PRESENT/PAST PERFORMANCE QUESTIONNAIRE
Present/Past Performance Questionnaire for Request for Proposal (RFP) FA8773-08-R-0082 (This questionnaire may be used on more than one solicitation within the office).
1. The company (also referred to as the “offeror” or “contractor”) listed below is responding to a RFP for Operations and Maintenance (O&M) of Base Telecommunications Systems (BTS) (also known as the base telephone switch and associated infrastructure). The scope of this work entails providing all personnel, equipment, parts, material, tools, for operation and maintenance on the switch system(s). This also includes other equipment/cabling connected to the voice switching system. The contract encompasses changing technology and mission requirements such as hardware and software system upgrades, configurations and other services subject to the Service Contract Act and/or Davis Bacon Act.
2. Present/past performance is a major evaluation factor as awardees are determined (after technical acceptability) by the price/past-performance trade-off technique (so price is not the only applicable factor in determining award). We have requested all offerors provide us with a list of references that can provide information about their present/past performance on similar contracts. The offeror has identified you as the point of contact for the contract listed below. Your assessment will be very helpful to our evaluation team.
4. Thank you for your prompt response and assistance in completing this questionnaire. Please complete and return the attached questionnaire before 3:00PM CST, 28 May 2008 to:
