed to confirm existing conditions and ensure the proposed design has accounted for current field conditions.

2.5.12. Warranty – Furnish an overall warranty for 1-year upon Beneficial Occupancy Date (BOD) by the Government.

4. Drawing and Specification Standards

4.1. Contract Drawings

4.1.1. Preliminary Design (65%): submit a preliminary design package that will include, as applicable, deliverables described in Section 4.3 incorporating responses to all Government review comments.

4.1.2. Pre-Final Design (95%): submit a pre-final design package that includes, applicable deliverables described in Section 4.3 incorporating responses to all Government review comments.

4.1.3. Final 100% Design: The final 100% design shall include all SIs, drawings and specifications with all comments and corrections incorporated.

4.1.4. Drawings shall be prepared in AutoCAD.

4.1.5. Full size drawings shall be 24" x 36". Reduced drawings shall be 11" x 17".

4.1.6. All applicable base activity representatives shall sign final drawings as having been reviewed. The licensed design professionals shall stamp, date, and sign all final drawings for their disciplines prior to final base representative signatures.

4.2. Specifications

4.2.1. Specifications shall be delivered at each design submittal stage in accordance with Table 4.1.

4.2.2. Provide Technical Specifications that are definitive and free of ambiguities. Use of the Unified Facilities Guide Specifications (UFGS) system is mandatory in preparing specifications for Design/Build projects. UFGS are available at the Whole Building Design Guide website (http://dod.wbdg.org/).

4.2.3. The UFGS are published only in electronic format and are intended to be used with Specs Intact software. The UFGS Master complies with UFC 1-300-02.

Updated UFGS are posted quarterly in February, May, August, and November.

4.2.4. KTR shall avoid the use of "proprietary type" specifications, except to set an acceptable standard, or when otherwise not available, and shall be re-written in UFGS format prior to inclusion in Contract Documents in accordance with UFC 1-300-02 UFGS Format Standard. When necessary to use a manufacturer's name to describe a type of product, at least three manufacturers shall be named, if possible, and shall include the words "or approved equal".

4.3. Delivery Schedule

4.3.1. Period of Performance

4.3.1.1. The start of design and construction shall be approved by the CO in writing as two (2) separate Notices to Proceed (NTP). Calendar days provided in the table below are “typical” timeframes to accomplish submittals and final inspection/acceptance which shall be considered when developing the overall schedule.

4.3.1.2. Provide 1) an overall schedule to meet the period of performance date/completion date; and 2) a 2-week look ahead with the existing week to show how work is to progress from start to completion. The days provided in the table below are ‘typical’ timeframes to accomplish plans review, submittal reviews and final inspection/acceptance which shall be considered when developing the overall schedule.

4.3.1.3. An acceptable Period of Performance (PoP) for this project shall be 323 calendar days of contract award. The following Table 4.1 shows the timeframe of each government response requirement. The design and build timeframes are 200 and 123 days respectively.

4.3.1.4. Provide 65%, 95%, and 100% design submittals in accordance with Table 4.2. The design analysis shall include investigative and research information. Allow 21 calendar days for Government review and the KTR response following each submittal.

4.3.1.5. Schedule 65% and 95% review conferences at Kirtland AFB after Government comments have been provided to the KTR and the KTR has prepared responses.

4.3.1.6. Upon Government approval of the 65%, and 95% Design, provide the 100% Design Submittal.

4.3.1.7. Upon Government approval of the 100% design, provide the final 100% Design Submittal.

4.3.1.8. The 100% final design shall be manually signed, dated, and sealed by the licensed design professionals responsible for the design. Final SI, drawing and specification set submitted shall be an original signed set;

copies will not be accepted.

5. General Construction Information

5.1. Facility/User Coordination

5.1.1. Project Coordination: All work shall be coordinated with the CO in conjunction with the PM and CE. The PM shall coordinate with the facility user.

5.1.2. Minimize Interference with regular KAFB activities: The KTR shall plan construction activities to minimize interference with the normal activities of Base occupants in the remaining areas of the facility and the neighboring facilities, surrounding streets, and parking areas.

5.1.3. The KTR will not have access to the facilities restrooms. Provide alternate facilities to accommodate the construction workforce.

5.1.4. Owner Occupancy: The project shall be scheduled and executed to minimize conflict and to facilitate User's operations. Changes to the approved schedule shall be vetted through the CO to support minimal operation impact.

5.2. Staging, Storage, and Parking

5.2.1. The Government will designate an area for construction staging and storage.

The exact location will be discussed and determined in the pre-construction meeting. The location shall not impede access for emergency response vehicles and personnel.

5.2.2. Parking shall be in designated parking spaces and in designated parking areas.

Exact areas designations to be discussed during the pre-construction meeting.

Parking will not be allowed on sidewalks or grass areas.

5.3. Availability of Site

5.3.1. Workdays are normally defined as 8-hour duty days of Monday through

Friday, excluding Federal holidays, and family days. Normal hours of operation are 7:00 AM to 4:00 PM. Outages to the facility shall be kept to a minimum, confined to the weekends, and scheduled 30 days in advance.

5.3.2. Work after business hours, on weekends, or Federal holidays will be allowed if requested 10 days in advance to the CO in writing and not interfere with base missions. The PM shall coordinate with the User if approved by the CO.

5.4. Schedule and Planning Requirements

5.4.1. Provide, with each progress payment, a CPM schedule for tracking work progress for the entire project from NTP to closeout documents. The CPM shall detail the project schedule and status using Gantt charts, which shall depict percent complete for each task.

5.4.2. Submit 3-Week look ahead schedule to show how work is to progress from start to completion, if warranted, also submit a drawing to visually to depict areas of work during the same period. 3-week look ahead schedule shall be submitted every 2 weeks unless major changes happen between the 2 weeks that can affect coordination with KAFB. See Spec. Section 01 32 16.00 20 for details.

5.5. Meeting and Conference Requirements

5.5.1. Meeting/Teleconference Support - conduct meetings necessary to support design coordination and each design review. Lead and support weekly meetings and/or teleconferences throughout the construction phase of the contract. prepare and maintain the weekly meeting agenda report which shall include the record of new and old discussion of technical issues, percent complete actual versus planned, and tabular report for Requests for Information (RFIs) and submittals (open and upcoming). Submit the meeting agenda in writing to the CO, PM, and CE prior to the weekly meeting.

Prepare, update, and submit in writing to the CO, PM, and CE on a weekly basis, a three week look ahead schedule to demonstrate construction progression from start to completion.

5.5.2. Prepare minutes for meetings, including teleconferences, design reviews, weekly meetings, and Red Zone Meetings, and distribute minutes to attendees within three (3) working days.

5.5.3. Red Zone Meeting - support “Red Zone Meetings” as part of the closeout process of the project. The “Red Zone” shall commence at 80 % of construction completion. At these meetings, the KTR and the project stakeholders shall discuss, define, and achieve consensus on actions necessary to complete construction and support user occupancy. Meetings at the beginning of the red zone period will be held every two weeks then increase to weekly towards the final month of the project.

5.6. Notification Requirements

5.6.1. The KTR is required to notify the CO and PM of anything they believe is outside their scope of work and/or beyond their control that could impact cost, schedule, or quality. Additionally, the KTR is required to notify the CO and PM of critical issues that may affect the contract performance and/or human health and the environment. The types of issues that require notification include but are not limited to health risks, spills, changes in critical or key personnel. If encountered. immediately stop work, report the discovery to the CO, and PM, and implement the appropriate safety precautions. Field activities shall not resume until clearance is received from the CO. On critical issues, verbal notification shall be made immediately, followed by written notification as soon as practical within the succeeding 48 hours.

5.6.2. Utility outages: Outages require 21-day notice to schedule and shall not take place during hours of operation.

5.7. Permits/ Environmental Requirements

5.7.1. The following permits are applicable, and approvals are required by KAFB prior to construction start, which includes Form 103 Utility Outage Request Form.

5.7.2. Obtain the following additional permits:

• KAFB Landfill Permit

• Hazardous Waste Disposal Permit

• Abatement Permit

• Hot Work Permit- obtained at KAFB Fire Dept.

5.7.3. Other permits may be required on this project, pay particular attention to the requirements specified in the KAFB Checklist of Environmental Requirements for this project, which detail regulations with which those executing the work of the contract shall comply.

5.7.4. If changes are made to the project, or new laws/policies are enacted, there may be additional permits required for construction. Prior to work starting, verify that all current permit requirements are being met.

5.8. Photo Documentation

5.8.1. Prepare digital photo documentation of field activities and progress. Digital photos shall be submitted using a minimum five (5) mega pixel photograph in JPEG format. Provide an index for each set of photographs submitted, identifying the project number, the KTR Team, and a brief description.

Photography of any kind shall be coordinated through the CO, PM, and CE.

The PM shall coordinate with facility Point of Contact (POC).

5.9. Accident Prevention Plan (APP) and Checklist - provide an Accident Prevention Plan (APP) and checklist in accordance with EM 385-1-1.

5.10. Site Protection, Cleanup, Repair or Replace, and Differing Site Conditions

5.10.1. Ensure that all adjacent property, buildings, and contents are protected from dust, dirt, runoff, debris, and other damages resulting from project activities.

5.10.2. Provide plastic sheeting, covering of ventilation, and the use of sticky mats to minimize the transfer of construction dust and debris to the occupied areas of the building during construction. Ensure that all trash and debris shall be disposed of daily.

5.10.3. Blockages or obstructions of any doorway or building egress shall be coordinated with the PM.

5.10.4. No food shall be prepared on the facility premises. Coordinate with the user to designate a location outside of the work area where breaks will be taken.

5.10.5. Replace or Repair: replace or repair to match original existing conditions, all existing adjacent finished surfaces, utilities, equipment, products, vegetation, and structures or parts thereof that have been damaged or removed to perform the work. Inform the CO, PM, and CE in sufficient time to allow scheduling inspection of all work prior to being enclosed.

5.10.6. Daily Cleanup: Ensure that all non-recyclable trash, debris, refuse, garbage, etc. is disposed in a safe and legal manner. The KAFB landfill will be available for disposal of non-hazardous Construction and Demolition waste.

5.10.7. Final cleanup: Ensure that the project site is completely clean before the project final walk-through. All new materials shall be properly cleaned and shall be free of scuffs, dirt, and dust. Areas surrounding the construction site shall also be cleaned of any dirt or dust. All unused construction materials shall be removed from the site.

5.11. DD1354

5.11.1. KTR is required to fill out a DD1354 for real property records and a component and subcomponent list for new and previous equipment, to include, but not limited to model, serial numbers, and manufacture specification, locations, and before and after pictures. KTR shall contact real property department for any questions and concerns. DD1354 and component list are to be completed, submitted, and accepted before any pre-finals and finals are scheduled.

There can be only one final version of a DD Form 1354 for a single construction project. The KTR is to update a draft version on a daily or semi-daily time frame. The KTR may simply have an adjusted draft version with a later date prepared (block 2) and the same serial number (block 4). However, the KTR may have multiple valid interim DD Forms 1354 for a phased turnover of facilities, each with its own serial number (block 4). This will be reviewed by the Government at the 65% completion of the construction project.

FINAL Form DD 1354 to include Approved Final Invoice with Checklist including Key Supporting Documents shall be submitted with approval of final invoice.
