About this file
This past performance questionnaire requests feedback on an offeror's previous maintenance and repair of active vehicle barriers. The Air Force seeks to award a contract for vehicle barrier maintenance and repair services worldwide. The past performance questionnaire collects data on an offeror's management, preventative maintenance, emergency service calls, routine service calls, and overall performance on prior vehicle barrier contracts. Customers are asked to provide ratings in these areas using an exceptional, very good, satisfactory, marginal, or unsatisfactory scale. The completed questionnaire will be used to evaluate offerors' ability to successfully perform the work required by the new contract.

CUSTOMER PAST PERFORMANCE QUESTIONNAIRE (PPQ)
The Department of the Air Force, Air Force Installation Contracting Agency (AFICA), 771st Enterprise Sourcing Squadron (771 ESS) located at Wright-Patterson Air Force Base (WPAFB), Ohio solicits your input regarding an Offeror, as self-identified in SECTION I.A. below, that has previously provided or is currently providing active vehicle barrier maintenance and repair (AVB M&R) services to your organization. The following PPQ is a part of the Offeror's proposal for a contract with the Air Force to provide Vehicle Barriers Maintenance and Repair (VB M&R) Services on an Air Force-wide basis. One of the considerations in the Government's proposal evaluation is the verification of an Offeror's past performance on efforts/contracts which reflect the Offeror's ability to perform on the proposed effort. The Government depends on information received from organizations such as yours which have had first-hand experience with the Offeror, for the evaluation of the Offeror’s performance on the submitted efforts/contracts. Offerors were required to fill out SECTION I of this PPQ then send the PPQ to the identified point of contact (POC) for the POC to fill out SECTION II of this PPQ. As an identified POC for one of these efforts/contracts, your feedback is very important to this program and we appreciate your willingness to participate by providing the requested information in SECTION II below. The completed PPQ will become part of the official Source Selection record. In order to meet the acquisition milestones, we request your written response no later than five (5) calendar days after your receipt of this PPQ. Please send the completed PPQ to the Government POCs indicated on the last page of this PPQ (not to the Offeror). Your assistance is greatly appreciated and your prompt response will be one of the keys to the successful and timely completion of this Source Selection.
The questions within this PPQ pertain to the Offeror’s record of past (within three years) and current performance related to maintenance and repair of active (versus passive) vehicle barrier systems. The information you provide will be used in the awarding of an Air Force-wide contract; therefore, it is important that your information be as factual and accurate as possible. The past performance evaluation considers each Offeror’s demonstrated recent and relevant record of performance in supplying AVB M&R services that meet the solicitation requirements. Please provide examples and/or explanations, which can include Federal, State, or Local Government and private industry for efforts similar to this Government requirement (use additional pages if necessary). In the case of an Offeror without a record of relevant past performance or for whom information on past performance is not available, the Offeror may not be evaluated favorably or unfavorably on past performance.
The following adjectival ratings shall be used in your response:
