Section J, Attachment 11 EEO Clearance
Equal Employment Opportunity (EEO) Clearance Request
In accordance with FAR 22.805, the Contracting Officer must request preaward clearance from the appropriate Office of Federal Contract Compliance Programs (OFCCP) for this acquisition.
All offerors shall provide the information listed below to assist with the clearance request process. Offeror’s format is acceptable, and it must be in compliance with Section L of this solicitation. If the offeror is listed in the OFCCP’s National Preaward Registry, the offeror shall submit verification of registration in addition to the information requested below.
Provide the following information for the prospective contractor and any corporate affiliate at which work is to be performed:
1. CAGE Code
2. DUNS Number
3. Name
4. Street address, including City, State and Zip. If outside of the United States, specify the country.
5. Telephone number
Provide the following information for each proposed first-tier subcontractor(s) with a proposed subcontract estimated at $10 million or more (including all options):
1. CAGE Code
2. DUNS Number
3. Name
4. Street address, including City, State and Zip. If outside of the United States, specify the country.
5. Telephone number
6. Estimated subcontract value (including all options)
If the offeror has no proposed subcontracts estimated at $10 million or more (including all options), provide a statement to that effect.
Include a statement to indicate whether the contractor and first-tier subcontractors have previously held any Government contracts or subcontracts.
Examples:
Yes, the contractor has previously held Government contracts. However, the first-tier subcontractors have not previously held Government contracts.
Yes, both the contractor and the first-tier subcontractors have previously held Government contracts.
No, neither the contractor nor the first-tier subcontractors have previously held Government contracts.
