About this file

This is a Past Performance Questionnaire (PPQ) template designed to collect contractor performance data from customers for evaluation purposes in the FY 27 A-E IDIQ solicitation (FA561326R0004). The questionnaire must be completed electronically and submitted to the 700th Contracting Squadron by October 8, 2026. Responses will be used to evaluate offerors' past performance on Architect-Engineer services contracts and become source selection information protected under FAR 2.101 and 3.104. The PPQ is not returned to the contractor but submitted directly by the customer/reference entity.

The questionnaire evaluates contractor performance across multiple service categories including general architectural services, interior design, landscape architecture, civil/structural/mechanical/electrical engineering, environmental engineering, cyber security, fire protection, geotechnical services, project management, cost estimation, construction inspection, SCIF planning and design, and surveying. Performance is rated using a five-point scale: Exceptional, Very Good, Satisfactory, Marginal, or Unsatisfactory. The template collects basic contract information (contractor name, project title, contract number, contract type and price), timeline compliance data, scope of work performed, and detailed ratings across 32 evaluation factors covering technical competence, design quality, schedule management, cost control, coordination abilities, and overall performance. Customers are also asked whether they would rehire the A-E firm and to provide narrative explanations for any Marginal or Unsatisfactory ratings.

PAST PERFORMANCE QUESTIONNAIRE (PPQ)

***Please DO NOT send the completed PPQ back to the contractor.

This questionnaire is provided to assist the 700th Contracting Squadron (700 CONS) in the performance evaluation of offerors pertaining to the solicitation for the fiscal year 2027 Architect and Engineering IDIQ contracts which will result in the award of a federal contract. Therefore, it is important that your information be as factual, accurate and as complete as possible to preclude the need for follow-up queries by the evaluators.

The questionnaire must be filled out electronically and emailed to sibel.wright@us.af.mil, fatima.berlitz.de@us.af.mil, and jonathan.brekken@us.af.mil no later than 08 October 2026. Please email if you have any questions. Your completed questionnaire, once received, will become Source Selection Information in accordance with RFO 2.101 and RFO 3.104-4 and shall not be disclosed to unauthorized persons.

Your assistance is greatly appreciated, and your prompt response will be one of the keys to the successful and timely completion of this Source Selection.
