SUBJECT: Solicitation FA8601-17-R-0025, Civil Engineering (CE) Facility Maintenance and Support, Past Performance Questionnaire
1. The Air Force Life Cycle Management, Operational Contracting Directorate, is in the process of selecting a contractor for the CE Facility Maintenance and Support acquisition at Wright-Patterson AFB.
2. One of the Evaluation Factors utilized in this source selection is Past Performance, which is a strong indicator of the offeror’s ability to perform on the proposed effort. We depend on information received from agencies such as yours, which have had firsthand experience with an offeror, for the evaluation of the offeror's performance on those contracts.
3. Our areas of interest with respect to evaluating Past Performance are summarized in the enclosed questionnaire. Our schedule is extremely tight and we need your written response no later than 04:30 PM EST on 4 August 2017. This schedule will allow us sufficient time to analyze the data. We thank you in advance for your participation!
4. The questionnaire may be filled out by hand and scanned/emailed or electronically filled out and emailed to jason.wilkin@us.af.mil AND scott.macdonald.2@us.af.mil. Please call TSgt Jason Wilkin at 937-522-4509 or myself at 937-522-4599 for any questions.
5. Your completed questionnaire will become a part of the official Source Selection records. Your help is greatly appreciated and your prompt response will be one of the keys to the successful and timely completion of this Source Selection.

Provide supporting information for each response in the space provided. Attach additional pages if more space is needed. The performance rating scale is defined as follows:
CODE PERFORMANCE LEVEL
B BLUE/EXCEPTIONAL - The contractor’s performance meets contractual requirements and exceeds many (requirements) to the Government’s benefit. The contractual performance was accomplished with few minor problems for which corrective actions taken by the contractor were highly effective.
P PURPLE/VERY GOOD- The contractor’s performance meets contractual requirements and exceeds some (requirements) to the Government’s benefit. The contractual performance was accomplished with some minor problems for which corrective actions taken by the contractor were effective.
G GREEN/SATISFACTORY – The contractor’s performance meets contractual requirements. The contractual performance contained some minor problems for which corrective actions taken by the contractor appear or were satisfactory.
Y YELLOW/MARGINAL – Performance does not meet some contractual requirements. The contractual performance reflects a serious problem for which the contractor has not yet identified corrective actions or the contractor’s proposed actions appear only marginally effective or were not fully implemented.
R RED/UNSATISFACTORY – Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance contains serious problem(s) for which the contractor’s corrective actions appear or were ineffective.
