About this file
This is a mandatory Hotel & Facility Coordination Attestation form that serves as a technical acceptability requirement for quoters responding to RFQ FA800326Q0012 for the FY27 Nursing Practice Oversight Course (NPOC). The attestation must be completed and signed by both an authorized Hotel Polaris representative and the bidding contractor, then submitted as part of File 2 (Technical Quotation) prior to the submission deadline. Failure to submit a completed, signed form results in an automatic "Technically Unacceptable" rating and exclusion from award consideration. The form confirms critical facility coordination details for the event scheduled February 21-24, 2027, at Hotel Polaris in Colorado Springs, Colorado.
The attestation requires Hotel Polaris to confirm: full unrestricted access to loading docks, freight elevators, and ballrooms beginning no later than 0800 hours on February 21, 2027, with all equipment installation and testing completed by 1800 hours; hotel responsibility for installation and removal of airwalls to create breakout rooms; compatibility of built-in power-drop interfaces with contractor equipment and provision of necessary power connections; waiver of all outside contractor surcharges, liaison fees, patch-in surcharges, or facility access fees for audiovisual vendors; and approval of the contractor's cable safety plan utilizing commercial rubber ramps and gaffer tape. The contractor must certify that its proposed equipment manifest is 100% compatible with hotel infrastructure and that all coordination costs are absorbed within the firm-fixed-price quotation.
