About this file
This is a mandatory Hotel & Facility Coordination Attestation form (Amendment 1) that serves as a technical acceptability requirement for RFQ FA800326Q0012, the FY27 Nursing Practice Oversight Course (NPOC). The form requires dual signatures from both an authorized Hotel Polaris representative and the bidding contractor, confirming coordination of audiovisual equipment installation and facility logistics for the event scheduled February 22-24, 2027, at Hotel Polaris in Colorado Springs, Colorado. Failure to submit a completed, signed attestation results in an automatic "Technically Unacceptable" rating and disqualification from award consideration.
The Hotel Polaris representative must attest to five key commitments: (1) providing full loading dock and ballroom access beginning at 1800 hours on February 21, 2027, with all equipment installed and tested by 0730 hours on February 22, 2027; (2) hotel responsibility for physical installation and removal of airwall partitions for breakout rooms on February 23; (3) compatibility of ballroom power-drop interfaces with the contractor's equipment specifications; (4) coordination of audiovisual services with the in-house provider, with explicit agreement that no additional facility access fees, outside contractor surcharges, or patch-in surcharges shall be assessed to the contractor, government, or travelers, while permitting contractors to bring self-contained powered speaker systems and projection packages; and (5) acceptance of the contractor's cable safety plan using commercial rubber cable ramps and heavy-duty gaffer tape as compliant with safety regulations. The contractor must certify equipment compatibility with the hotel infrastructure and absorb all coordination costs within their firm-fixed-price quotation.

HOTEL REPRESENTATIVE ATTESTATION:
This section must be completed and signed by an authorized Hotel Polaris Event Manager, Banquet Manager, or Technical Director.
I, the undersigned authorized representative of Hotel Polaris, hereby attest and confirm the following statements regarding the proposed coordination with [Insert Bidding Contractor Name] (hereafter referred to as the "Contractor") for the 2027 NPOC event:
1. Facility and Loading Dock Access: Hotel Polaris agrees to provide the Contractor with full, unrestricted access to the facility's loading docks, and the assigned ballrooms starting no later than 1800 hours on Sunday, 21 February 2027 (Day 0), to facilitate the complete installation and testing of all leased audiovisual equipment by the mandated 0730 22 February 2027 deadline..
2. Space and Airwall Coordination: Hotel Polaris confirms that the hotel staff will be responsible for the physical installation, operation, and removal of the ballroom airwalls to partition the space into Breakout Room 1 and Breakout Room 2 for the Day 2 (23 February) sessions, and that the hotel has coordinated the physical layout with the Contractor's proposed equipment placement.
3. Power and Technical Compatibility: The hotel’s technical staff has reviewed the Contractor's equipment layout and power-consumption requirements. The hotel confirms that the ballroom's built-in power-drop interfaces are fully compatible with the Contractor’s equipment and that the hotel will provide the necessary power connections to support the Contractor's layout at prevailing rates.
4. In accordance with the terms established for this Government event, Hotel Polaris and the Contractor agree that audiovisual services and outside vendor access shall be coordinated in advance with Hotel Polaris and its in-house audiovisual provider. The Contractor acknowledges the in-house provider’s exclusivity regarding Internet, Rigging, and house Power, and agrees to the established production supervision and liaison fee structure. Following operational and schedule coordination between Hotel Polaris, the in-house provider, and the Contractor, Hotel Polaris agrees that no additional fees, post-coordination overages, "Outside Contractor Surcharges," "Facility Access Fees," or "Patch-in Surcharges" shall be assessed to the Contractor, the Government, or individual travelers for utilizing an outside audiovisual leasing vendor. The Contractor is permitted to bring in and operate their own self-contained powered speaker systems and projection packages, and no further auxiliary hotel or vendor fees will be charged beyond the agreed, coordinated liaison services.
5. Cabling Safety: The hotel has reviewed the Contractor’s cable safety plan and agrees that the use of commercial rubber cable ramps and heavy-duty gaffer tape is sufficient to meet hotel and local safety regulations for all public corridors and meeting spaces.
